Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. This removes all call-sites I can currently fix. Tomorrow I'll try to get someone to checkin my changes to security/ and I'll get some help with the Netscape side of things.
nsString::GetUnicode()'s final death-blow will be dealt soon. Please keep this in mind as you add new code :-)

Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load.
