James Bottomley a6da74cb07 [SCSI] mpt fusion: clear list of outstanding commands on host reset
A bug in the fusion driver was exposed by the switch to block timeout.
Basically, drivers are supposed to terminate commands once error
handling begins on them.  The fusion apparently wasn't doing this.
Under the old timeout regime, completions on terminated commands would
by and large get ignored because of the way command timeouts used to
work. The new block timers are very intolerant to this, though,
becuase the request gets cleaned and freed.

Al Viro 233e70f422 saner FASYNC handling on file close
As it is, all instances of ->release() for files that have ->fasync()
need to remember to evict file from fasync lists; forgetting that
creates a hole and we actually have a bunch that *does* forget.

So let's keep our lives simple - let __fput() check FASYNC in
file->f_flags and call ->fasync() there if it's been set.  And lose that
crap in ->release() instances - leaving it there is still valid, but we
don't have to bother anymore.

Al Viro d4430d62fa [PATCH] beginning of methods conversion
To keep the size of changesets sane we split the switch by drivers;
to keep the damn thing bisectable we do the following:
	1) rename the affected methods, add ones with correct
prototypes, make (few) callers handle both.  That's this changeset.
	2) for each driver convert to new methods.  *ALL* drivers
are converted in this series.
	3) kill the old (renamed) methods.

Note that it _is_ a flagday; all in-tree drivers are converted and by the
end of this series no trace of old methods remain.  The only reason why
we do that this way is to keep the damn thing bisectable and allow per-driver
debugging if anything goes wrong.

New methods:
	open(bdev, mode)
	release(disk, mode)
	ioctl(bdev, mode, cmd, arg)		/* Called without BKL */

Alan Cox 9d793b0bcb i2o: Fix 32/64bit DMA locking
The I2O ioctls assume 32bits.  In itself that is fine as they are old
cards and nobody uses 64bit.  However on LKML it was noted this
assumption is also made for allocated memory and is unsafe on 64bit
systems.

Fixing this is a mess.  It turns out there is tons of crap buried in a
header file that does racy 32/64bit filtering on the masks.

So we:
- Verify all callers of the racy code can sleep (i2o_dma_[re]alloc)
- Move the code into a new i2o/memory.c file
- Remove the gfp_mask argument so nobody can try and misuse the function
- Wrap a mutex around the problem area (a single mutex is easy to do and
  none of this is performance relevant)
- Switch the remaining problem kmalloc holdout to use i2o_dma_alloc

Wang Chen 0293902a4d I2O: handle sysfs_create_link() failures
Compile warning:
ignoring return value of `sysfs_create_link', declared with attribute warn_unused_result.

If sysfs_create_link failed, take care of the return value and do some
error handle after the failure.

Since sysfs_remove_link() will check whether a link exists, when removing the
link in error path, we don't need to care whether a link was created.

Prakash, Sathya d54d48b80f [SCSI] mpt fusion : Adding FAULT Reset polling work
When the firmware is in Fault state it will be identifed only when the next time
the driver access the IOC state.
This patch includes a polling function in the driver which will be executed in
regular interval to check the status of the firmware and if it is in Fault
state, then the firmware will be reset by the driver.

James Bottomley 2789898817 [SCSI] fusion: default MSI to disabled for SPI and FC controllers
There's a fault on the FC controllers that makes them not respond
correctly to MSI.  The SPI controllers are fine, but are likely to be
onboard on older motherboards which don't handle MSI correctly, so
default both these cases to disabled.  Enable by setting the module
parameter mpt_msi_enable=1.

For the SAS case, enable MSI by default, but it can be disabled by
setting the module parameter mpt_msi_enable=0.

James Bottomley 081a5bcb39 [SCSI] mptspi: fix oops in mptspi_dv_renegotiate_work()
The problem here is that if the ioc faults too early in the bring up
sequence (as it usually does for an irq routing problem), ioc_reset gets
called before the scsi host is even allocated.  This causes an oops when
it later schedules a renegotiation.  Fix this by checking ioc->sh before
trying to renegotiate.

Michael Reed 7ba2db5f38 [SCSI] fusion mpt: fix target missing after resetting external raid
Following a hard reset of a SAS raid, one of the raid targets is occasionally
missing.  I tracked this down to a pretty obscure little bug.

The LSI fusion drivers for SAS and Fibre Channel both use their respective
transport layers.  Those transport layers increment the target number
assigned to new targets.

