Alison Schofield 9c9bd593f3 scsi: pmcraid: replace struct timeval with ktime_get_real_seconds()
Replace the use of struct timeval and do_gettimeofday() with
64 bit ktime_get_real_seconds. Prevents 32-bit type overflow
in year 2038 on 32-bit systems.

Driver was using the seconds portion of struct timeval (.tv_secs)
to pass a millseconds timestamp to the firmware. This change maintains
that same behavior using ktime_get_real_seconds.

The structure used to pass the timestamp to firmware is 48 bits and
works fine as long as the top 16 bits are zero and they will be zero
for a long time..ie. thousands of years.

Alternative Change:  Add sub second granularity to timestamp

As noted above, the driver only used the seconds portion of timeval,
ignores the microseconds portion, and by multiplying by 1000 effectively
does a <<10 and always writes zero into timestamp[0].

The alternative change would pass all the bits to the firmware:

Christoph Hellwig 64d513ac31 scsi: use host wide tags by default
This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

Johannes Berg 053c095a82 netlink: make nlmsg_end() and genlmsg_end() void
Contrary to common expectations for an "int" return, these functions
return only a positive value -- if used correctly they cannot even
return 0 because the message header will necessarily be in the skb.

This makes the very common pattern of

  if (genlmsg_end(...) < 0) { ... }

be a whole bunch of dead code. Many places also simply do

  return nlmsg_end(...);

and the caller is expected to deal with it.

This also commonly (at least for me) causes errors, because it is very
common to write

  if (my_function(...))
    /* error condition */

and if my_function() does "return nlmsg_end()" this is of course wrong.

Additionally, there's not a single place in the kernel that actually
needs the message length returned, and if anyone needs it later then
it'll be very easy to just use skb->len there.

Remove this, and make the functions void. This removes a bunch of dead
code as described above. The patch adds lines because I did

I could have preserved all the function's return values by returning
skb->len, but instead I've audited all the places calling the affected
functions and found that none cared. A few places actually compared
the return value with <= 0 in dump functionality, but that could just
be changed to < 0 with no change in behaviour, so I opted for the more
efficient version.

One instance of the error I've made numerous times now is also present
in net/phonet/pn_netlink.c in the route_dumpit() function - it didn't
check for <0 or <=0 and thus broke out of the loop every single time.
I've preserved this since it will (I think) have caused the messages to
userspace to be formatted differently with just a single message for
every SKB returned to userspace. It's possible that this isn't needed
for the tools that actually use this, but I don't even know what they
are so couldn't test that changing this behaviour would be acceptable.

Christoph Hellwig efc3c1df5f scsi: remove ->change_queue_type method
Since we got rid of ordered tag support in 2010 the prime use case of
switching on and off ordered tags has been obsolete.  The other function
of enabling/disabling tagging entirely has only been correctly implemented
by the 53c700 driver and isn't generally useful.

Christoph Hellwig c8b09f6fb6 scsi: don't set tagging state from scsi_adjust_queue_depth
Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth.  For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway.  The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Christoph Hellwig 2ecb204d07 scsi: always assign block layer tags if enabled
Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq.  This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Christoph Hellwig 5066863337 scsi: remove abuses of scsi_populate_tag
Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.

Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

Martin K. Petersen 54b2b50c20 [SCSI] Disable WRITE SAME for RAID and virtual host adapter drivers
Some host adapters do not pass commands through to the target disk
directly. Instead they provide an emulated target which may or may not
accurately report its capabilities. In some cases the physical device
characteristics are reported even when the host adapter is processing
commands on the device's behalf. This can lead to adapter firmware hangs
or excessive I/O errors.

This patch disables WRITE SAME for devices connected to host adapters
that provide an emulated target. Driver writers can disable WRITE SAME
by setting the no_write_same flag in the host adapter template.

Johannes Berg 5e53e689b7 genetlink/pmcraid: use proper genetlink multicast API
The pmcraid driver is abusing the genetlink API and is using its
family ID as the multicast group ID, which is invalid and may
belong to somebody else (and likely will.)

