David S. Miller bcd896bae0 sparc64: Handle hugepage TSB being NULL.
Accomodate the possibility that the TSB might be NULL at
the point that update_mmu_cache() is invoked.  This is
necessary because we will sometimes need to defer the TSB
allocation to the first fault that happens in the 'mm'.

Seperate out the hugepage PTE test into a seperate function
so that the logic is clearer.

David S. Miller 89a77915e0 sparc64: Fix get_user_pages_fast() wrt. THP.
Mostly mirrors the s390 logic, as unlike x86 we don't need the
SetPageReferenced() bits.

On sparc64 we also lack a user/privileged bit in the huge PMDs.

In order to make this work for THP and non-THP builds, some header
file adjustments were necessary.  Namely, provide the PMD_HUGE_* bit
defines and the pmd_large() inline unconditionally rather than
protected by TRANSPARENT_HUGEPAGE.

Greg Kroah-Hartman 7c9503b838 SPARC: drivers: remove __dev* attributes.
CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Pull big execve/kernel_thread/fork unification series from Al Viro:
 "All architectures are converted to new model.  Quite a bit of that
  stuff is actually shared with architecture trees; in such cases it's
  literally shared branch pulled by both, not a cherry-pick.

  A lot of ugliness and black magic is gone (-3KLoC total in this one):

   - kernel_thread()/kernel_execve()/sys_execve() redesign.

     We don't do syscalls from kernel anymore for either kernel_thread()
     or kernel_execve():

     kernel_thread() is essentially clone(2) with callback run before we
     return to userland, the callbacks either never return or do
     successful do_execve() before returning.

     kernel_execve() is a wrapper for do_execve() - it doesn't need to
     do transition to user mode anymore.

     As a result kernel_thread() and kernel_execve() are
     arch-independent now - they live in kernel/fork.c and fs/exec.c
     resp.  sys_execve() is also in fs/exec.c and it's completely
     architecture-independent.

   - daemonize() is gone, along with its parts in fs/*.c

   - struct pt_regs * is no longer passed to do_fork/copy_process/
     copy_thread/do_execve/search_binary_handler/->load_binary/do_coredump.

   - sys_fork()/sys_vfork()/sys_clone() unified; some architectures
     still need wrappers (ones with callee-saved registers not saved in
     pt_regs on syscall entry), but the main part of those suckers is in
     kernel/fork.c now."

Yinghai Lu 961f8fa04b sparc, mm: Remove calling of free_all_bootmem_node()
Now NO_BOOTMEM version free_all_bootmem_node() does not really
do free_bootmem at all, and it only call
register_page_bootmem_info_node instead.

That is confusing, try to kill that free_all_bootmem_node().

Before that, this patch will remove calling of free_all_bootmem_node()

We add register_page_bootmem_info() to call register_page_bootmem_info_node
directly.

Also could use free_all_bootmem() for numa case, and it is just
the same as free_low_memory_core_early().

Move that sucker to just before TI_FPDEPTH and replace stb with sth in
etrap_save().  Take current_ds to its old place, so that we don't push
wsaved into TI_... flags.  That allows to lose clearing syscall_noerror
on return from syscall.

David S. Miller f88620b9c5 sparc64: Fix deficiencies in sun4v error reporting.
Missing error types, attributes, and report fields.  Pad out
to 64-bytes.

Make string reporting cleaner and easier to extend in the future using
"const char *" arrays that index by either bit position, or absolute
field value.

Report the raw 64-byte error report as a sequence of u64s before the
annotated version.

Only report fields which are valid, given the context and the
attribute bits which are set.

For shutdown requests, use the local copy of the error report not the
one we just freed up back to the queue.  Also, use orderly_poweroff()
just like the Domain Services shutdown request code does.

If the real-address reported is "-1" (unknown) try to disassemble the
instruction to report the effective address of the access.  Only do
this in privileged mode.

David Miller 9e695d2ecc sparc64: Support transparent huge pages.
This is relatively easy since PMD's now cover exactly 4MB of memory.

Our PMD entries are 32-bits each, so we use a special encoding.  The
lowest bit, PMD_ISHUGE, determines the interpretation.  This is possible
because sparc64's page tables are purely software entities so we can use
whatever encoding scheme we want.  We just have to make the TLB miss
assembler page table walkers aware of the layout.

set_pmd_at() works much like set_pte_at() but it has to operate in two
page from a table of non-huge PTEs, so we have to queue up TLB flushes
based upon what mappings are valid in the PTE table.  In the second regime
we are going from huge-page to non-huge-page, and in that case we need
only queue up a single TLB flush to push out the huge page mapping.

We still have 5 bits remaining in the huge PMD encoding so we can very
likely support any new pieces of THP state tracking that might get added
in the future.

