Bug 300030: Move intrinsic width computation out of nsIFrame::Reflow and into its own methods on nsIFrame. Replace reflow reasons, types, and commands with dirty bits/notifications. Thanks to bzbarsky for almost all of the HTML form controls (mozilla/layout/forms) changes, and many others for help testing and patching. For detailed commit logs, see REFLOW_YYYYMMDD_BRANCH, where YYYYMMDD is one of 20061031, 20060830, 20060603, 20060302, 20060119, 20051011, 20050804, 20050429, 20050315, 20050111, and 20041213.

Rename {nsIDocument,nsIDocumentObserver,nsIFrame}::ContentChanged to CharacterDataChanged, since that's the only thing it's used for. Replace its nsISupports* parameter with a PRBool, since we don't need to allocate an object to pass a boolean parameter. Remove some (but probably not all) unneeded implementations on frames whose content is never a text node. b=234960 r+sr=jst

Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. area, and not the padding area as per the CSS 2 Errata, CSS2.1 and CSS3. Also, implementing the CSS3 'background-clip'
and 'background-origin' properties (currently with -moz- prefixes) to control this behavior.
