Bhanu Prakash Gollapudi 1f953b0dbc libfcoe: Check for unusable FCFs before looking for conflicting FCFs
When there are multiple FCFs in the fabric, and one of them becomes
unavailable, the fabric name for the unavailable FCF becomes 0 along
with FIP_FL_AVAIL getting reset. In this case, FCF selection logic does
not select any FCF as it first checks for conflicting FCFs (since fabric
name is 0, it fails the condition), instead of first checking if it is
usable or not. Fix it by first checking if FCF is usable and skip that
FCF, and go to the next one in the list to check if it can be selected.

Bhanu Prakash Gollapudi b2593cbe18 libfcoe: Handle CVL while waiting to select an FCF
When a CVL is received while we wait to select best FCF, we drop it
without handling it. This causes initiator and the switch to go
out-of-sync. Initiator proceeds selecting one of the FCFs and tries to
send FIP FLOGI. However the switch may reject the FLOGI, as it has
cleared its internal state, and expects the initiator to start FIP
discovery protocol. Fix this condition by resetting the fcoe
controller.

Based on feedback and discussion with Neil Horman it seems that the above patch
may have a case where the fcoe_vport_destroy() and fcoe_destroy_work() can
race; hence that patch has been withdrawn with this patch that is trying to
solve the same problem in a different way.

In the current approach instead of removing the fcoe_config_mutex from the
vport_delete callback function; I've chosen to delete all the NPIV ports first
on a given root lport before continuing with the removal of the root lport.

Neil Horman f9184df3b9 fcoe: close race on link speed detection in fcoe code
When creating an fcoe interfce, we call fcoe_link_speed_update before we add the
lports fcoe interface to the fc_hostlist.  Since network device events like
NETDEV_CHANGE are only processed if an fcoe interface is found with an
underlying netdev that matches the netdev of the event.  Since this processing
in fcoe_device_notification is how link_speed changes get communicated to the
libfc  code (via fcoe_link_speed_update), we have a race condition - if a
NETDEV_CHANGE event is sent after the call to fcoe_link_speed_update in
fcoe_netdev_config, but before we add the interface to the fc_hostlist, we will
loose the event and attributes like /sys/class/fc_host/hostX/speed will not get
updated properly.

Fix this by moving the add to the fc_hostlist above the serialized call to
fcoe_netdev_config, ensuring that we catch netdev envents before we make a
direct call to fcoe_link_speed_update.

Also use this opportunity to clean up access to the fc_hostlist a bit by
creating a fcoe_hostlist_del accessor and replacing the cleanup in fcoe_exit to
use it properly.

Akinobu Mita 496f2f93b1 random32: rename random32 to prandom
This renames all random32 functions to have 'prandom_' prefix as follows:

The purpose of this renaming is to prevent some kernel developers from
assuming that prandom32() and random32() might imply that only
prandom32() was the one using a pseudo-random number generator by
prandom32's "p", and the result may be a very embarassing security
exposure.  This concern was expressed by Theodore Ts'o.

And furthermore, I'm going to introduce new functions for getting the
requested number of pseudo-random bytes.  If I continue to use both
prandom32 and random32 prefixes for these functions, the confusion
is getting worse.

As a result of this renaming, "prandom_" is the common prefix for
pseudo-random number library.

Currently, srandom32() and random32() are preserved because it is
difficult to rename too many users at once.

AFAICS, the situation for fcoe_transport_disable() seems to be
the same as for fcoe_transport_enable().  IOW, shouldn't it have
restart_syscall() removed as well?  I don't see any in-tree ->disable()
instances that could return -ERESTARTSYS, anyway...

Robert Love 435c86679a fcoe: Use the fcoe_sysfs control interface
This patch adds support for the new fcoe_sysfs
control interface to fcoe.ko. It keeps the deprecated
interface in tact and therefore either the legacy
or the new control interfaces can be used. A mixed mode
is not supported. A user must either use the new
interfaces or the old ones, but not both.

The fcoe_ctlr's link state is now driven by both the
netdev link state as well as the fcoe_ctlr_device's
enabled attribute. The link must be up and the
fcoe_ctlr_device must be enabled before the FCoE
Controller starts discovery or login.

Robert Love 6a891b071b libfcoe, fcoe, bnx2fc: Add new fcoe control interface
This patch does a few things.

1) Makes /sys/bus/fcoe/ctlr_{create,destroy} interfaces.
   These interfaces take an <ifname> and will either
   create an FCoE Controller or destroy an FCoE
   Controller depending on which file is written to.

   The new FCoE Controller will start in a DISABLED
   state and will not do discovery or login until it
   is ENABLED. This pause will allow us to configure
   the FCoE Controller before enabling it.

2) Makes the 'mode' attribute of a fcoe_ctlr_device
   writale. This allows the user to configure the mode
   in which the FCoE Controller will start in when it
   is ENABLED.

   Possible modes are 'Fabric', or 'VN2VN'.

   The default mode for a fcoe_ctlr{,_device} is 'Fabric'.
   Drivers must implement the set_fcoe_ctlr_mode routine
   to support this feature.

   libfcoe offers an exported routine to set a FCoE
   Controller's mode. The mode can only be changed
   when the FCoE Controller is DISABLED.

   This patch also removes the get_fcoe_ctlr_mode pointer
   in the fcoe_sysfs function template, the code in
   fcoe_ctlr.c to get the mode and the assignment of
   the fcoe_sysfs function pointer to the fcoe_ctlr.c
   implementation (in fcoe and bnx2fc). fcoe_sysfs can
   return that value for the mode without consulting the
   LLD.

3) Make a 'enabled' attribute of a fcoe_ctlr_device. On a
   read, fcoe_sysfs will return the attribute's value. On

Instead of creating a structure with an enum and a pointer
to a string, simply allocate an array of strings and use
the enum values for the indicies.

This means that we do not need to iterate through the list
of entries when looking up a string name by its enum key.

This will also help with a latter patch that will add
more fcoe_sysfs attributes that will also use the
fcoe_enum_name_search macro. One attribute will also do
a reverse lookup which requires less code when the
enum-to-string mappings are organized as this patch makes
them to be.

Neerav Parikh 31c37a6f21 [SCSI] fcoe: Fix write errors on NPIV ports
SCSI errors were generated while writing to LUNs
connected via NPIV ports.

Debugging this it was found that the FCoE packets
transmitted via the NPIV ports were not tagged with
correct user priority as negotiated with peer by DCB
agent. This resulted in FCoE traffic going with priority
zero(0) that did not have priority flow control (PFC)
enabled for it. The initiator after transferring data
to the target never saw any reply indicating the transfer
was complete. This resulted in error recovery (ABTS) and
SCSI command retries by the scsi-mid layer; eventually
resulting in I/O errors.

This patch fixes this issue by keeping the FCoE user
priority information in the fcoe_interface instance
that is common for both the physical port as well as
NPIV ports connected to that physical port; instead
of storing it in fcoe_port structure that has a per
port instance.

