Bug 1170958 - Refactor DOMMediaStream to contain a 3-stage track chain. r=roc

This lets us separate tracks by ownership like so:
* Input - Owned by the producer of the DOMMediaStream (gUM etc.)
* Owned - Contains Input tracks (per above) or tracks cloned tracks
if this DOMMediaStream is a clone.
* Playback - Contains Owned tracks plus tracks addTrack()ed to this
DOMMediaStream minus tracks removeTrack()ed from this
DOMMediaStream.

Bug 1207741 - Remove gfxIntSize. r=nical.

gfxIntSize is just a typedef of gfx::IntSize, so this is very mechanical. The
only tricky part is deciding for each occurrence whether to replace it with
IntSize, gfx::IntSize or mozilla::gfx::IntSize; in all cases I went with the
shortest one that worked given the existing "using namespace" declarations.

This is straightforward mapping of PR_LOG levels to their LogLevel
counterparts:
PR_LOG_ERROR -> LogLevel::Error
PR_LOG_WARNING -> LogLevel::Warning
PR_LOG_WARN -> LogLevel::Warning
PR_LOG_INFO -> LogLevel::Info
PR_LOG_DEBUG -> LogLevel::Debug
PR_LOG_NOTICE -> LogLevel::Debug
PR_LOG_VERBOSE -> LogLevel::Verbose
Instances of PRLogModuleLevel were mapped to a fully qualified
mozilla::LogLevel, instances of PR_LOG levels in #defines were mapped to a
fully qualified mozilla::LogLevel::* level, and all other instances were
mapped to us a shorter format of LogLevel::*.
Bustage for usage of the non-fully qualified LogLevel were fixed by adding
|using mozilla::LogLevel;| where appropriate.