David Miller c460bec78d sparc64: Eliminate PTE table memory wastage.
We've split up the PTE tables so that they take up half a page instead of
a full page.  This is in order to facilitate transparent huge page
support, which works much better if our PMDs cover 4MB instead of 8MB.

What we do is have a one-behind cache for PTE table allocations in the
mm struct.

This logic triggers only on allocations.  For example, we don't try to
keep track of free'd up page table blocks in the style that the s390 port
does.

There were only two slightly annoying aspects to this change:

1) Changing pgtable_t to be a "pte_t *".  There's all of this special
   logic in the TLB free paths that needed adjustments, as did the
   PMD populate interfaces.

2) init_new_context() needs to zap the pointer, since the mm struct
   just gets copied from the parent on fork.

David Miller 15b9350a17 sparc64: Only support 4MB huge pages and 8KB base pages.
Narrowing the scope of the page size configurations will make the
transparent hugepage changes much simpler.

In the end what we really want to do is have the kernel support multiple
huge page sizes and use whatever is appropriate as the context dictactes.

Shaohua Li 45cac65b0f readahead: fault retry breaks mmap file read random detection
.fault now can retry.  The retry can break state machine of .fault.  In
filemap_fault, if page is miss, ra->mmap_miss is increased.  In the second
try, since the page is in page cache now, ra->mmap_miss is decreased.  And
these are done in one fault, so we can't detect random mmap file access.

Add a new flag to indicate .fault is tried once.  In the second try, skip
ra->mmap_miss decreasing.  The filemap_fault state machine is ok with it.

I only tested x86, didn't test other archs, but looks the change for other
archs is obvious, but who knows :)

Akinobu Mita 5da444aae5 sparc: fix format string argument for prom_printf()
prom_printf() takes printf style arguments.  Specifing GCC's format
attribute reveals that there are several wrong usages of prom_printf().

This fixes those wrong format strings and arguments, and also leaves
format attributes in order to detect similar mistakes at compile time.

David S. Miller c69ad0a3f7 sparc64: Use cpu_pgsz_mask for linear kernel mapping config.
This required a little bit of reordering of how we set up the memory
management early on.

We now only know the final values of kern_linear_pte_xor[] after we
take over the trap table and start processing TLB misses ourselves.

So once we fill those values in we re-clear the kernel's 4M TSB and
flush the TLBs.  That way if we find we support larger than 4M pages
we won't have any stale smaller page size entries in the TSB.

SUN4U Panther support for larger page sizes should now be extremely
trivial but I have no hardware on which to test it and I believe
that some of the sun4u TLB miss assembler needs to be audited first
to make sure it really can handle larger than 4M PTEs properly.

On sun4v, interrogate the machine description.  This code is extremely
defensive in nature, and a lot of the checks can probably be removed.

On sun4u things are a lot simpler.  There are the page sizes all chips
support, and then Panther adds 32MB and 256MB pages.

Report the probed value in /proc/cpuinfo

David S. Miller 4f93d21d25 sparc64: Support 2GB and 16GB page sizes for kernel linear mappings.
SPARC-T4 supports 2GB pages.

So convert kpte_linear_bitmap into an array of 2-bit values which
index into kern_linear_pte_xor.

Now kern_linear_pte_xor is used for 4 page size aligned regions,
4MB, 256MB, 2GB, and 16GB respectively.

Enabling 2GB pages is currently hardcoded using a check against
sun4v_chip_type.  In the future this will be done more cleanly
by interrogating the machine description which is the correct
way to determine this kind of thing.

David S. Miller 2856cc2e4d sparc64: Be less verbose during vmemmap population.
On a 2-node machine with 256GB of ram we get 512 lines of
console output, which is just too much.

This mimicks Yinghai Lu's x86 commit c2b91e2eec
(x86_64/mm: check and print vmemmap allocation continuous) except that
we aren't ever going to get contiguous block pointers in between calls
so just print when the virtual address or node changes.

This decreases the output by an order of 16.

Also demote this to KERN_DEBUG.

sparc32 does not support fixmaps - so do not pretend so by
having the fixmap.h file.
Move relevant parts to vaddrs.h.

I looked at simplifying this even more but failed to understand
the reasoning behind the extra guard page involved and due to

David S. Miller 5d83d66635 sparc32: Move cache and TLB flushes over to method ops.
This eliminated most of the remaining users of btfixup.

There are some complications because of the special cases we
have for sun4d, leon, and some flavors of viking.

It was found that there are no cases where a flush_page_for_dma
method was not hooked up to something, so the "noflush" iommu
methods were removed.

Add some documentation to the viking_sun4d_smp_ops to describe exactly
the hardware bug which causes us to need special TLB flushing on
sun4d.