Neil Horman 95fdd5e980 [SCSI] fcoe: Cleanup locking on fcoe_percpu_receive_thread
Noticed that we can shuffle the code around in fcoe_percpu_receive_thread a bit
and avoid taking the fcoe_rx_list lock twice per iteration.  This should improve
throughput somewhat.  With this change we take the lock, and check for new
frames in a single critical section.  Only if the list is empty do we drop the
lock and re-acquire it after being signaled to wake up.

Change Notes:
v2) did some further cleanup on the patch by replacing the 2nd call of
spin_lock/splice_init with a goto to the top of the outer loop.  This allows me
to change the inner while loop to an if conditional and remove the sencond check
of kthread_should_stop.  Based on suggestion from Vasu Dev.

strtoul returns an 'unsigned long' so there is no
reason to check if the value is less than zero.

strtoul already checks for the '-' character deep
in its bowels. It will return an error if the user
has provided a negative value and fcoe_str_to_dev_loss
will return that error to its caller.

This patch fixes the following Coverity reported warning:

CID 703581 -  NO_EFFECT Unsigned compared against 0 - This

Recent changes to add fcoe_sysfs caused libfcoe_init to call fcoe_transport_exit
in a module initialization routine. The change resulted in the below error. This
patch removes the __exit keyword from the fcoe_transport_exit definition such
that it may be called from an __init routine.

The function __init init_module() references
a function __exit fcoe_transport_exit().

Robert Love 9a74e884ee [SCSI] libfcoe: Add fcoe_sysfs
This patch adds a 'fcoe bus' infrastructure to the kernel
that is driven by changes to libfcoe which allow LLDs to
present FIP (FCoE Initialization Protocol) discovered
entities and their attributes to user space via sysfs.

This patch adds the following APIs-

fcoe_ctlr_device_add
fcoe_ctlr_device_delete
fcoe_fcf_device_add
fcoe_fcf_device_delete

They allow the LLD to expose the FCoE ENode Controller
and any discovered FCFs (Fibre Channel Forwarders, e.g.
FCoE switches) to the user. Each of these new devices
has their own bus_type so that they are grouped together
for easy lookup from a user space application. Each
new class has an attribute_group to expose attributes
for any created instances. The attributes are-

A device loss infrastructre similar to the FC Transport's
is also added by this patch. It is nice to have so that a
link flapping adapter doesn't continually advance the count
used to identify the discovered FCF. FCFs will exist in a
"Disconnected" state until either the timer expires or the
FCF is rediscovered and becomes "Connected."

This patch generates a few checkpatch.pl WARNINGS that
I'm not sure what to do about. They're macros modeled
around the FC Transport attribute building macros, which
have the same 'feature' where the caller can ommit a cast
in the argument list and no cast occurs in the code. I'm
not sure how to keep the code condensed while keeping the
macros. Any advice would be appreciated.

Robert Love 619fe4bed4 [SCSI] fcoe: Allocate fcoe_ctlr with fcoe_interface, not as a member
Currently the fcoe_ctlr associated with an interface is allocated
as a member of struct fcoe_interface. This causes problems when
attempting to use the new fcoe_sysfs APIs which allow us to allocate
the fcoe_interface as private data to the fcoe_ctlr_device instance.
The problem is that libfcoe wants to be able use pointer math to find a
fcoe_ctlr's fcoe_ctlr_device as well as finding a fcoe_ctlr_device's
assocated fcoe_ctlr. To do this we need to allocate the
fcoe_ctlr_device, with private data for the LLD. The private data
contains the fcoe_ctlr and its private data is the fcoe_interface.
This patch only allocates the fcoe_interface with the fcoe_ctlr, the
fcoe_ctlr_device will be added in a later patch, which will complete
the below diagram-

Vasu Dev 433eba04c2 [SCSI] fcoe: remove lport from net device before doing per cpu rx thread cleanup
Remove lport from net device and then do synchronize net device to flush
inflight rx frames for the lport before doing fcoe_percpu_clean.

In case of master lport, remove all rx packet handlers completely and
then only do fcoe_percpu_clean. This required splitting fcoe_interface_cleanup
to do remove part separately and for that added func fcoe_interface_remove
and then  call it from fcoe_if_destory before doing fcoe_percpu_clean.
However if fcoe_interface_remove() is already called then
don't call again from fcoe_interface_cleanup() to preserve its
existing flows.

This patch along with Neil's other patch to avoid soft irq context
on ingress will avoid passing up frames on disabled lport as
discussed in this mail thread:-

The rtnl_lock is primarily used to serialize networking
driver changes as well as to ensure that a networking driver
is not removed when making changes to it. fcoe also uses
the rtnl_lock to protect the fcoe hostlist.

fcoe_create holds the rtnl_lock over the entirity of the
routine including a the call to fcoe_ctlr_link_up.
This causes the below deadlock because fcoe_ctlr_link_up
acquires the fcoe_ctlr ctlr_mutex and this deadlocks with
a libfcoe thread that acquires the fcoe_ctlr ctlr_mutex and
then the rtnl_lock (to update a MAC address).

This patch drops the rtnl_lock before calling
fcoe_ctlr_link_up and therefore the deadlock is prevented.

Neil Horman 20dc3811a2 [SCSI] fcoe: reduce contention for fcoe_rx_list lock [v2]
There is potentially lots of contention for the rx_list_lock.  On a cpu that is
receiving lots of fcoe traffic, the softirq context has to add and release the
lock for every frame it receives, as does the receiving per-cpu thread.  We can
reduce this contention somewhat by altering the per-cpu threads loop such that
when traffic is detected on the fcoe_rx_list, we splice it to a temporary list.
In this way, we can process multiple skbs while only having to acquire and
release the fcoe_rx_list lock once.

Neil Horman 94aa29f28e [SCSI] foce: remove bh disable from fcoe sw transport rcv function
The fcoe sw recive packet function (fcoe_rcv) only ever executes in softirq
context.  Given that, and the fact that no use of the fcoe_rx_list is made in
irq context, its not necessecary to disable bottom halves while actually
receiving the frame.  Convert spin_*_bh calls in that function to their
lock-only equivalents

Bhanu Prakash Gollapudi 81c11dd2ed [SCSI] libfcoe: Support extra MAC descriptor to be used as FCoE MAC
Some switch implementations (eg., HP virtual connect FlexFabric) send two MAC
descriptors in FIP FLOGI response, with first MAC descriptor (granted_mac) used
as FPMA, and the second one (fcoe_mac) used as destination address for
sending/receiving FCoE packets. fip_mac continues to be used for FIP traffic.
This patch introduces fcoe_mac in fcoe_fcf structure. For regular switches,
both fcoe_mac and fip_mac will be the same. For the switches that send
additional MAC descriptor, fcoe_mac is updated.

When handling CVL with no Vx port descriptors, lports for NPIV ports are reset

Neil Horman 5e70c4c43e [SCSI] fcoe: Ensure fcoe_recv_frame is always called in process context
commit 859b7b649a introduced the ability to call
fcoe_recv_frame in softirq context.  While this is beneficial to performance,
its not safe to do, as it breaks the serialization of access to the lport
structure (i.e. when an fcoe interface is being torn down, theres no way to
serialize the teardown effort with the completion of receieve operations
occuring in softirq context.  As a result, lport (and other) data structures can
be read and modified in parallel leading to corruption.  Most notable is the
vport list, which is protected by a mutex, that will cause a panic if a softirq
receive while said mutex is locked.  Additionaly, the ema_list, discussed here:

Can be corrupted if a list traversal occurs in softirq context at the same time
as a list delete in process context.  And generally the lport state variables
will not be stable, and may lead to unpredictable results.

