Bug 1350377 - Remove `getPlacesInfo` and change associated files and tests, r=mak

Files which make use of `getPlacesInfo` have been replaced with `History.fetch`.
The code for `GetPlacesInfo` has been deleted from the cpp and idl files.
The test for `getPlacesInfo` has been suitably rewritten and moved alongside the
other History.jsm tests.
There were 2 places where the fact that `getPlacesInfo` takes an array as opposed
to a single uri mattered, in `test_getPlacesInfo.js` and `test_refresh_firefox.py`.

Bug 1359203 Part 1 Replace AddonConstants with AddonSettings

AddonConstants duplicated AppConstants, but having it just include
the build-time constants meant that the logic for whether to enforce
signing was repeated in a few places (and it was incorrect in
the front-end code!). Now we centralize that logic in AddonSettings.jsm

Bug 1355252 - Don't assume default codecs for MP4 and WebM in EME MediaCapabilities r=jwwang

Step 10 of EME's "Get Supported Capabilities for Audio/Video Type" algorithm
says we can assume default codecs only if a container normatively implies a
specific set of codec and codec constraints. Our code assumes that WebM implies
Vorbis/VP8 and MP4 implies AAC/H.264, but those aren't actually normatively
required by either of these containers' specifications. So we shouldn't assume
these containers imply those codecs.

This change means that any related http pages driven through content (window.open, links, etc.) will continue to be loaded in the file content process.
Same-origin loads via the UI will also remain in the file content process.
Cross-origin loads via the UI will cause a process switch.
History navigation will stay in the process, if it was originally loaded in that process.