This set of changes displays one major danger of btfixup, interface
signatures are not always type checked fully.  As seen here the iounit
variant of the map_dma_area routine had an incorrect type for one of
it's arguments.

It turns out to be harmless in this case, but just imagine trying to
debug something involving this kind of problem.  No thanks.

These were used on sun4c during floppy data transfers since on that
chip we had to lock the cpu mappings into the TLB because we cannot
take a TLB miss during the assembler floppy interrupt handler that
does the data transfer.

That is no longer necessary since we've removed sun4c support, thus
this stuff can disappear completely.

either BTFIXUPCALL_SWAPO0G0 or BTFIXUPCALL_NORM.

What a mess, glad we could untangle this stuff.

We used to runtime patch the trap table for srmmu.
With the removal of sun4c support this is no longer required.

With the sun4c trap removed we can remove all the referenced
trap handling which is sun4c specific.
This also allows us to get rid of the nosun4c.c file that
contained only dummy functions/data.

Machines with sun4c support are very rare these days, and noone
is using them for any practical purposes.
The sun4c support has been know broken for quite some time too.

So rather than trying to keep it up-to-date, lets get rid of it.
This allows us to do some very welcome cleanup of sparc32 support.

Updated the former sun4c specifc nmi (which was also used
for sun4m UP) to be a generic UP NMI.

David S. Miller a5a737e090 sparc64: Do not clobber %g2 in xcall_fetch_glob_regs().
%g2 is meant to hold the CPUID number throughout this routine, since
at the very beginning, and at the very end, we use %g2 to calculate
indexes into per-cpu arrays.

However we erroneously clobber it in order to hold the %cwp register
value mid-stream.

Fix this code to use %g3 for the %cwp read and related calulcations
instead.

Paul Gortmaker aa6f079075 sparc: fix build fail in mm/init_64.c when NEED_MULTIPLE_NODES is off
Commit 625d693e97 (linux-next)

    "sparc64: Convert over to NO_BOOTMEM."

causes the following compile failure for sparc64 allnoconfig:

  arch/sparc/mm/init_64.c:822:16: error: unused variable 'paddr'
  arch/sparc/mm/init_64.c:1759:7: error: unused variable 'node'
  arch/sparc/mm/init_64.c:809:12: error: 'memblock_nid_range' defined but not used

The paddr decl can easily be shuffled within the ifdef.  The
memblock_nid_range is just a stub function for when NEED_MULTIPLE_NODES
is off, but the only caller is within a NEED_MULTIPLE_NODES enabled
section, so we can simply delete it.

The unused "node" is slightly more interesting.  In the case of
"# CONFIG_NEED_MULTIPLE_NODES is not set" we no longer get the
definition of:

 #define NODE_DATA(nid)          (node_data[nid])

from arch/sparc/include/asm/mmzone.h - but instead we get:

 #define NODE_DATA(nid)          (&contig_page_data)

from include/linux/mmzone.h -- and since the arg is ignored,
the thing really is unused.  Rather than put in a confusing
looking __maybe_unused, simply splitting the declaration
from the assignment seemed to me to be the least offensive.

memblock_init() initializes arrays for regions and memblock itself;
however, all these can be done with struct initializers and
memblock_init() can be removed.  This patch kills memblock_init() and
initializes memblock with struct initializer.

The only difference is that the first dummy entries don't have .nid
set to MAX_NUMNODES initially.  This doesn't cause any behavior
difference.

The LEON MMU Model (SRMMU) does not implement MMu Table probing
in hardware, instead it is implemented in software. However the
software implementation does not return the PTE as it should which
always results in INVALID entires and the PROM mappings are not
inherited as they should during startup. The following patch
removes the masking of the PTE.

David S. Miller f4142cba4e sparc64: Force the execute bit in OpenFirmware's translation entries.
In the OF 'translations' property, the template TTEs in the mappings
never specify the executable bit.  This is the case even though some
of these mappings are for OF's code segment.

Therefore, we need to force the execute bit on in every mapping.

This problem can only really trigger on Niagara/sun4v machines and the
history behind this is a little complicated.

Previous to sun4v, the sun4u TTE entries lacked a hardware execute
permission bit.  So OF didn't have to ever worry about setting
anything to handle executable pages.  Any valid TTE loaded into the
I-TLB would be respected by the chip.

But sun4v Niagara chips have a real hardware enforced executable bit
in their TTEs.  So it has to be set or else the I-TLB throws an
instruction access exception with type code 6 (protection violation).

We've been extremely fortunate to not get bitten by this in the past.

The best I can tell is that the OF's mappings for it's executable code
were mapped using permanent locked mappings on sun4v in the past.
Therefore, the fact that we didn't have the exec bit set in the OF
translations we would use did not matter in practice.

With the recent mmu_gather changes that included generic RCU freeing of
page-tables, it is now quite straightforward to implement gup_fast() on
sparc64.