The most direct fix is to remove the bits from the above commit that allowed
fcoe_recv_frame to be called in softirq context.  We just force all frames to be
handled by the per-cpu rx threads.  This will allow the fcoe_if_destroy's use of
fcoe_percpu_clean to function properly, ensuring that no frames are being
received while the lport is being torn down.

Yi Zou cf64bc8f09 fcoe: use CHECKSUM_UNNECESSARY instead of CHECKSUM_PARTIAL on tx
Fix a bug when using 'ethtool -K ethx tx off' to turn off tx ip checksum,
FCoE CRC offload should not be impacte. The skb_checksum_help() is needed
only if it's not FCoE traffic for ip checksum, regardless of ethtool toggling
the tx ip checksum on or off. Instead of using CHECKSUM_PARTIAL, we will
use CHECKSUM_UNNECESSARY as a proper indication to avoid sw ip checksum
on FCoE frames.

Robert Love ccefd23ed2 [SCSI] fcoe: Do not switch context in vport_delete callback
Currently all port deletion is routed though the FCoE
workqueue (fcoe_wq). When fc_remove_host is called on
an N_Port (for example, from fcoe_destroy) the vports
are queued into a FC Transport workqueue. fc_remove_host
flushes that queue and each vport is passed to fcoe's
fcoe_vport_destroy, which simply queues the associated
fcoe_ports for later deletion. This queue cannot be
flushed within the N_Ports destroy path because of
circular locking issues. The result is that the NPIV
ports are destroyed after the N_Port, which is reverse
of how they are created.

This quirk causes fcoe to keep references on the
fcoe_interface shared by each of these ports (N_Port
and NPIV). Changing the ordering such that NPIV ports
are destroyed before the N_Port will allow us to remove
reference counting on the fcoe_interface instances.

This patch simply allows fcoe_vport_destory to destroy
NPIV ports without deferring them to a workqueue context.
This ensures that when fc_remove_host is called the
NPIV ports will be destroyed first before the N_Port and
allows reference counting on the fcoe's fcoe_interface
to be remove in a later patch.

The label implies that it should be called when
there is 'nomod.' I read that to mean that the
module reference 'get' failed. However, it's only
called when the module reference 'get' succeeded.

I think it makes more sense to name the label,
'out_putmod' since it should be called when we
need to 'put' the module reference taken in the
routine before returning.

log entries anytime any netdev goes up or down, so when booting

Yi Zou a762dce41c [SCSI] fcoe: fix regression on offload em matching function for initiator/target
This is a regression introduced by commit
1ff9918b62 The else statement here is breaking
the initiator logic of allocating xid from the offloaded em xid pool for READ
I/O only to use DDP, as shown by the snippet of trace below, where the WRITE
is using xid 0x5 from the offloaded em xid pool:

The bug is in the else statement, for both initiator and target, the
new command will have FC frame header bit 23 (FC_FC_EX_CTX) cleared as it was
originated from the initiator. Also, this is assuming the frame header is
already filled up, which is only true for target since for initiator, this is a
new frame and oem_match gets called when em tries get xid for this i/o before
it is filled up and sent out.

The fix is to check if there is a fc_fcp_pkt associated w/ this frame from
fr_fsp(fp), since fr_fsp(fp) is NULL for tcm_fc target and non-I/O frame in
initiator. This should also return true for target only if it is an
FC_RCTL_DD_UNSOL_CMD and rx_id is not allocated.

Use DCB notifiers to set the skb priority to allow packets
to be steered and tagged correctly over DCB enabled drivers
that setup traffic classes.

This allows queue_mapping() routines to be removed in these
drivers that were previously inspecting the ethertype of
every skb to mark FCoE/FIP frames.

Eric Dumazet 5c609ff937 [SCSI] fcoe: use kthread_create_on_node
Since fcoe_percpu_thread_create() creates percpu kthread, it makes sense
to use kthread_create_on_node() to get proper NUMA affinity for kthread
stack.

Jiri Pirko 4bc71cb983 net: consolidate and fix ethtool_ops->get_settings calling
This patch does several things:
- introduces __ethtool_get_settings which is called from ethtool code and
  from drivers as well. Put ASSERT_RTNL there.
- dev_ethtool_get_settings() is replaced by __ethtool_get_settings()
- changes calling in drivers so rtnl locking is respected. In
  iboe_get_rate was previously ->get_settings() called unlocked. This
  fixes it. Also prb_calc_retire_blk_tmo() in af_packet.c had the same
  problem. Also fixed by calling __dev_get_by_index() instead of
  dev_get_by_index() and holding rtnl_lock for both calls.
- introduces rtnl_lock in bnx2fc_vport_create() and fcoe_vport_create()
  so bnx2fc_if_create() and fcoe_if_create() are called locked as they
  are from other places.
- use __ethtool_get_settings() in bonding code

Robert Love 848e7d5b46 [SCSI] fcoe: Fix deadlock between fip's recv_work and rtnl
The rtnl cannot be held durrng the fcoe_interface_put.
If it is the last reference on the fcoe_interface the
fcoe_ctlr_destroy will be called as a part of the
cleanup, ultimately calling cancel_work_sync(&fip->recv_work);

If we are processing a flogi response we will be in
the recv_work context and we will lock the rtnl to
add a new unicast MAC address. This is how the deadlock
can occur.

The fix is simply to move the rtnl_lock/unlock into
fcoe_interface_cleanup so that it can be unlocked before
fcoe_interface_put is called.

Vasu Dev d272281c39 [SCSI] fcoe: cleanup cpu selection for incoming requests
Cleanup to:

- have selection for all types of frames, not just FCP.
- remove redundant cpu_online check once fcoe_select_cpu called
  as this is not required since later code flow check for offlined
  cpu.
- Simplify fcoe_select_cpu() by removing unnecessary checks to
  skip curr_cpu, this also fixes possibly infinite loop in case
  of curr_cpu is the only cpu while iterating in the loop.

This cleanup mainly applies to target as incoming request are
mostly for target, therefore Kiran has verified the patch
with target also.

Drop the rx frame having xid with wrong cpu info
or received with xid  not matching to our xid.

Not dropping such frame is causing panic as

Neerav Parikh b2085a4efc [SCSI] fcoe: Rearrange fcoe port and NPIV port cleanup
When NPIV port destroy handler is called it does not do all the cleanup
required for the given NPIV port. This was happening as some of the
lport cleanup moved to fcoe_interface_cleanup() routine, which is not
called as part of the vport delete process.

This patch rearranges the sequence in which the fcoe_if_destory() and
fcoe_interface_cleanup() functions are being called from various places
in the code. It now matches the sequence they are constructed during the
create process for both N_Port as well as NPIV port.

