Bug 171549: help: discourage page breaks inside headings. r=IanN

This is an updated patch for bug 171549, which adds the same page
break-related properties as in Brant's patch.
(Note: It seems that "page-break-after: avoid;" is currently not
supported and page breaking after headings will still happen.)

Bug 588191 - Update bookmarks help. r=frg

- Rename "Personal Toolbar" to "Bookmarks Toolbar" in help (the
toolbar was renamed in bug 588807).
- Reword instructions that refer to UI elements of the old Bookmarks
Manager.
- Remove text on designating folders as "New Bookmarks Folder" and
"[Bookmarks] Toolbar Folder", as these features do not seem
to exist anymore.
- Rewrite help for some features that have changed: search,
import/export and bookmarking groups of open tabs.
- Removes the part that says the bookmarks.html format is used
internally (bug 1288184).
- Remove "(not in a folder)" from the tip about using "Delete" in the
context menu, as this now works for such bookmarks.
- Remove the reference to "Groupmark" as I don't think this word is
used anywhere in the current UI.