The routine __scsi_scan_target uses the "this_id" element of the Scsi_Host
structure to avoid scanning the scsi host adapter.  Both fusion drivers set
"this_id" from a value returned in a firmware PortFacts response.  For my
particular test case (SAS) the firmware id assigned to the initiator was
173.  After enough raid resets to cause the raid targets to go and come a
sufficient number of times, the id assigned by the transport to a raid
target would match the id assigned by the host adapter to the "this_id"
field, resulting in that target not being scanned.

Fix by not assigning this_id and not checking it in slave_configure.

Michael Reed 7748369f49 [SCSI] mptsas: do not use ioc->handle to locate hba portinfo structure
While performing hardware raid reset testing via the raid's client, I
noticed that sometimes, following the reset, that there would be more
raid targets in the lsscsi output than there actually were raid
targets.  I tracked this down to the following issue.

Fusion cannot always find the mptsas_portinfo structure for the hba
because it uses the handle stored in ioc->handle to locate it.  The
problem is that the firmware can change the handle associated with the
hba when h/w raid is reset (via the raid client).  When this happens,
the driver will allocate another mptsas_portinfo structure and link it
into the chain of said structures.  This ultimately causes confusion
within the driver resulting in targets not being removed when they
should be.

Eric Moore pointed out that the hba's portinfo structure is always the
first structure on the sas_topology list.  This patch modifies
mptsas.c to access the hba's portinfo structure by taking the first
structure on said list.

This patch modifies the I/O resource allocation behavior of FUSION
driver.  The current version of driver allocates the I/O resources
even if they are not required and this creates trouble in low resource
environments.  This driver now uses
pci_enable_device_mem/pci_enable_device functions to differentiate the
resource allocations.

Eric Moore e1fc2b5167 [SCSI] mptlan: bug fix, only half the message frame is dma'd resulting in corruption
NB = number of blocks.  This represents the number of blocks to
transfer.  The block size is based on the message frame size provided
in the ioc_facts. A value of zero indicates the entire message frame
should be copied. This is two bit value.  So by setting this to
non-zero vaule, you increase performance by reducing amount of data
needing to be dma'd.  The value that is stored in ioc->ReqeustNB is
sometimes a non-zero vaule, which creates a bug in mptlan, where not
the entire message frame is getting transfer to firware, resulting in
corruption.  This fix sets the default to zero, thus entire message
frame is copied.

Eric Moore e8206381f5 [SCSI] mpt fusion: lock down ScsiLookup
ScsiLookup is an array of pending scmd pointers that the scsi lld
maintains. This array is touched from queuecommand, eh threads, and
interrupt context. This array should put under locks, hence this patch
to synchronize its access.  I've added some nice little function
wrappers for this, and moved the ScsiLookup array over to MPT_ADAPTER
struct.

Jesper Juhl 568761955a [SCSI] mpt fusion: fix two potential mem leaks
The Coverity checker spotted two potential memory leaks in
drivers/message/fusion/mptbase.c::mpt_attach().

There are two returns that may leak the storage allocated for 'ioc'
(sizeof(MPT_ADAPTER) bytes).

A simple fix would be to simply add two kfree() calls before the return
statements, but a better fix (that this patch implements) is to reorder the
code so that if we hit the first return condition we don't have to do the
allocation at all and then just add a kfree() call for the second case.

It's been a useless no-op for long enough in 2.6 so I figured it's time to
remove it.  The number of people that could object because they're
maintaining unified 2.4 and 2.6 drivers is probably rather small.

msg->u.head[1] = cpu_to_le32(I2O_CMD_PRIVATE << 24 | HOST_TID << 12 | tid);
and I2O_CMD_PRIVATE is defined as 0xFF.  This gets "0xFF0100 | tid" and fits
into 32-bit unsigned but not into 32-bit signed integer properly.  Target
value is defined as u32 so the claculation does not fit during computation.

Change local variable tid to u32 so the whole expression is of u32 type and
fits well into u32 result.

Paul Mundt 20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

Vasily Averin 010904cbed wrong memory access in i2o_block_device_lock()
This patch fixes access to memory that has not been allocated:
i2o_msg_get_wait() can returns errors different from I2O_QUEUE_EMPTY.  But the
result is checked only against this code.  If it is not I2O_QUEUE_EMPTY then
we dereference the error code as the pointer later.