Kiran Patil 29bdd2bb3e [SCSI] fcoe: Amends previous patch, Round-robin based selection of CPU for post processing of incoming request for FCoE target
Problem: Selection of RX queue on target is based on RX-ID. FCoE used
8 Net Rx queues.  HW post the packets based on rx_id %
num_rx_queue. Due to this has based filtering, only one CPU is busy
servicing incoming request including post-processing of incoming
request. This is gating factor because

1. Only one CPU is utilized 100% while others CPUs are not used at all.

2. CPU which received request assign "sequence' by selecting exchange
   from per CPU pool (num_ddp_context / num_online_cpus,
   approxi.). Due to which if if rate of incoming request is higher
   than rate of servicing request, existing code path end of sending
   "BUSY" response (SAM_STAT_BUSY because unable to allocate
   exchange).

Fix: Fan-out incoming request to all other CPUs excluding the CPU
which is receiving all incoiming request. This path also addresses,
selecting same CPU based on rx_id from received frame for completion

Kiran Patil 064287eee3 [SCSI] fcoe: Round-robin based selection of CPU for post-processing of incoming commands
Problem: Earlier mechanism of selection of CPU was, to select the same CPU
which has received incoming request. Hence in case of rx_id = 0xFFFF,
request was always posted to same NetRx queue, hence only 1 CPU is utilized
for handling the command. It was also causing problem of "running out of
exchanges from per CPU pool of exchanges (in case of DDP offload)

Fix: Implemented new algo. to select CPU for post-processing of
incoming commands when rx_id is unknown. This is simple Round robin
algo. for CPU selection.

Kiran Patil 1ff9918b62 [SCSI] fcoe: Unable to select the exchangeID from offload pool for storage targets
Problem: When initiator sends write command to target, target tries to
assign new sequence. It allocates new exchangeID (RX_ID)
always from non-offloaded pool (Non-offload EMA)

Fix: Enhanced fcoe_oem_match routine to look at F_CTL flags and if it
is exchange responder and command type is WRITEDATA, then function
returns TRUE instead of FALSE. This function is used to determine
which pool to use (offload pool of exchange is used only if this
function returns TRUE).

concern arose that the module parameters were being added
with write permissions before the module had completed

This check was called out as unhelpful as the module can
go dead at any time and therefore its state isn't a reliable
thing to look at as a sign of stability and initialization
completion. Also, that functional interfaces like these
should be added after module initialization.

This patch removes the unnecessary checks and hopes to
disprove the concern about initialization ordering.

Recent fcoe transport rework changes now require fcoe
transports to register with libfcoe before any operation
can take place. libfcoe may access some static variables
but nothing that could cause a problem. Once a fcoe transport
is registered, libfcoe is usable and any interface calls will
be functional.

Vasu Dev f04ca1b654 [SCSI] fcoe: have fcoe log off and lport destroy before ndo_fcoe_disable
Currently fcoe interface cleanup is done after ndo_fcoe_disable
and that prevents logoff going out to the peer, so this patch
moves all netdev cleanup and its releasing inside
fcoe_interface_cleanup to have log off before ndo_fcoe_disable
disables the fcoe.

This patch also fixes asymmetric rtnl locking around fcoe_if_destroy,
as currently this function requires rtnl held by its caller
and then have this func drops the lock, instead now don't have
any processing under rtnl inside fcoe_if_destroy, this required
moving few func to get build working again.

When rmmoving the underlying fcoe transport driver module by force when
it's attached and in use, the correspoding netdev mapping should be
cleaned up properly as well, otherwise the lookup for a given netdev
for the transport would still return non NULL pointer, causing "unable
to handle paging request" bug.

The check of module state being MODULE_STATE_LIVE is no longer needed for the
individual fcoe transport driver, e.g., fcoe.ko, as sysfs entries now go to

writable before module_init function is called and after module_exit.

David Decotigny 8ae6daca85 ethtool: Call ethtool's get/set_settings callbacks with cleaned data
This makes sure that when a driver calls the ethtool's
get/set_settings() callback of another driver, the data passed to it
is clean. This guarantees that speed_hi will be zeroed correctly if
the called callback doesn't explicitely set it: we are sure we don't
get a corrupted speed from the underlying driver. We also take care of
setting the cmd field appropriately (ETHTOOL_GSET/SSET).

This applies to dev_ethtool_get_settings(), which now makes sure it
sets up that ethtool command parameter correctly before passing it to
drivers. This also means that whoever calls dev_ethtool_get_settings()
does not have to clean the ethtool command parameter. This function
also becomes an exported symbol instead of an inline.

All drivers visible to make allyesconfig under x86_64 have been
updated.

Vasu Dev d2f809528a [SCSI] fcoe: fix broken fcoe interface reset
Reset using "fcoeadm -r" also needs to restart FIP before
doing libfc lport reset, this is needed for new switch firmware
requiring FIP solicitation before doing FLOGI again during reset.
So this patch does this by doing fcoe_ctlr_link_down and then
fcoe_ctlr_link_up to reset the interface.

The fcoe_ctlr_link_down call path also does lport reset
and then fcoe_ctlr_link_up re-starts the fabric login after
doing FIP solicitation first to get reset feature working
again.

Bhanu Prakash Gollapudi 70be6344ea [SCSI] libfcoe: Remove stale fcoe-netdev entries
When L2 driver is unloaded, libfcoe_destroy tries to access the fcoe
transport structure matching the netdev. However, since the netdev is
unregistered by that time, it fails to do so. Hence the stale mappings
exists in the fcoe-netdev list. Handle NETDEV_UREGISTER device
notification mechanism to remove the stale fcoe-netdev mapping.

Bhanu Prakash Gollapudi 8597ae8bfe [SCSI] libfcoe: Move common code from fcoe to libfcoe module
To facilitate LLDDs to reuse the code, skb queue related functions are moved to
libfcoe, so that both fcoe and bnx2fc drivers can use them. The common structures
fcoe_port, fcoe_percpu_s are moved to libfcoe. fcoe_port will now have an
opaque pointer that points to corresponding driver's interface structure.
Also, fcoe_start_io and fcoe_fc_crc are moved to libfcoe.

As part of this change, fixed fcoe_start_io to return ENOMEM if
skb_clone fails.

Tejun Heo 2ca32b4848 [SCSI] fcoe: use dedicated workqueue instead of system_wq
fcoe uses the system_wq to destroy ports and the work items need to be
flushed before the driver is unloaded.  As the work items free the
containing data structure, they can't be flushed directly.  The
workqueue should be flushed instead.

Also, the destruction works can be chained - ie. destruction of a port
may lead to destruction of another port where the work item for the
former queues the work for the latter.  Currently, the depth of chain
can be at most two and fcoe_exit() makes sure everything is complete
by calling flush_scheduled_work() twice.

With commit c8efcc25 (workqueue: allow chained queueing during
destruction), destroy_workqueue() can take care of chained works on
workqueue destruction.  Add and use fcoe_wq instead.  Simply
destroying fcoe_wq on driver unload takes care of flushing.

