sspitzer%netscape.com 8c7ba70734 fix for #65704. offline status indicators aren't changing. 
now the offline indicators in multiple browser, mailnews, msg compose, editor windows will
update when we go offline.  also, allow windows to 'jump in' before the offline state
changes so they can do certain tasks and allow the user to cancel the state change, if needed.
(important for mailnews).  there is still some work to do, but this is the bulk of it.

moz_logging = 0  (mac)
to turn it off globally.

These changes remove nsIDocumentLoaderObserver.idl from the build, and all of it's implementations and registrations have been moved over to nsIWebProgressListener.idl and nsIWebProgress.idl respectively. there are two (78762, 78760) mailnews printing bugs that I overturned *before* landing this. No need for m_hdrs because we already have m_keys and can fetch the hdrs.
m_keys and m_folders are rebuilt on a sort operation.
75761 The problem is with first message for local folders because they have
m_messageKey as 0 and we were caching msg headers. This was causing problems
for search window because results are from multiple folders. fix suggested by david.
