Tejun Heo 72ec24bd77 SCSI: implement sd_unlock_native_capacity()
Implement sd_unlock_native_capacity() method which calls into
hostt->unlock_native_capacity() if implemented.  This will be invoked
by block layer if partitions extend beyond the end of the device and
can be used to implement, for example, on-demand ATA host protected
area unlocking.

Benjamin Herrenschmidt c2cdf6aba0 powerpc/macio: Fix probing of macio devices by using the right of match table
Grant patches added an of mach table to struct device_driver. However,
while he changed the macio device code to use that, he left the match
table pointer in struct macio_driver and didn't update drivers to use
the "new" one, thus breaking the probing.

This completes the change by moving all drivers to setup the "new"
one, removing all traces of the old one, and while at it (since it
changes the exact same locations), I also remove two other duplicates
from struct driver which are the name and owner fields.

st_open() suggests that llseek() doesn't work: "We really want to do
nonseekable_open(inode, filp); here, but some versions of tar incorrectly
call lseek on tapes and bail out if that fails.  So we disallow pread()
and pwrite(), but permit lseeks."

Instead of using the fallback default_llseek() the driver should use
noop_llseek() which leaves the file->f_pos untouched but succeeds.

osst_read()/osst_write() modify file->f_pos directly instead of the ppos
given to them.  The VFS later updates the file->f_pos and overwrites it

Alan Stern f9e8894ae5 [SCSI] fix race in scsi_target_reap
This patch (as1357) fixes a race in SCSI target allocation and
release.  Putting a target in the STARGET_DEL state isn't protected by
the host lock, so an old target structure could be reused by a new
device even though it's about to be deleted.  The cure is to change
the state while still holding the host lock.

Wayne Boyer 64ffdb7622 [SCSI] ipr: improve interrupt service routine performance
During performance testing on P7 machines it was observed that the interrupt
service routine was doing unnecessary MMIO operations.

This patch rearranges the logic of the routine and moves some of the code out
of the main routine.  The result is that there are now fewer MMIO operations in
the performance path of the code.

In bring up testing for the new 64 bit adapters, the first read command failed
after loading the driver.  The cause was that the command requires more than
one scatter gather element and the corresponding code to set the data list
length in the request control block was missing.  This patch adds the correct
assignment.

Wayne Boyer 7be96900a2 [SCSI] ipr: fix a register read to use the correct address for 64 bit adapters
Fix ipr_reset_enable_ioa() to read the correct IOA to host interrupt register
address for 64 bit adapters.  We need to read the lower 32 bits, not the upper
32 bits.

Also change the write of the 64 bit mask value to a single writeq instead
of two writel calls.

Finally, use the correct u8 type for the type field in the ipr_resource_entry
structure.

Dan Carpenter e6b50c30fa [SCSI] be2iscsi: correct return value in mgmt_invalidate_icds()
This function should return 0 on error.  Returning -1 would cause a
crash.

Also there is an extra space before the newline character and a missing
space between the "for" and the "mgmt_invalidate_icds".  I put the string
on one line.  The current version of checkpatch.pl complains that the
line is too long, but it makes grepping easier.

Grant Likely 4018294b53 of: Remove duplicate fields from of_platform_driver
.name, .match_table and .owner are duplicated in both of_platform_driver
and device_driver.  This patch is a removes the extra copies from struct
of_platform_driver and converts all users to the device_driver members.

This patch is a pretty mechanical change.  The usage model doesn't change
and if any drivers have been missed, or if anything has been fixed up
incorrectly, then it will fail with a compile time error, and the fixup
will be trivial.  This patch looks big and scary because it touches so
many files, but it should be pretty safe.

Linus Torvalds 70ca0a42fe Fix networking tree iscsi_tcp.c mis-merge
The removal of the 'waitqueue_active()' test in commit d7d05548a6
("[SCSI] iscsi_tcp: fix relogin/shutdown hang") got incorrectly resolved
by David when he back-merged the main git tree into the networking tree
in commit 278554bd65 ("Merge branch 'master' of master.kernel.org:...").