Yi Zou 78a582463c [SCSI] fcoe: convert fcoe.ko to become an fcoe transport provider driver
Remove the existing sysfs entry points of the fcoe.ko module parameters that
are used to create/destroy/enable/disable an FCoE instance, rather, use the
newly added fcoe transport code to attach itself as an FCoE transport provider
when fcoe.ko gets loaded. There is no functionality change on the logic of
fcoe interacts with upper libfc and lower netdev. The fcoe transport only acts
as thin layer to provide a unified interface for all fcoe transport providers
so all FCoE instances on any network interfaces from all vendors can be
managed through the same Open-FCoE.org's user space tool package, which also
has full DCB support.

Yi Zou fdecf31b7c [SCSI] libfcoe: add implementation to support fcoe transport
Add the new fcoe_transport.c file that implements basic fcoe transport
interface. Eventually, the sysfs entries to create/destroy/enable/disable
an FCoE instance will be coming to the fcoe transport layer, who does a
look-up to find the corresponding transport provide and pass the corresponding
action over to the identified provider.

The fcoe.ko will become the default fcoe transport provider that can support
FCoE on any given netdev interfaces, as the Open-FCoE.org's default software
FCoE HBA solution. Any vendor specific FCoE HBA driver that is built on top
of Open-FCoE's kernel stack of libfc & libfcoe as well as the user land tool
of fcoe-utils can easily plug-in and start running FCoE on their network
interfaces. The fcoe.ko will be converted to act as the default provider if
no vendor specific transport provider is found, as it is always added to the
very end of the list of attached transports.

Vasu Dev 52ee832195 [SCSI] fcoe: drop FCoE LOGO in FIP mode
Allowing FCoE LOGO followed by CVL in this case prevents
FIP login back to the FCF and then keeps lport offline,
only FIP LOGO and CLV needs to be processed while in
FIP mode, therefore this patch drops FCoE LOGO in FIP mode.

Added fcoe_filter_frames() to filter out above mentioned LOGO
in fcoe rx path along with other existing filtering in code
for bad CRC frames. Adding separate fcoe_filter_frames function
helped with better code indentations and if needed then same
will allow adding more filters at one place in future.

This LOGO drop is added after FCP frames passed up to avoid
any additional checks on fast path for this.

Robert Love 7287fb9114 [SCSI] fcoe: Fix module reference count for vports
vports are not grabbing module references but are
releasing them. This causes the module reference count
to decrement too many times and it wraps around past 0.

The solution is to do a module_put() in
fcoe_interface_release() so that the reference is only
released when the fcoe_interface is released. There is a
one-to-one relationship between the N_Port and the
fcoe_interface, so the module reference will only be
dropped when the N_Port is destroyed

To create symetry in the code this patch moves the
try_module_get() call into fcoe_interface_create(). This
means that only the N_Port will grab a reference to the
module when its corresponding fcoe_interface is created.

This patch also makes it so that the fcoe_interface_create()
routine encodes any error codes in the fcoe_interface
pointer returned. This way its caller, fcoe_create(), can
return an accurate error code.

Joe Eykholt 981c1154b2 [SCSI] libfcoe: reorder FCF list to put latest advertiser first
When there are several FCFs to choose from, the one most likely
to accept a FLOGI on certian switches is the one that last
answered a multicast solicit.

So, when receiving an advertisement, move the FCF to the front
of the list so that it gets chosen first among those with the
same priority.

Without this, more FLOGIs need to be sent in a test with
multiple FCFs and a switch in NPV mode, but it still
eventually finds one that accepts the FLOGI.

Switches using multiple-FCFs may reject FLOGI in order to
balance the load between multiple FCFs.  Even though the FCF
was available, it may have more load at the point we actually
send the FLOGI.

If the FLOGI fails, select a different FCF
if possible, among those with the same priority.  If no other
FCF is available, just deliver the reject to libfc for retry.

Yi Zou 80e736f8a3 [SCSI] libfc: fix NULL pointer dereference bug in fc_fcp_pkt_release
This happens when then tearing down the fcoe interface with active I/O.
The back trace shows dead000000200200 in RAX, i.e., LIST_POISON2, indicating
that the fsp is already being dequeued, which is probably why no complaining
was seen in fc_fcp_destroy() about outstanding fsp not freed, since we dequeue
it in the end of fc_io_compl() before releasing it. The bug is due to the
fact that we have already destroyed lport's scsi_pkt_pool while on-going i/o
is still accessing it through fc_fcp_pkt_release(), like this trace or the
similar code path from scsi-ml to fc_eh_abort, etc. This is fixed by moving
the fc_fcp_destroy() after lport is detached from scsi-ml since fc_fcp_destroy
is supposed to called only once where no lport lock is taken, otherwise the
fc_fcp_pkt_release() would have to grab the lport lock.

Robert Love 0ee31cb5df [SCSI] fcoe: Fix broken NPIV with correction to MAC validation
A previous patch attempted to validate the destination
MAC address of a FCoE frame by checking that MAC
address against the received port's MAC address. The
implementation seems fine on the surface, but any
VN_Ports added using the NPIV feature will have their
own MAC addresses and these MACs were not being checked,
which prevented any NPIV VN_Ports from receiving frames.

In other words, the following patch has broken NPIV.

 [SCSI] fcoe: adds src and dest mac address
              checking for fcoe frames

Part of the offending patch is correct, but the part
that broke NPIV was attempting to satisfy FC-BB-5
section D.5, 2.1-

(discard frames that) "contain a destination MAC
address/destination N_Port_ID pair that was not
assigned by an FCF to one of the VN_Ports on the ENode"

The language does _not_ say to compare the destination
FC-MAP/destination N_Port_ID, but instead to compare
the destination MAC address/destination N_Port_ID.

>From the FC-BB-5 specification,

"A properly formed FPMA is one in which the 24 most
significant bits equal the Fabric’s FC-MAP value and
the least significant 24 bits equal the N_Port_ID
assigned to the VN_Port by the FCF."

This means that we need to compare the FC Frame's
destination FCID against the embedded FCID in the
destination MAC address. This patch checks the lower
24 bits of the destination MAC address against
destination FCID in the Fibre Channel frame.

For MAC validation the first line of defense is the
hardware MAC filtering. Each VN_Port will have a
unicast MAC addresses added to the hardware's
filtering table. The Ethernet driver should drop any
MACs not destined for a programmed MAC. This patch
adds a second line of defense that very specfically
compares an element in the FC frame against an element
in the Ethernet header, which is appropriate for the
FCoE layer.

Many alternative approaches were considered, including
a LLD callback from libfc. The second most reasonable
approach seemed to be walking the list of NPIV ports
and check each of their MAC addresses against the
destination MAC address of the received frame. The
problem with this approach was that it is likely that
performance would suffer with the more NPIV ports added
to the system since every received frame would need to
walk this list, comparing each entry's MAC.

Kiran Patil 2dc02ee52f [SCSI] libfcoe: VN2VN connection setup causing stack memory corruption.
Fix: When FIP frame is received, function fcoe_ctlr_vn_recv calls function
fcoe_ctlr_vn_parse which does memset for addr (&buf.rdata) which leads to
memory corruption. Code was trying to treat "buf" as struct but it was defined
as union. Fix is to change from union to struct for "buf" in function fcoe_ctlr_vn_recv.

