Fix too-much-overflow regression by making border-collapse cells store the full width of the border rather than just the inside half of it. Also make BCPixelSize 16-bit instead of 8-bit and use it in more places, and a little other cleanup. b=286794 r=bernd sr=roc a=brendan

Remove nsIStyleContext (bug 114713), r/sr=dbaron. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. Correcting our background painting code per the latest CSS specs. By default, backgrounds should be painted to the border

area, and not the padding area as per the CSS 2 Errata, CSS2.1 and CSS3. Also, implementing the CSS3 'background-clip'
and 'background-origin' properties (currently with -moz- prefixes) to control this behavior.

Cells are only notified that they should observe a percent height element if the element is inside the table's cell.
