Andreas Pehrson 7bdf7c09ae Bug 1514158 - Bring MediaRecorder to spec on new bitrate handling. r=bryce 
This brings the MediaRecorder constructor and start() to spec on most parts
except mime type handling (bug 1512175). The flow is also improved to better
follow along in the algorithms of the spec.

This renames the following (in alphabetical order, non-exhaustive):

AudioCaptureStream -> AudioCaptureTrack
AudioNodeStream -> AudioNodeTrack
AudioNodeExternalInputStream -> AudioNodeExternalInputTrack
DirectMediaStreamTrackListener -> DirectMediaTrackListener
MediaStream -> MediaTrack
  - Note that there's also dom::MediaTrack. Namespaces differentiate them.
MediaStreamGraph -> MediaTrackGraph
MediaStreamTrackListener -> MediaTrackListener
MSG -> MTG (in comments)
ProcessedMediaStream -> ProcessedMediaTrack
SharedDummyStream -> SharedDummyTrack
SourceMediaStream -> SourceMediaTrack
StreamTime -> TrackTime
TrackUnionStream -> ForwardedInputTrack
  - Because this no longer takes a union of anything, but only a single track
    as input.

Other minor classes, members and comments have been updated to reflect these

Emilio Cobos Álvarez d2ed260822 Bug 1517241 - Rename nsIDocument to mozilla::dom::Document. r=smaug 
Summary: Really sorry for the size of the patch. It's mostly automatic
s/nsIDocument/Document/ but I had to fix up in a bunch of places manually to
add the right namespacing and such.

Overall it's not a very interesting patch I think.

nsDocument.cpp turns into Document.cpp, nsIDocument.h into Document.h and
nsIDocumentInlines.h into DocumentInlines.h.

I also changed a bunch of nsCOMPtr usage to RefPtr, but not all of it.

While fixing up some of the bits I also removed some unneeded OwnerDoc() null
checks and such, but I didn't do anything riskier than that.

Bumsik Kim c3572e1952 Bug 1458538 - Add pause and resume events for MediaRecorder. r=jya,bzbarsky 
This resolves the two problems of MediaRecorder:

1. MediaRecorder does not fire pause/resume events when the corresponding methods are called, as mentioned in D7910.
2. The WebIDL for MediaRecorder does not specify onpause/onresume event handler attributes neither.

DispatchSimpleEvent() is used because there are no event attributes needed.

Andreas Pehrson ca0438115d Bug 1296531 - Refactor MediaRecorder. r=jesup,SingingTree 
The main purpose of this patch is to make the TrackEncoders run in a TaskQueue
to avoid multi-threaded access to members, and to change to track listeners to
allow for recording all kinds of tracks (the description of bug 1296531).

Bryce Van Dyk 9fec9bfac9 Bug 1388909 - Rework MediaRecorder to tranisition to inactive upon error. r=pehrsons 
The MediaRecorder should transition to a 'inactive' state immediately upon
error. This changeset updates the recorder to do so. Previously the recorder
would fire an error event before transitioning, resuling in the state still
being 'recording' for handling of the the thrown error.

these exceptions must be created at the time of the operation which led to the
