- convert atoms to UTF8 because 99% of the time, that is what they are used for
- add support for static atoms, where the strings are declared in a large const array and not copied into the heap

Remove nsIStyleContext (bug 114713), r/sr=dbaron. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. Turn on nsRefPtr (bug 104346), and change nsDerivedSafe to not declare AddRef and Release explicitly virtual, r=jag, sr=dbaron.

Remove ClearUndisplayedContentMap hack from ReconstructStyleData (expanded for bug 118014; see bug 136728). Fix multiple content inserted notifications from XBL (the real cause of bug 118014) by checking the undisplayed content map in addition to the primary frame map.
