Support listing endpoints and published nodes with namespace information using OPC server endpoint
Since the existing GetPublishedNodes method call only returns NodeId, whose namespace index
lives in a different server, add a new opc method that calls into the existing and tested iot hub method to expose the new list format.

Support IoTEdgeHubDev simulator as host

EdgeHubConnectionString environment variable is set by IoTEdgeHubDev tooling
to pass CS to a module. Without it publisher fails to start under simulation.
Fix: Check for environment variable to be present and thus declare it to be
running int edge context.
The ModuleClient.CreateFromEnvironmentVariable will first check the
EdgeHubConnectionString environment variable before using the individual
variables thus the simple check for it to be defined will cause the module
client to be used. Thus no more work needed.

Change the way how the worker task for session creation and node monitoring is started and signaled. This fixes an issue if a high number of nodes is published via IoTHub direct methods, which earlier has blocked execution of the app.