This patch:

Remove the page table quicklists.  They are pointless and make it harder
to use RCU page table freeing and share code with other architectures.

BTW, this is the second time this has happened, see commit 3c93646524
("[SPARC64]: Kill pgtable quicklists and use SLAB.")

Tejun Heo f9b18db3b1 memblock: Don't allow archs to override memblock_nid_range()
memblock_nid_range() is used to implement memblock_[try_]alloc_nid().
The generic version determines the range by walking early_node_map
with for_each_mem_pfn_range().  The generic version is defined __weak
to allow arch override.

Currently, only sparc overrides it; however, with the previous update
to the generic implementation, there isn't much to be gained with arch
override.  Sparc would behave exactly the same with the generic
implementation.

This patch disallows arch override for memblock_nid_range() and make
both generic and sparc versions static.

sparc is only compile tested.

Peter Zijlstra a8b0ca17b8 perf: Remove the nmi parameter from the swevent and overflow interface
The nmi parameter indicated if we could do wakeups from the current
context, if not, we would set some state and self-IPI and let the
resulting interrupt do the wakeup.

For the various event classes:

  - hardware: nmi=0; PMI is in fact an NMI or we run irq_work_run from
    the PMI-tail (ARM etc.)
  - tracepoint: nmi=0; since tracepoint could be from NMI context.
  - software: nmi=[0,1]; some, like the schedule thing cannot
    perform wakeups, and hence need 0.

As one can see, there is very little nmi=1 usage, and the down-side of
not using it is that on some platforms some software events can have a
jiffy delay in wakeup (when arch_irq_work_raise isn't implemented).

The up-side however is that we can remove the nmi parameter and save a
bunch of conditionals in fast paths.

Rework the sparc mmu_gather usage to conform to the new world order :-)

Sparc mmu_gather does two things:
 - tracks vaddrs to unhash
 - tracks pages to free

Split these two things like powerpc has done and keep the vaddrs
in per-cpu data structures and flush them on context switch.

The remaining bits can then use the generic mmu_gather.

David Rientjes 7bf02ea22c arch, mm: filter disallowed nodes from arch specific show_mem functions
Architectures that implement their own show_mem() function did not pass
the filter argument to show_free_areas() to appropriately avoid emitting
the state of nodes that are disallowed in the current context.  This patch
now passes the filter argument to show_free_areas() so those nodes are now
avoided.

This patch also removes the show_free_areas() wrapper around
__show_free_areas() and converts existing callers to pass an empty filter.

ia64 emits additional information for each node, so skip_free_areas_zone()
must be made global to filter disallowed nodes and it is converted to use
a nid argument rather than a zone for this use case.

David Rientjes b2b755b5f1 lib, arch: add filter argument to show_mem and fix private implementations
Commit ddd588b5dd ("oom: suppress nodes that are not allowed from
meminfo on oom kill") moved lib/show_mem.o out of lib/lib.a, which
resulted in build warnings on all architectures that implement their own
versions of show_mem():

The fix is to remove __show_mem() and add its argument to show_mem() in
all implementations to prevent this breakage.

Architectures that implement their own show_mem() actually don't do
anything with the argument yet, but they could be made to filter nodes
that aren't allowed in the current context in the future just like the
generic implementation.

pte alloc routines must wait for split_huge_page if the pmd is not present
and not null (i.e.  pmd_trans_splitting).  The additional branches are
optimized away at compile time by pmd_trans_splitting if the config option
is off.  However we must pass the vma down in order to know the anon_vma
lock to wait for.

Peter Zijlstra 20273941f2 mm: fix race in kunmap_atomic()
Christoph reported a nice splat which illustrated a race in the new stack
based kmap_atomic implementation.

The problem is that we pop our stack slot before we're completely done
resetting its state -- in particular clearing the PTE (sometimes that's
CONFIG_DEBUG_HIGHMEM).  If an interrupt happens before we actually clear
the PTE used for the last slot, that interrupt can reuse the slot in a
dirty state, which triggers a BUG in kmap_atomic().

Fix this by introducing kmap_atomic_idx() which reports the current slot
index without actually releasing it and use that to find the PTE and delay
the _pop() until after we're completely done.

Peter Zijlstra 3e4d3af501 mm: stack based kmap_atomic()
Keep the current interface but ignore the KM_type and use a stack based
approach.

The advantage is that we get rid of crappy code like:

	#define __KM_PTE			\
		(in_nmi() ? KM_NMI_PTE : 	\
		 in_irq() ? KM_IRQ_PTE :	\
		 KM_PTE0)

and in general can stop worrying about what context we're in and what kmap
slots might be appropriate for that.

The downside is that FRV kmap_atomic() gets more expensive.

