Hopefully fixed some cast complaints smaug had (presumably they relate to
the comments "I doubt this cast pair is correct").
Tried to clean up the REQUIRES a /bit/ in an effort to address the lock
problem.

The only method we needed was RemoveAllPages, which i've moved to be accessible via nsIObserver,
it's a private communication from the specific history client.

Variable length arrays is a C99 ism, in gecko we use nsAutoArrayPtr.

Instead, change nsDocShell to make the content viewer on demand, and go ahead and pass the activate/deactivate onto the dom window. This fixes a serious focus bug in Camino.

  nsIWebProgressListener2, and give them 64-bit progress values
- make nsIHelperAppLauncher's listener setWebProgressListener function take an
  nsIWebProgressListener2 interface

And fix up all implementations according to that.
