Bug 78510. Maintain a per-document hashmap from URI-spec-hash to set of pointers to content elements that are links to those URIs whose state has been queried by the style system. Rename {nsIDocument,nsIDocumentObserver,nsIFrame}::ContentChanged to CharacterDataChanged, since that's the only thing it's used for. Replace its nsISupports* parameter with a PRBool, since we don't need to allocate an object to pass a boolean parameter. Remove some (but probably not all) unneeded implementations on frames whose content is never a text node. b=234960 r+sr=jst

Final patch for bug 195350. Make XUL use new nsAttrAndChildren class. Kill nsXULAttributeValue and nsXULAttributes in favour of nsAttrValue resp. nsDOMAttributeMap. Should fix a few bugs, improve performance and reduce bloat.