For now we use a CPP trick suggested by Andrew:

  #define kmap_atomic(page, args...) __kmap_atomic(page)

to avoid having to touch all kmap_atomic() users in a single patch.

David S. Miller 9088333e3d sparc32: Fix unaligned stack handling on trap return.
When the rett stack checking code sees the stack is unaligned (in both
the sun4c and srmmu cases) it jumps to the window fault-in path.

But that just tries to page the stack pages in, it doesn't do anything
special if the stack is misaligned.

Therefore we essentially just loop forever in the trap return path.

Fix this by emitting a SIGILL in the stack fault-in code if the stack
is mis-aligned.

Yinghai Lu c7fc2de0c8 memblock, bootmem: Round pfn properly for memory and reserved regions
We need to round memory regions correctly -- specifically, we need to
round reserved region in the more expansive direction (lower limit
down, upper limit up) whereas usable memory regions need to be rounded
in the more restrictive direction (lower limit up, upper limit down).

This introduces two set of inlines:

	memblock_region_memory_base_pfn()
	memblock_region_memory_end_pfn()
	memblock_region_reserved_base_pfn()
	memblock_region_reserved_end_pfn()

Although they are antisymmetric (and therefore are technically
duplicates) the use of the different inlines explicitly documents the
programmer's intention.

The lack of proper rounding caused a bug on ARM, which was then found
to also affect other architectures.

Cesar Eduardo Barros 597781f3e5 kmap_atomic: make kunmap_atomic() harder to misuse
kunmap_atomic() is currently at level -4 on Rusty's "Hard To Misuse"
list[1] ("Follow common convention and you'll get it wrong"), except in
some architectures when CONFIG_DEBUG_HIGHMEM is set[2][3].

kunmap() takes a pointer to a struct page; kunmap_atomic(), however, takes
takes a pointer to within the page itself.  This seems to once in a while
trip people up (the convention they are following is the one from
kunmap()).

Make it much harder to misuse, by moving it to level 9 on Rusty's list[4]
("The compiler/linker won't let you get it wrong").  This is done by
refusing to build if the type of its first argument is a pointer to a
struct page.

The real kunmap_atomic() is renamed to kunmap_atomic_notypecheck()
(which is what you would call in case for some strange reason calling it
with a pointer to a struct page is not incorrect in your code).

The previous version of this patch was compile tested on x86-64.

David S. Miller c87fe1c05d sparc32: Kill none_mask, it's bogus.
For some reason, the pte_none() calculation for srmmu sparc32
chips was masking out the top 4 bits.  That doesn't make any
sense, as those are just some of the physical bits of the PTE
encoding.

Furthermore, this mistake breaks things when the offset of of a swap
entry has a large enough offset as reported by Тхай Кирилл.

Sun4c always set it to zero, so it's really completely useless,
kill it.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Russell King 4b3073e1c5 MM: Pass a PTE pointer to update_mmu_cache() rather than the PTE itself
On VIVT ARM, when we have multiple shared mappings of the same file
in the same MM, we need to ensure that we have coherency across all
copies.  We do this via make_coherent() by making the pages
uncacheable.

This used to work fine, until we allowed highmem with highpte - we
now have a page table which is mapped as required, and is not available
for modification via update_mmu_cache().

Ralf Beache suggested getting rid of the PTE value passed to
update_mmu_cache():

  On MIPS update_mmu_cache() calls __update_tlb() which walks pagetables
  to construct a pointer to the pte again.  Passing a pte_t * is much
  more elegant.  Maybe we might even replace the pte argument with the
  pte_t?

Ben Herrenschmidt would also like the pte pointer for PowerPC:

  Passing the ptep in there is exactly what I want.  I want that
  -instead- of the PTE value, because I have issue on some ppc cases,
  for I$/D$ coherency, where set_pte_at() may decide to mask out the
  _PAGE_EXEC.

So, pass in the mapped page table pointer into update_mmu_cache(), and
remove the PTE value, updating all implementations and call sites to
suit.

David S. Miller bffbc94a4d sparc64: Fix definition of VMEMMAP_SIZE.
This was the cause of various boot failures on V480, V880, etc.
systems.

Kernel image memory was being overwritten because the vmemmap[]
array was being sized to small.  So if you had physical memory
addresses past a certain point, the early bootup would spam
all over variables in the kernel data section.

The vmemmap mappings map page structs, not page struct pointers.
And that was the key thinko in the macro definition.

This was fixable thanks to the help, reports, and tireless patience
of Hermann Lauer.

David S. Miller 1a78cedb99 sparc64: Fix D-cache flushing on swapin from SW devices.
Thanks to tip form ARM folks and Russell King.

If flush_dcache_page() occurs on a swapin it will have a mapping
and we'll try to defer the flush by setting the dirty bit.