There was a content conflict due to 'sock->sk->sk_sleep' being changed
into 'sk_sleep(sock->sk)' in the networking tree, but David didn't pick
up the iscsi change from the main tree.

Tejun Heo 3e4ec3443f libata: kill ATA_FLAG_DISABLED
ATA_FLAG_DISABLED is only used by drivers which don't use
->error_handler framework and is largely broken.  Its only meaningful
function is to make irq handlers skip processing if the flag is set,
which is largely useless and even harmful as it makes those ports more
likely to cause IRQ storms.

Kill ATA_FLAG_DISABLED and makes the callers disable attached devices
instead.  ata_port_probe() and ata_port_disable() which manipulate the
flag are also killed.

This simplifies condition check in IRQ handlers.  While updating IRQ
handlers, remove ap NULL check as libata guarantees consecutive port
allocation (unoccupied ports are initialized with dummies) and
long-obsolete ATA_QCFLAG_ACTIVE check (checked by ata_qc_from_tag()).

Problem description:

The problem reported by one of the customer was when a logical array
is deleted(from the SDK, from the GUI, from arcconf) then the
corresponding physical device (/dev/sdb, for example) is not removed
from the Linux namespace. So you end up with a "dead" device
entry. And some of the linux tools go slightly wonky.

Solution:
---------

Based on the notification from FW, the driver calls
"scsi_remove_device" for the DELETED drive. This call not only informs
the scsi device status to the SCSI mid layer and also it will remove
corresponding scsi device entries from the Linux sysfs.

Problem description:

The issue reported by one of the customer was able to read LBA beyond
the array reported size with "sg_read" utility. If N is the last block
address reported, then should not be able to read past N,
i.e. N+1. But in their case, reported last LBA=143134719.  So should
not have been able to read with LBA=143134720, but it is read without
failure, which means reported size to the OS is not correct and is
less than the actual last block address.

Solution:
---------

Firmware layer exposes lesser container capacity than the actual
one. It exposes [Actual size - Spitfire space(10MB)] to the OS, IO's
to the 10MB should be prohibited from the Linux driver. Driver checks
LBA boundary, if its greater than the array reported size then sets
sensekey to HARDWARE_ERROR and sends the notification to the MID
layer.

'SRB_STATUS_ERROR' condition.

1. When the "CC" bit is set by the host in ATA pass-through CDB

   - Even for the successful completion, SCSI target shall generate
     check condition.

   - Driver returns a result code of SAM_STAT_CHECK_CONDITION, with a
     driver byte of DID_OK to the mid layer.

     Below is the snippet of existing code which fills a result code
     of SAM_STAT_CHECK_CONDITION:

Rajashekhara, Mahesh e3cc268fe4 [SCSI] aacraid: expose physical devices for models with newer firmware
The default driver setting is "expose_physicals=0", which means raw
physical drives are not exposed to OS.  If the user wants to expose
connected physical drives, enable "expose_physicals" module parameter.
With the new JBOD firmware, physical drives are not available for
"expose_physicals>0".  In function "aac_expose_phy_device", modified
to reset the appropriate bit in the first byte of inquiry data.  This
fix exposes the connected physical drives.

Vasu Dev 15af974dcb [SCSI] fcoe: fix fcoe module ref counting
Currently fcoe module ref count is used for tracking
active fcoe instances, it means each fcoe instance create
increments the count while destroy dec the count.

The dec is done only if fcoe instance is destroyed from
/sysfs but not if destroyed due to NETDEV_UNREGISTER event.
So this patch moves only module_put doing dec to common
fcoe_if_destroy function, so that dec would occur on ever
fcoe instance destroy.

Vasu Dev 34ce27bcf9 [SCSI] fcoe: fix a circular locking issue with rtnl and sysfs mutex
Currently rtnl mutex is grabbed during fcoe create, destroy, enable
and disable operations while sysfs s_active read mutex is already
held, but simultaneously other networking events could try grabbing
write s_active mutex while rtnl is already held and that is causing
circular lock warning, its detailed log pasted at end.