Since sometimes current FIP_MODE_AUTO mode falls back to non-FIP
mode while DCB link still getting ready in fabric mode with
its peer switch, it falls back after few libfc flogi retries
and that is not we want while working with FIP enabled
switches in FABRIC mode, therefore sets default as FIP_MODE_FABRIC
as discussed and agreed before in this mail thread

Vasu Dev 240778e821 [SCSI] fcoe: remove check for zero fabric name
This check prevents FCF selection in NPV mode due to zero fabric name
in that case and in turn flogi fails. Though NPV mode should not have
this zero and should be fixed there also but spec also does not require
initiator to ensure that fabric name must be non-zero, therefore dropping
this check to get flogi working in NPV mode.

Vasu Dev cf4aebcafb [SCSI] Revert "[SCSI] fcoe: Fix using VLAN ID in creating lport's WWWN/WWPN"
This reverts commit cc0136c2e9.

That commit introduced vlan id info to WWPN but WWPN needs to
remain static as an unique port identifier in the fabric, therefore
variable fabric vlan id info doesn't need to be coded inside WWPN.

After this revert, port arg to fcoe_wwn_from_mac is always zero
but still leaving it as-is okay to later allow users to use NAA 2
scheme with this additional port arg.

Note with this patch, existing zoning using WWPN would require
re-zoning this time only and later no more re-zoning due to any
vlan id changes.

Joe Eykholt 9226115695 [SCSI] libfc: don't require a local exchange for incoming requests
Incoming requests shouldn't require a local exchange if we're
just going to reply with one or two frames and don't expect
anything further.  Don't allocate exchanges for such requests
until requested by the upper-layer protocol.

The sequence is always NULL for new requests, so remove
that as an argument to request handlers.

Also change the first argument to lport->tt.seq_els_rsp_send
from the sequence pointer to the received frame pointer, to
supply the exchange IDs and destination ID info.

Joe Eykholt e10f8c667b [SCSI] libfcoe: fcoe: fnic: add FIP VN2VN point-to-multipoint support
The FC-BB-6 committee is proposing a new FIP usage model called
VN_port to VN_port mode.  It allows VN_ports to discover each other
over a loss-free L2 Ethernet without any FCF or Fibre-channel fabric
services.  This is point-to-multipoint.  There is also a variant
of this called point-to-point which provides for making sure there
is just one pair of ports operating over the Ethernet fabric.

We add these new states:  VNMP_START, _PROBE1, _PROBE2, _CLAIM, and _UP.
These usually go quickly in that sequence.  After waiting a random
amount of time up to 100 ms in START, we select a pseudo-random
proposed locally-unique port ID and send out probes in states PROBE1
and PROBE2, 100 ms apart.  If no probe responses are heard, we
proceed to CLAIM state 400 ms later and send a claim notification.
We wait another 400 ms to receive claim responses, which give us
a list of the other nodes on the network, including their FC-4
capabilities.  After another 400 ms we go to VNMP_UP state and
should start interoperating with any of the nodes for whic we
receivec claim responses.  More details are in the spec.j

Add the new mode as FIP_MODE_VN2VN.  The driver must specify
explicitly that it wants to operate in this mode.  There is
no automatic detection between point-to-multipoint and fabric
mode, and the local port initialization is affected, so it isn't
anticipated that there will ever be any such automatic switchover.

It may eventually be possible to have both fabric and VN2VN
modes on the same L2 network, which may be done by two separate
local VN_ports (lports).

When in VN2VN mode, FIP replaces libfc's fabric-oriented discovery
module with its own simple code that adds remote ports as they
are discovered from incoming claim notifications and responses.
These hooks are placed by fcoe_disc_init().

A linear list of discovered vn_ports is maintained under the
fcoe_ctlr struct.  It is expected to be short for now, and
accessed infrequently.  It is kept under RCU for lock-ordering
reasons.  The lport and/or rport mutexes may be held when we
need to lookup a fcoe_vnport during an ELS send.

Change fcoe_ctlr_encaps() to lookup the destination vn_port in
the list of peers for the destination MAC address of the
FIP-encapsulated frame.

Add a new function fcoe_disc_init() to initialize just the
discovery portion of libfcoe for VN2VN mode.

Joe Eykholt fdb068c6cd [SCSI] libfcoe: convert FIP to lock with mutex instead of spin lock
It turns out most of the FIP work is now done from worker threads
or process context now, so there's no need to use a spin lock.

Change to use mutex instead of spin lock and delayed_work instead
of a timer.

This will make it nicer for the VN_port to VN_port feature that
will interact more with the libfc layers requiring that
spinlocks not be held.

Vasu Dev 519e5135e2 [SCSI] fcoe: adds src and dest mac address checking for fcoe frames
This is  per FC-BB-5 Annex-D recommendation and per that
if address checking fails then drop the frame.

FIP code paths are already doing this so only needed for fcoe
frames.

The src address checking is limited to only fip mode since
this might break non-fip mode used in p2p due to used OUI
based addressing in some p2p code paths, going forward FIP
will be the only mode, therefore limited this to only FIP
mode so that it won't break non-fip p2p mode for now.

Removes FCOE packet type checking since fcoe_rcv is
registered to receive only FCoE type packets from netdev
and it is already checked by netdev.

Vasu Dev 9d4cbc05f3 [SCSI] fcoe: cleans up fcoe_disable and fcoe_enable
The fc_fabric_logoff and fc_fabric_login are redundant
here after recently added fcoe_ctlr_link_down/up to
these functions, therefore this patch removes logoff
and login to only use link down and up here. This works
best for their current usages with fcoe DCB link down or up.

This also works well to avoid EIO errors when fcoe DCB link
goes down as lport state moves out of ready quickly from
fcoe_ctlr_link_down and that allows re-queuing timed out IOs
for this case also.

keep alives are disabled due to fd_flags set and also
stop updating keep alive values in that case.

Update select fcf time only if fcf is not already selected or
select time is not already determined from parse adv, and then
have select time cleared only once after fcf is selected.

Changed deadline check to time_after_eq() from time_after()
since now next timeout will be on exact 2.5 times FKA followed
by first advertisement.

Joe Eykholt 8690cb8359 [SCSI] libfcoe: fix lenient aging of FCF advertisements
[This patch has several improvements to the code in
the fip timers.  It hasn't been tested yet.
I'm sending it out for review.  Vasu, perhaps you can
merge this with your patch and test it together.]

The current code allows an advertisement to be used
even if it has been 3 times the FCF keep-alive
advertisement period (FKA) since one was received from
that FCF.  The spec. calls for 2.5 times FKA.

Fix this and make sure we detect missed keep-alives promptly.

Eric Dumazet 28172739f0 net: fix 64 bit counters on 32 bit arches
There is a small possibility that a reader gets incorrect values on 32
bit arches. SNMP applications could catch incorrect counters when a
32bit high part is changed by another stats consumer/provider.