But when it hits update_dcache_page() we won't flush because the
page won't have a mapping any more.  So remove the mapping
requirement in flush_dcache().

David S. Miller d8ed1d43e1 sparc64: Validate linear D-TLB misses.
When page alloc debugging is not enabled, we essentially accept any
virtual address for linear kernel TLB misses.  But with kgdb, kernel
address probing, and other facilities we can try to access arbitrary
crap.

So, make sure the address we miss on will translate to physical memory
that actually exists.

In order to make this work we have to embed the valid address bitmap
into the kernel image.  And in order to make that less expensive we
make an adjustment, in that the max physical memory address is
decreased to "1 << 41", even on the chips that support a 42-bit
physical address space.  We can do this because bit 41 indicates
"I/O space" and thus covers non-memory ranges.

The result of this is that:

1) kpte_linear_bitmap shrinks from 2K to 1K in size

2) we need 64K more for the valid address bitmap

We can't let the valid address bitmap be dynamically allocated
once we start using it to validate TLB misses, otherwise we have
crazy issues to deal with wrt. recursive TLB misses and such.

If we're in a TLB miss it could be the deepest trap level that's legal
inside of the cpu.  So if we TLB miss referencing the bitmap, the cpu
will be out of trap levels and enter RED state.

To guard against out-of-range accesses to the bitmap, we have to check
to make sure no bits in the physical address above bit 40 are set.  We
could export and use last_valid_pfn for this check, but that's just an
unnecessary extra memory reference.

On the plus side of all this, since we load all of these translations
into the special 4MB mapping TSB, and we check the TSB first for TLB
misses, there should be absolutely no real cost for these new checks
in the TLB miss path.

This allows the callers to now pass down the full set of FAULT_FLAG_xyz
flags to handle_mm_fault().  All callers have been (mechanically)
converted to the new calling convention, there's almost certainly room
for architectures to clean up their code and then add FAULT_FLAG_RETRY
when that support is added.

Commit b696fdc259 ("sparc64: Defer
cpu_data() setup until end of per-cpu data initialization.") broke
bootup for UP builds because the cpu_data() initialization only
occurs in setup_per_cpu_areas() which is never compiled in nor
called in UP builds.

Fix this up by calling the setups directly from init_64.c when
non-SMP.

Stephen Rothwell 6ac5c61082 sparc: replace uses of CPU_MASK_ALL_PTR
CPU_MASK_ALL is the (deprecated) "all bits set" cpumask, defined as so:

	#define CPU_MASK_ALL (cpumask_t) { { ... } }

Taking the address of such a temporary is questionable at best,
unfortunately 321a8e9d (cpumask: add CPU_MASK_ALL_PTR macro) added
CPU_MASK_ALL_PTR:

	#define CPU_MASK_ALL_PTR (&CPU_MASK_ALL)

Which formalizes this practice.  One day gcc could bite us over this
usage (though we seem to have gotten away with it so far).

Rusty Russell ad6561dffa module: trim exception table on init free.
It's theoretically possible that there are exception table entries
which point into the (freed) init text of modules.  These could cause
future problems if other modules get loaded into that memory and cause
an exception as we'd see the wrong fixup.  The only case I know of is
kvm-intel.ko (when CONFIG_CC_OPTIMIZE_FOR_SIZE=n).

Amerigo fixed this long-standing FIXME in the x86 version, but this
patch is more general.

This implements trim_init_extable(); most archs are simple since they
use the standard lib/extable.c sort code.  Alpha and IA64 use relative
addresses in their fixups, so thier trimming is a slight variation.

Sparc32 is unique; it doesn't seem to define ARCH_HAS_SORT_EXTABLE,
yet it defines its own sort_extable() which overrides the one in lib.
It doesn't sort, so we have to mark deleted entries instead of
actually trimming them.

Upon further consideration, we actually should never see any
fault addresses for 32-bit tasks with the upper 32-bits set.

If it does every happen, by definition it's a bug.  Whatever
context created that fault would only have that fault satisfied
if we used the full 64-bit address.  If we truncate it, we'll
always fault the wrong address and we'll always loop faulting
forever.

So catch such conditions and mark them as errors always.  Log
the error and fail the fault.

David S. Miller eeabac7386 sparc64: Validate kernel generated fault addresses on sparc64.
In order to handle all of the cases of address calculation overflow
properly, we run sparc 32-bit processes in "address masking" mode
when running on a 64-bit kernel.

Address masking mode zeros out the top 32-bits of the address
calculated for every load and store instruction.

However, when we're in privileged mode we have to run with that
address masking mode disabled even when accessing userspace from
the kernel.

To "simulate" the address masking mode we clear the top-bits by
hand for 32-bit processes in the fault handler.

It is the responsibility of code in the compat layer to properly
zero extend addresses used to access userspace.  If this isn't
followed properly we can get into a fault loop.