In this log, the rtnl was held before write s_active during device
renaming but there are more such cases as Joe reported another
instance with tg3 open at:-

This patch fixes this issue by not waiting for rtnl mutex during
fcoe ops, that means if rtnl mutex is not immediately available
then restart_syscall() to allow others waiting in line to
grab s_active along with rtnl mutex to finish their work first
under these mutex.

Currently rtnl mutex was grabbed twice during fcoe_destroy call flow,
second grab was from fcoe_if_destroy called from fcoe_destroy after
dropping rtnl mutex before calling fcoe_if_destroy, so instead made
fcoe_if_destroy always called with rtnl mutex held to have this mutex
grabbed only once in this code path.

However left matching rtnl_unlock as-is in its original place as it was
dropped there for good reason since very next call causes synchronous
fip worker flush and if rtnl mutex is still held before flush
then that would cause new circular warning between fip->recv_work and
rtnl mutex, I've added detailed comment for this on fcoe_if_destroy
calling and rtnl muxtes unlocking.

Robert Love 7b2787ec15 [SCSI] libfc: Move the port_id into lport
This patch creates a port_id member in struct fc_lport.
This allows libfc to just deal with fc_lport instances
instead of calling into the fc_host to get the port_id.

This change helps in only using symbols necessary for
operation from the libfc structures. libfc still needs
to change the fc_host_port_id() if the port_id changes
so the presentation layer (scsi_transport_fc) can provide
the user with the correct value, but libfc shouldn't
rely on the presentation layer for operational values.

Vasu Dev 721cafafb6 [SCSI] fcoe: fixes wrong error exit in fcoe_create
fcoe_create exits using out_nodev label when module is not
yet LIVE but this exit path unlocks the rtnl_lock though
rtnl lock was not held in this case.

So this patch replaces out_nodev with out_nomod to exit
w/o unlocking rtnl_lock.

Joe Eykholt b3667f9177 [SCSI] libfc: set seq_id for incoming sequence
After the recent patch "fixes unnecessary seq id jump"
the SCST module fcst stopped working because multi-sequence
write data wasn't finding the sequence after the first frame.

Add back the setting of the seq_id when the first frame arrives.
Also fix indentation on two lines.

Lalit Chandivade 86fbee86e9 [SCSI] qla2xxx: ensure flash operation and host reset via sg_reset are mutually exclusive
The problem occurring is a hw-race where there's an outstanding read-flash
operation occurring while the chip is being reset (done via an sg_reset script).
After the chip is paused, the read-flash operation never completes and the
DPC thread, while trying to complete the reset, is never able to recover,
as the HW appears to be hung...

The fix is to wait for outstanding flash operation prior to doing a sg_reset -h.
And to wait for reset to complete before any flash operations.

Note, during the wait, if any of the operation (reset/flash) does not complete,
failure is returned to the upper layer. The upper layer either need to
fail or retry.

Madhuranath Iyengar 3822263eb1 [SCSI] qla2xxx: Support for asynchronous TM and Marker IOCBs.
Currently we can only issue the task management (TM)
commands via the mailbox mechanism. This is a limitation,
since only one mailbox command can be issued at a time.
The purpose of this effort is to provide support for
issuing and processing the respose to TM and Marker
IOCBs asynchronously. Towards achieving this, the
consolidated srb architecture that is currently used for
BSG and IOCB/Logio commands has been enhanced and used.

Migrate to a consistent set of debug entries during status-IOCB
handling:

* group CS_TIMEOUT handling with CS_PORT_UNAVAILABLE and the like
  (more regrouping of common behaviour).

* drop CS_DATA_OVERRUN handling as it now falls into the
  'default' case (still returns DID_ERROR).

* consolidate CS_RESET and CS_ABORTED handling, as we the only
  functional difference was a printk() (still returns DID_RESET).

* dropped all the earlier inconsistent [qla_]printk()s sprinkled
  throught the needlessly large case-statement.  Failure case
  I/Os are now logged with a 'standard' format:

	<command failure details>
	<command generic details>

  so, for example a dropped-frame is logged as:

	qla2xxx 0000:13:00.0: scsi(16:0:0) Dropped frame(s) detected (4000 of 4000 bytes).

