Eliminate nsIStyleSet as an XPCOM interface, making the implementation internal to layout. Frame constructor ownership moves to the pres shell, and the style set methods that forward to the frame constructor are gone. Rewrote style set sheet add/remove to reduce code size. Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. This partially backs out the previous patch. We no longer need to set/get anything in the nsIPrintOptions.

What this does is:
1) Reflows the first time constrained and remembers mOverflowArea.XMost() of
the HTML frame and then later uses that for the calculation of the ratio.
2) Renames some variables
3) This also changes the the maximum STF ratio from 0.5 to 0.3 to match the
dropdown in PP

The problem here is the "edge" prefs are now preficed with the printer name,

and the nsPrintOptions know how to convert the printer name, so we need to add

This patch does the following:

1) Introduces a new public interface nsIPrintSettingService which emables consumers to create PS objects whithout a nsIWebBrowserPrint interface
2) The implementation is nsPrintOptionsImpl which now impls the private interface nsIPrintOptions and the public iface nsIPrintSettingsService
3) Several methods/attrs have been moved from nsIWebBrowserPrint iface to nsIPrintSettingsService
4) The CIDs for nsIPrintOptions and nsIPrinSettingsService are now the same.
5) There is no longer a CID "@mozilla.org/gfx/printoptions;1"
6) nsIWebBrowserPrint still has the "globalPrintSettings" attr so if you already have an nsIWebBrowserPrint you don't have to create the PS Service to get it.
7) A bug has been filed to rename the imlp of nsPrintOptionsImpl to nsPrintSettingsService.

The problem is that on windows widget can only be positioned to a y coord of

32767. This ends up being about 32 pages. The nsPageFrame's view creates a
widget for clipping absolutely positioned content and plugins.
This fix will allow a document to have up to 32 pages of clipping after that it
stops creating the widget. The down side is that any absolutely positioned
content or plugins beyond page 32 will not be clippe din print preview.
Also, fixed a leaked widget

5) Fixed a couple of minor issue with Canceling and cancelling mail printings

Fixes for printing bugs 84017 and 89151. The fix for bug 84017 removes the need for a resize reflow to get form frame values to print. The restoration of these values into the new frame model is done as part of the initial reflow. The fix for bug 89151 gets rid of the extra frame model built and forces printed images to come out of the decoded image cache. r=darinf, heikki, sr=rpotts

This checkin enables mozilla to support the printing of selection, the printing of page ranges, and

the printing of headers and footers.
Printing of selection is implemented by the frames figuring out if they are in the selection and painting
if they or not they they don't paint. This also only allows the printing of the first page of
selections, alothough it is well documented where this is implemeted so it can be removed.