Say that the user address is 0xf0000000 but for whatever reason
the kernel code sign extends this to 64-bit, and then the kernel
tries to access the result.

In such a case we'll fault on address 0xfffffffff0000000 but the fault
handler will process that fault as if it were to address 0xf0000000.
We'll loop faulting forever because the fault never gets satisfied.

So add a check specifically for this case, when the kernel is faulting
on a user address access and the addresses don't match up.

This code path is sufficiently slow path, and this bug is sufficiently
painful to diagnose, that this kind of bug check is warranted.

Sam Ravnborg 0157141ae2 sparc: refactor code in fault_32.c
The sparc allmodconfig build broke due to enabling of the
branch_tracer that does some very clever things with
all if conditions. This caused my gcc 3.4.5 to be so confused that
it emitted a warning:

arch/sparc/mm/fault_32.c:176: warning: 'fixup' might be used uninitialized in this function

And with -Werror this broke the build.

Refactor code so it:
1) becomes more readable
2) no longer emit a warning with the branch_tracer enabled

Sam Ravnborg ff9aefbf4d sparc64: refactor code in init_64.c
The sparc64 allmodconfig build broke due to enabling of the
branch_tracer that does some very clever things with
all if conditions. This caused my gcc 3.4.5 to be so confused that
it emitted two warnings:

arch/sparc/mm/init_64.c: In function `update_mmu_cache':
arch/sparc/mm/init_64.c:271: warning: 'pg_flags' might be used uninitialized in this function
arch/sparc/mm/init_64.c:272: warning: 'page' might be used uninitialized in this function

And with -Werror this broke the build.

Refactor code so it:
1) becomes more readable
2) no longer emit a warning with the branch_tracer enabled

The refactoring uses a small helper function (flush_dcache()).

David S. Miller 0871420fad sparc64: Add tsb-ratio sysctl.
Add a sysctl to tweak the RSS limit used to decide when to grow
the TSB for an address space.

In order to avoid expensive divides and multiplies only simply
positive and negative powers of two are supported.

The function computed takes the number of TSB translations that will
fit at one time in the TSB of a given size, and either adds or
subtracts a percentage of entries.  This final value is the
RSS limit.

See tsb_size_to_rss_limit().

Al Viro 409832f548 sparc32 cpuinit flase positives
All noise since we don't have CPU hotplug there.  However, they
did expose something very odd-looking in there - poke_viking()
does a bunch of identical btfixup each time it's called (i.e.
for each CPU).  That one is left alone for now; just the trivial
misannotation fixes.

Adrian Bunk 5110bd21b8 sparc: remove CONFIG_SUN4
While doing some easy cleanups on the sparc code I noticed that the
CONFIG_SUN4 code seems to be worse than the rest - there were some
"I don't know how it should work, but the current code definitely cannot
work." places.

And while I have seen people running Linux on machines like a
SPARCstation 5 a few years ago I don't recall having seen sun4
machines, even less ones running Linux.

And stick the iommu archdata pointer into the generic OF device tree
of_device struct as well.

We still have to pass the sbus_bus object down into the routines so
that the SBUS bus objects get the iommu cookies set properly.  After
drivers get converted to being pure OF drivers, that can go away.

Johannes Weiner 9109fb7b35 mm: drop unneeded pgdat argument from free_area_init_node()
free_area_init_node() gets passed in the node id as well as the node
descriptor.  This is redundant as the function can trivially get the node
descriptor itself by means of NODE_DATA() and the node's id.

I checked all the users and NODE_DATA() seems to be usable everywhere
from where this function is called.

The code in arch/sparc/prom/memory.c computes three tables, the list
of total memory, the list of available memory (total minus what
firmware is using), and the list of firmware taken memory.

Only the available memory list is even used.

Therefore, kill those unused tables and make prom_meminfo() return
just the available memory list.

Martin Schwidefsky 2f569afd9c CONFIG_HIGHPTE vs. sub-page page tables.
Background: I've implemented 1K/2K page tables for s390.  These sub-page
page tables are required to properly support the s390 virtualization
instruction with KVM.  The SIE instruction requires that the page tables
have 256 page table entries (pte) followed by 256 page status table entries
(pgste).  The pgstes are only required if the process is using the SIE
instruction.  The pgstes are updated by the hardware and by the hypervisor
for a number of reasons, one of them is dirty and reference bit tracking.
To avoid wasting memory the standard pte table allocation should return
1K/2K (31/64 bit) and 2K/4K if the process is using SIE.

Problem: Page size on s390 is 4K, page table size is 1K or 2K.  That means
the s390 version for pte_alloc_one cannot return a pointer to a struct
page.  Trouble is that with the CONFIG_HIGHPTE feature on x86 pte_alloc_one
cannot return a pointer to a pte either, since that would require more than
32 bit for the return value of pte_alloc_one (and the pte * would not be
accessible since its not kmapped).

