Add support for using gcc's visibility pragma to treat all function and method declarations as "hidden" unless declared otherwise. Create wrappers for system headers that cause them to be treated as default-visibility declarations. Bug 224829 - docshell should not require non-gecko interface nsIBrowserHistory. This was fixed by introducing a new interface nsIGlobalHistory2 to replace the current nsIGlobalHistory. There are two-way adapters so that the frozen interface still works. r=biesi sr=bz a=mscott for toolkit/ changes.

Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell.