Make it use the correct API, but since this may already be used
as-is by userspace, reserve a family ID for this code and also
reserve that group ID to not break userspace assumptions.

My previous patch broke event delivery in the driver as I missed
that it wasn't using the right API and forgot to update it later
in my series.

While changing this, I noticed that the genetlink code could use
the static group ID instead of a strcmp(), so also do that for
the VFS_DQUOT family.

    We have to retain these fixed IDs to keep existing userland tools
    working, but reserve them so that other multicast groups used by

    management for genetlink operations and multicast groups.

    drivers that simply do not validate that the hwtstamp_config value

CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*

Tejun Heo 43829731dd workqueue: deprecate flush[_delayed]_work_sync()
flush[_delayed]_work_sync() are now spurious.  Mark them deprecated
and convert all users to flush[_delayed]_work().

If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.

This patch doesn't make any functional difference.

Dan Rosenberg b5b515445f [SCSI] pmcraid: reject negative request size
There's a code path in pmcraid that can be reached via device ioctl that
causes all sorts of ugliness, including heap corruption or triggering the
OOM killer due to consecutive allocation of large numbers of pages.

First, the user can call pmcraid_chr_ioctl(), with a type
PMCRAID_PASSTHROUGH_IOCTL.  This calls through to
pmcraid_ioctl_passthrough().  Next, a pmcraid_passthrough_ioctl_buffer
is copied in, and the request_size variable is set to
buffer->ioarcb.data_transfer_length, which is an arbitrary 32-bit
signed value provided by the user.  If a negative value is provided
here, bad things can happen.  For example,
pmcraid_build_passthrough_ioadls() is called with this request_size,
which immediately calls pmcraid_alloc_sglist() with a negative size.
The resulting math on allocating a scatter list can result in an
overflow in the kzalloc() call (if num_elem is 0, the sglist will be
smaller than expected), or if num_elem is unexpectedly large the
subsequent loop will call alloc_pages() repeatedly, a high number of
pages will be allocated and the OOM killer might be invoked.

It looks like preventing this value from being negative in
pmcraid_ioctl_passthrough() would be sufficient.

Jeff Garzik f281233d3e SCSI host lock push-down
Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.

The patch below presents a simple SCSI host lock push-down as an
equivalent transformation.  No locking or other behavior should change
with this patch.  All existing bugs and locking orders are preserved.

Additionally, add one parameter to queuecommand,
	struct Scsi_Host *
and remove one parameter from queuecommand,
	void (*done)(struct scsi_cmnd *)

Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.

Minimal code disturbance was attempted with this change.  Most drivers
needed only two one-line modifications for their host lock push-down.

Arnd Bergmann 6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Martin K. Petersen 086fa5ff08 block: Rename blk_queue_max_sectors to blk_queue_max_hw_sectors
The block layer calling convention is blk_queue_<limit name>.
blk_queue_max_sectors predates this practice, leading to some confusion.
Rename the function to appropriately reflect that its intended use is to
set max_hw_sectors.

Also introduce a temporary wrapper for backwards compability.  This can
be removed after the merge window is closed.

Anil Ravindranath a70757ba9a [SCSI] pmcraid: fix to avoid twice scsi_dma_unmap for a command
For a particular driver error condition, driver was doing double
scsi_dma_unmaps. Driver was calling scsi_dma_unmap in
pmcraid_error_handler and return 0. This pmcraid_error_handler is called
by pmcraid_io_done which will do scsi_dma_unmap again when it has
return 0 from pmcraid_error_handler.

Anil Ravindranath 729c845666 [SCSI] pmcraid: support SMI-S object model of storage pool
PMC-Sierra mgmt application uses SMI-S model. According to SMI-S, the
object model exposed by the SMI-S provider should show an StoragePool
which contains member disks of a RAID Virtual disk and StorageVolume
based on the StoragePool. But according to SMI-S, there is a possibility
where StoragePool is created but StorageVolume is not yet created. To
satisfy this scenario, we are trying a hidden RAID Virtual disk. The
hidden RAID virtual disk will not be exposed to OS. Once a StorageVolume
is created for this RAID virtual disk it is exposed.