Solution: The only solution I found to this dilemma is a new typedef: a
pgtable_t.  For s390 pgtable_t will be a (pte *) - to be introduced with a
later patch.  For everybody else it will be a (struct page *).  The
additional problem with the initialization of the ptl lock and the
NR_PAGETABLE accounting is solved with a constructor pgtable_page_ctor and
a destructor pgtable_page_dtor.  The page table allocation and free
functions need to call these two whenever a page table page is allocated or
freed.  pmd_populate will get a pgtable_t instead of a struct page pointer.
 To get the pgtable_t back from a pmd entry that has been installed with
pmd_populate a new function pmd_pgtable is added.  It replaces the pmd_page
call in free_pte_range and apply_to_pte_range.

Bernhard Walle 72a7fe3967 Introduce flags for reserve_bootmem()
This patchset adds a flags variable to reserve_bootmem() and uses the
BOOTMEM_EXCLUSIVE flag in crashkernel reservation code to detect collisions
between crashkernel area and already used memory.

This patch:

Change the reserve_bootmem() function to accept a new flag BOOTMEM_EXCLUSIVE.
If that flag is set, the function returns with -EBUSY if the memory already
has been reserved in the past.  This is to avoid conflicts.

Because that code runs before SMP initialisation, there's no race condition
inside reserve_bootmem_core().

Will Schmidt dcca2bde4f During VM oom condition, kill all threads in process group
We have had complaints where a threaded application is left in a bad state
after one of it's threads is killed when we hit a VM: out_of_memory
condition.

Killing just one of the process threads can leave the application in a bad
state, whereas killing the entire process group would allow for the
application to restart, or be otherwise handled, and makes it very obvious
that something has gone wrong.

This change allows the entire process group to be taken down, rather
than just the one thread.

Mark Fortescue 70b0e7a919 [SPARC32]: Remove superfluous 'kernel_end' alignment on sun4c.
In sun4c_init_clean_mmu(), aligning 'kernel_end' using
SUN4C_REAL_PGDIR_ALIGN() is unnecessary since the caller
does this already.

In sun4c_paging_init(), 4 page sizes of "fluff" were added
to the address of &end.  This was necessary a long time ago
when sparc32 would allocate some early data structures
by carving out memory chunks after &end but that no longer
occurs.

Mark Fortescue da4e9fea85 [SPARC]: Fix exec failures on sun4c.
This deals with a sun4c issue caused by commit b6a2fea39318e43fee84fa7b0b90d68bed92d2ba:
mm: variable length argument support.

The new way the code works means that sun4c_update_mmu_cache gets
called before a context has been selected, which results in invalid
operation of the underling mm code.

Simply ignoring update requests when there is no valid context solves
the problem.

Christoph Hellwig 1eeb66a1bb move die notifier handling to common code
This patch moves the die notifier handling to common code.  Previous
various architectures had exactly the same code for it.  Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at.  avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

Peter Zijlstra a866374aec [PATCH] mm: pagefault_{disable,enable}()
Introduce pagefault_{disable,enable}() and use these where previously we did
manual preempt increments/decrements to make the pagefault handler do the
atomic thing.

Currently they still rely on the increased preempt count, but do not rely on
the disabled preemption, this might go away in the future.

(NOTE: the extra barrier() in pagefault_disable might fix some holes on
       machines which have too many registers for their own good)

Dave McCracken 46a82b2d55 [PATCH] Standardize pxx_page macros
One of the changes necessary for shared page tables is to standardize the
pxx_page macros.  pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address.  pud_page and pgd_page, on the
other hand, return the kernel virtual address.

Shared page tables needs pud_page and pgd_page to return the actual page
structures.  There are very few actual users of these functions, so it is
simple to standardize their usage.

Since this is basic cleanup, I am submitting these changes as a standalone
patch.  Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

Bob Breuer 3185d4d287 [SPARC]: Fix iommu_flush_iotlb end address
Fix the calculation of the end address when flushing iotlb entries to
ram.  This bug has been a cause of esp dma errors, and it affects
HyperSPARC systems much worse than SuperSPARC systems.

Todo items:
 - IRQ_INPROGRESS flag - use sparc64 irq buckets, or generic irq_desc?
 - sun4d
 - re-indent large chunks of sun4m_smp.c
 - some places assume sequential cpu numbering (i.e. 0,1 instead of 0,2)

Last I checked (with 2.6.14), random programs segfault with dual
HyperSPARC.  And with SuperSPARC II's, it seems stable but will
eventually die from a write lock error (wrong lock owner or something).

I haven't tried the HyperSPARC + highmem combination recently, so that
may still be a problem.