One way to solve this is to add a rtnl_link_stats64 param to all
ndo_get_stats64() methods, and also add such a parameter to
dev_get_stats().

Rule is that we are not allowed to use dev->stats64 as a temporary
storage for 64bit stats, but a caller provided area (usually on stack)

Old drivers (only providing get_stats() method) need no changes.

Vasu Dev 15af974dcb [SCSI] fcoe: fix fcoe module ref counting
Currently fcoe module ref count is used for tracking
active fcoe instances, it means each fcoe instance create
increments the count while destroy dec the count.

The dec is done only if fcoe instance is destroyed from
/sysfs but not if destroyed due to NETDEV_UNREGISTER event.
So this patch moves only module_put doing dec to common
fcoe_if_destroy function, so that dec would occur on ever
fcoe instance destroy.

Vasu Dev 34ce27bcf9 [SCSI] fcoe: fix a circular locking issue with rtnl and sysfs mutex
Currently rtnl mutex is grabbed during fcoe create, destroy, enable
and disable operations while sysfs s_active read mutex is already
held, but simultaneously other networking events could try grabbing
write s_active mutex while rtnl is already held and that is causing
circular lock warning, its detailed log pasted at end.

In this log, the rtnl was held before write s_active during device
renaming but there are more such cases as Joe reported another
instance with tg3 open at:-

This patch fixes this issue by not waiting for rtnl mutex during
fcoe ops, that means if rtnl mutex is not immediately available
then restart_syscall() to allow others waiting in line to
grab s_active along with rtnl mutex to finish their work first
under these mutex.

Currently rtnl mutex was grabbed twice during fcoe_destroy call flow,
second grab was from fcoe_if_destroy called from fcoe_destroy after
dropping rtnl mutex before calling fcoe_if_destroy, so instead made
fcoe_if_destroy always called with rtnl mutex held to have this mutex
grabbed only once in this code path.

However left matching rtnl_unlock as-is in its original place as it was
dropped there for good reason since very next call causes synchronous
fip worker flush and if rtnl mutex is still held before flush
then that would cause new circular warning between fip->recv_work and
rtnl mutex, I've added detailed comment for this on fcoe_if_destroy
calling and rtnl muxtes unlocking.

Robert Love 7b2787ec15 [SCSI] libfc: Move the port_id into lport
This patch creates a port_id member in struct fc_lport.
This allows libfc to just deal with fc_lport instances
instead of calling into the fc_host to get the port_id.

This change helps in only using symbols necessary for
operation from the libfc structures. libfc still needs
to change the fc_host_port_id() if the port_id changes
so the presentation layer (scsi_transport_fc) can provide
the user with the correct value, but libfc shouldn't
rely on the presentation layer for operational values.

Chris Leech 9f8f3aa640 [SCSI] libfc, fcoe: normalize format specifies for world wide names
Print all world wide node names (node, port and fabric) with the same
format specifier of "%16.16llx".  That makes sure they all print as a
16 character hex string, with lower case letters, no 0x prefix, and
without stripping off any leading 0s.

Vasu Dev da87bfab8a [SCSI] fcoe, fnic, libfc: increased CDB size to 16 bytes for fcoe.
No reason to restrict CDB size to 12 bytes in fcoe, so
increased to 16 so that 16 bytes SCSI CDB doesn't fail.

Uses common define to set max_cmd_len for fcoe and fnic,
fnic is already setting max_cmd_len to 16.

sg_readcap -l fails without this fix.

A disable/enable sequence should be treated as a link event.
Otherwise, when using disable to mask a time when the link

enable.

Robert Love 593abc0720 [SCSI] libfcoe: Don't fill MAC desc in FLOGI if FIP negotiated FPMA
FPMA indicates that the Fabric will provide the host's
N_Port's MAC address. When sending a FLOGI/FDISC frame
and FPMA was negotiated through FIP discovery we still
need to provide the MAC descriptor, as per the
specification, but the MAC should be zero'd out since
the FCF will be providing it in the FLOGI/FDISC ACC.

In FC-BB-5 section 7.8.7.4.2 (Fabric login) it states:

The MAC address field in the MAC address descriptor of a FIP FLOGI
Request operation or a FIP NPIV FDISC Request operation shall contain:
a) the proposed MAC address to use as VN_Port MAC address if the ENode
   is requesting to use SPMA (see table 27);
b) all zeroes to indicate no MAC address is proposed if the ENode is
   requesting to use FPMA (see table 27); or
c) the proposed MAC address to use as VN_Port MAC address if the ENode
   supports both SPMA and FPMA and leaves the decision of which
   addressing scheme to use to the FCF (i.e., if both the FP and SP
   bits are set to one, see table 27).

This patch fixes case B.

This patch also adds debug statements to illustrate
whether a FPMA or SPMA MAC is added to a FLOGI/FDISC
frame.

Joe Eykholt f018b73af6 [SCSI] libfc, libfcoe, fcoe: use smp_processor_id() only when preempt disabled
When the kernel is configured for preemption, using smp_processor_id()
when preemption is enabled causes a warning backtrace and is wrong
since we could move off of that CPU as soon as we get the ID,
and we would be referencing the wrong CPU, and possibly an invalid one
if it could be hotswapped out.

Remove the fc_lport_get_stats() function and explicitly use per_cpu_ptr()
to get the statistics.  Where preemption has been disabled by holding
a _bh lock continue to use smp_processor_id(), but otherwise use
get_cpu()/put_cpu().

In fcoe_recv_frame() also changed the cases where we return in the
middle to do a goto to the code which bumps ErrorFrames and does
a put_cpu().  Two of these cases didn't bump ErrorFrames before, but
doing so is harmless because they "can't happen", due to prior length
checks.

Also rearranged code in fcoe_recv_frame() to have only one call to
fc_exch_recv().  It's just as efficient and saves a call to put_cpu().

In fc_fcp.c, adjusted a FIXME comment for code which doesn't need fixing.

Joe Eykholt 4291365784 [SCSI] libfcoe: eliminate unused link and last_link fields
The link and last_link fields in the fcoe_ctlr struct are no
longer useful, since they are always set to the same value,
and FIP always calls libfc to pass link information to the lport.

Eliminate those fields and rename link_work to timer_work, since
it no longer has any link change work to do.

Joe Eykholt 7d65b0df6c [SCSI] fcoe: save gateway address when receiving FLOGI request
In point-to-point mode, we need to save the source MAC
from received FLOGI requests to use as the destination MAC
for all outgoing frames.  We stopped doing that at some point.

Use the lport_set_port_id method to catch incoming FLOGI frames
and pass them to fcoe_ctlr_recv_flogi() so it can save the source MAC.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Rob Love 6409ea65b3 [SCSI] fcoe: Only rmmod fcoe.ko if there are no active connections
Currently we're gracefully tearing down each active connection
when fcoe.ko is removed. We shouldn't allow the user to destroy
connections by removing the module. We should force the user to
destroy each connection and then the module can be removed.

This patch makes it so a refrerence count on the module is taken
each time a fcoe_interface is created. The reference count
is dropped when the fcoe_interface is destroyed. This makes it
so that module_exit() doesn't get called unless all fcoe_interfaces
have been destroyed.