Vasily Averin 1725d71d99 i2o_cfg_passthru cleanup
This patch fixes a number of issues in i2o_cfg_passthru{,32}:
- i2o_msg_get_wait() return vaile is not checked;
- i2o_message memory leaks on error paths;
- infinite loop to sg_list_cleanup in passthru32

It's important issue because of i2o_cfg_passthru is used by raidutils for
monitorig controllers state, and in case of memory shortage it leads to the
node crash or disk IO stall.

Eric Moore cc78d30af0 [SCSI] mpt fusion: fix for mounted raid volume filesytem that goes read-only
If there is IO going to the volume while a hidden disk is being torn down,
there is a case where we would return a DID_NO_CONNECT for IO sent to the
volume. The end result is the volume goes read-only. This problem is due to the
fact the firmware mapped target ids saved in per device object is phys_disk_num
for hidden  raid components, and target_id for the volume. There is a single
case when both phys_disk_num and target_id are equal, so enters this issue. We
fix this issue by checking the tflags when the device is torned down,
insuring the IO being completed is meant for hidden raid component, not the
volume.  In addition to this fix, there are a couple other cases to address
hidden raid components. For instance task_abort and device reset are not
supported by mpt fw for hidden raid components, a bus reset would be required
or target reset to volume.

A recent code cleanup that moved code from mptscsih to mptspi
inadvertently change the order some code was called.  This caused
a massive slowdown (of 150x to 300x) on the CD/DVD drive on the

Akinobu Mita be324797d9 i2o: fix notifiers when max_drivers is configured
Maximum number of I2O drivers which could be registered is configurable by
max_drivers module parameter.

But the module parameter is ignored and default value (I2O_MAX_DRIVERS = 8)
is used in the loops to notify all registered drivers.

i2o_driver_register() initalizes event queue for driver only when
drv->event is set.  So similarly the event queue should be destroyed only
when drv->event is set in the error path.  Otherwise destroy_workqueue()
will called with NULL.

Eric Moore 7297824581 [SCSI] fusion: fix domain validation loops
After host reset, the device are programmed to default asyn narrow nego.
We need to reprogram the parameter back to previous values.  If the host
reset is called as a result of spi_dv_device() commands timing out, its
possible to get into an infinite loop of dv to host reset.  This will
prevent that case, as we merely program old values.  If host reset is
called outside context of domain validation, then we can  call
spi_dv_device.

For the places where we need a pointer to the mac header, it is still legal to
touch skb->mac.raw directly if just adding to, subtracting from or setting it
to another layer header.

regex missed as it had no spaces before nor after '=', ugh.

For the common, open coded 'skb->mac.raw = skb->data' operation, so that we can
later turn skb->mac.raw into a offset, reducing the size of struct sk_buff in
64bit land while possibly keeping it as a pointer on 32bit.

This one touches just the most simple case, next will handle the slightly more
"complex" cases.

Vasily Averin d1985ad1da [PATCH] i2o: block IO errors on i2o disk
I2O subsystem has been broken in mainstream several months ago (after
2.6.18).  Commit 4aff5e2333 from Jens
Axboe split struct request ->flags into two parts: cmd_type and
cmd_flags.

In i2o layer this patch has replaced flag REQ_SPECIAL by the according
cmd_type.  However i2o has used REQ_SPECIAL not as command type but as
driver-specific flag for the debug purposes.  As result all i2o requests
have type "special" now, are not processed to the hardware and fail with
I/O error:

   i2o/hda:<3>Buffer I/O error on device i2o/hda, logical block 0
  Buffer I/O error on device i2o/hda, logical block 0
  Buffer I/O error on device i2o/hda, logical block 0
   unable to read partition table
  block-osm: device added (TID: 207): i2o/hda

The following patch removes the extra debug checks without any drawbacks and
restores the normal driver's work.

Eric Moore ad8c31bb69 [SCSI] fusion: remove VMWare guest OS remounted as read only work around
This address the issue of VMWare guest OS being remounted as read-only
becuase the underlying device was held busy too long and at the
same time address Engenio MPP driver concerns over infinite retries.
This patch removes the code that snoops the SAM STATUS on busy, which
would be returning DID_BUS_BUSY, instead we return the status as is.
Retry hanlding seems to be properly handled in scsi_softirq_done,
where a busy sam status would only occurr for the time specified by
(cmd->allowed +1) * cmd->timeout_per_command.