This patch leaves the removal of interfaces in the module_exit
routine so that if the user does a 'rmmod -f' we'll clean everything
up before removing the module.

The module_put line was put before the out_putdev goto line because
we should only be decrementing the reference count if a
fcoe_interface is actually destroyed. If we can't find the netdev
or the fcoe_interface then it's assumed that something else has
destroyed the fcoe_interface and it would have decremented the
reference count at that time.

Vasu Dev 55a66d3c1e [SCSI] fcoe, libfc: adds enable/disable for fcoe interface
This is to allow fcoemon util to enable or disable a fcoe interface
according to DCB link state change.

Adds sysfs module param enable and disable for this and also
updates existing other module param description to be consistent
and more accurate since older description had double "fcoe" word
with less meaningful netdev reference to user space.

Adds code to ignore redundant fc_lport_enter_reset handling for a
already disabled fcoe interface by checking LPORT_ST_DISABLED
or LPORT_ST_LOGO states, this also prevents lport state transition
on link flap on a disabled interface.

Above changes required lport state transition to get out of
disabled or logo state on call to fc_fabric_login.

Chris Leech 859b7b649a [SCSI] fcoe: allow SCSI-FCP to be processed directly in softirq context
Allow FCP frames to bypass the FCoE receive processing threads and handle
them directly in softirq context, if they are received on the correct CPU.
This preserves the queuing to threads for scaling out receive processing
to multiple CPUs, but allows FCoE-aware multi-queue network drivers that
direct frames to the originating CPUs to handle FCP processing with less
scheduling latency.

Only FCP is handled directly, because libfc makes use of mutexes in ELS
handling routines.

The bulk of this change is just moving the FCoE receive processing out of
the receive thread function, leaving behind just the thread and queue
management.  The interesting bits are in fcoe_rcv()

Joe Eykholt b94f8951bf [SCSI] libfc fcoe: increase ELS and CT timeouts
The FC-LS spec. says ELS timeouts should be 2 x R_A_TOV.
The FC-GS spec. says CT timeouts should be 3 x R_A_TOV.

We've been using E_D_TOV for both of those.

Change for all ELS and CT requests except FLOGI, which we
leave at 2 seconds (using E_D_TOV).  One could argue that
R_A_TOV is locally determined until after FLOGI succeeds.

This does change FLOGI for vports which becomes FDISC.
This does not change the REC/SRR timeout which is 2 seconds.

Chris Leech 18fa11efc2 [SCSI] libfc, fcoe: fixes for highmem skb linearize panics
There are cases outside of our control that may result in a transmit
skb being linearized in dev_queue_xmit.  There are a couple of bugs
in libfc/fcoe that can result in a panic at that point.  This patch
contains two fixes to prevent those panics.

1) use fast cloning instead of shared skbs with dev_queue_xmit

dev_queue_xmit doen't want shared skbuffs being passed in, and
__skb_linearize will BUG if the skb is shared.  FCoE is holding an extra
reference around the call to dev_queue_xmit, so that when it returns an
error code indicating the frame has been dropped it can maintain it's
own backlog and retransmit.  Switch to using fast skb cloning for this
instead.

2) don't append compound pages as > PAGE_SIZE skb fragments

fc_fcp_send_data will append pages from a scatterlist to the nr_frags[]
if the netdev supports it.  But, it's using > PAGE_SIZE compound pages
as a single skb_frag.  In the highmem linearize case that page will be
passed to kmap_atomic to get a mapping to copy out of, but
kmap_atomic will only allow access to the first PAGE_SIZE part.
The memcpy will keep going and cause a page fault once is crosses the
first boundary.

If fc_fcp_send_data uses linear buffers from the start, it calls
kmap_atomic one PAGE_SIZE at a time.  That same logic needs to be
applied when setting up skb_frags.

Yi Zou 75ea89ef63 [SCSI] fcoe: Fix setting lport's WWNN/WWPN to use san mac address
We are still using netdev->dev_addr to generate lport's WWNN/WWPN even if the
LLD has support for NETDEV_HW_ADDR_T_SAN. Instead, we should just use the
fip->ctl_src_addr, which is the NETDEV_HW_ADDR_T_SAN if LLD supports it or it
is just the netdev->dev_addr if it does not.

Joe Eykholt 386309ce92 [SCSI] libfcoe: fcoe: simplify receive FLOGI response
There was a locking problem where the fip->lock was held during
the call to update_mac().  The rtnl_lock() must be taken before
the fip->lock, not the other way around.  This fixes that.

Now that fcoe_ctlr_recv_flog() is called only from the response handler
to a FLOGI request, some checking can be eliminated.  Instead of calling
update_mac(), just fill in the granted_mac address for the passed-in
frame (skb).

Eliminate the passed-in source MAC address since it is also in the skb.

Also, in fcoe, call fcoe_set_src_mac() directly instead of going thru
the fip function pointer.  This will generate less code.
Then, since fip isn't needed for LOGO response, use lport as the arg.

john fastabend 59d9251684 [SCSI] fcoe: add check to fail gracefully in bonding mode
This patch adds a check to fail gracefully when the netdevice
is bonded.  Previously, the error was detected but the stack
would continue to load.  This resulted in a partially enabled
fcoe intance and errors when the fcoe instance was destroy.

Joe Eykholt f31f2a1c32 [SCSI] libfcoe: don't send ELS in FIP mode if no FCF selected
If link is up, but no FCF is selected, don't send any ELS frames.

This came up when an fnic received a multicast advertisement but
no solitited advertisments, so no FCF was selected.  It tried
to send FLOGIs anyway.

Joe Eykholt dd42dac4ec [SCSI] libfcoe: FIP should report link to libfc whether selected or not
The fnic driver with FIP is reporting link up, even though it's down.

When the interface is shut down by the switch, we receive a clear
virtual link, and set the state reported to libfc as down, although
we still report it up.  Clearly wrong.  That causes the subsequent
link down event not to be reported, and /sys shows the host "Online".

Currently, in FIP mode, if an FCF times out, then link to libfc
is reported as down, to stop FLOGIs.  That interferes with the LLD
link down being reported.

Users really need to know the physical link information, to diagnose
cabling issues, so physical link status should be reported to libfc.

If the selected FCF needs to be reported, that should be done
separately, in a later patch.

Joe Eykholt 1f4aed818d [SCSI] libfcoe: fip: allow FIP receive to be called from IRQ.
FIP's fcoe_ctlr_recv() function was previously only called from
the soft IRQ in FCoE.  It's not performance critical and is more
convenient for some drivers to call it from the IRQ level.  Just
Change to use skb_queue()/dequeue() which uses spinlock_irqsave
instead of separate locking with _bh locks.

Joe Eykholt 22bcd225bf [SCSI] libfcoe: Allow FIP to be disabled by the driver
Allow FIP to be disabled by the driver for devices
that want to use libfcoe in non-FIP mode.

The driver merely sets the fcoe_ctlr mode to the state which
should be entered when the link comes up.  The default is auto.
No change is needed for fcoe.c which uses auto mode.