Lalit Chandivade 3e46f03132 [SCSI] qla2xxx: Clear error status after uncorrectable non-fatal errors.
Currently error status is cleared only after the uncorrectable fatal errors
in the qla2xxx_pci_slot_reset. This fix is added to clear the error status in
qla2xxx_pci_resume. This way for both fatal and non-fatal errors the error
status gets cleared properly.

Dominik Brodowski 317b6d6300 pcmcia: dev_node removal (write-only drivers)
dev_node_t was only used to transport some minor/major numbers
from the PCMCIA device drivers to deprecated userspace helpers.
However, only a few drivers made use of it, and the userspace
helpers are deprecated anyways. Therefore, get rid of dev_node_t .

As a first step, remove any usage of dev_node_t from drivers which
only wrote to this typedef/struct, but did not make use of it.

Instead of the old pcmcia_request_irq() interface, drivers may now
choose between:

- calling request_irq/free_irq directly. Use the IRQ from *p_dev->irq.

- use pcmcia_request_irq(p_dev, handler_t); the PCMCIA core will
  clean up automatically on calls to pcmcia_disable_device() or
  device ejection.

- drivers still not capable of IRQF_SHARED (or not telling us so) may
  use the deprecated pcmcia_request_exclusive_irq() for the time
  being; they might receive a shared IRQ nonetheless.

James Bottomley 77a4229719 [SCSI] Retry commands with UNIT_ATTENTION sense codes to fix ext3/ext4 I/O error
There's nastyness in the way we currently handle barriers (and
discards): They're effectively filesystem commands, but they get
processed as BLOCK_PC commands.  Unfortunately BLOCK_PC commands are
taken by SCSI to be SG_IO commands and the issuer expects to see and
handle any returned errors, however trivial.  This leads to a huge
problem, because the block layer doesn't expect this to happen and any
trivially retryable error on a barrier causes an immediate I/O error
to the filesystem.

The only real way to hack around this is to take the usual class of
offending errors (unit attentions) and make them all retryable in the
case of a REQ_HARDBARRIER.  A correct fix would involve a rework of
the entire block and SCSI submit system, and so is out of scope for a
quick fix.

Hannes Reinecke c213e1407b [SCSI] Enable retries for SYNCRONIZE_CACHE commands to fix I/O error
Some arrays are giving I/O errors with ext3 filesystems when
SYNCHRONIZE_CACHE gets a UNIT_ATTENTION.  What is happening is that
these commands have no retries, so the UNIT_ATTENTION causes the
barrier to fail.  We should be enable retries here to clear any
transient error and allow the barrier to succeed.

Link Down -> Mark all devices missing
 Previously, the driver took no action on a Link Down,
 and waited for the I/O on a dead connection to timeout
 in the firmware before marking the DDB missing.

Link Up -> Mark all devices online
 F/W will do auto login to all the devices only once.
 After that its the responsibility of the driver to
 relogin to devices whenever there is :
        * Any sort of connection failure or
        * KATO expires indicating target has logged out or
        * I/O times out etc.

will not be cleaned up, leading to the iscsi transport being left
registered.  Fix this by adding a call to cxgb3i_iscsi_cleanup() on the

Douglas Gilbert 5447ed6c96 [SCSI] scsi_debug: virtual_gb ignores sector_size
In the scsi_debug driver, the virtual_gb option ignores the
sector_size, implicitly assuming that is 512 bytes.  So if
'virtual_gb=1 sector_size=4096' the result is an 8 GB (virtual) disk.

Dan Carpenter 2471b89406 [SCSI] pm8001: potential null dereference in pm8001_dev_gone_notify()
In the original code we dereferenced "pm8001_dev" before checking if it
was null.  This patch moves the dereference inside the condition.

This was found by a static checker (smatch).  I looked, but I couldn't
tell if "pm8001_dev" dev was ever actually null.  The approach in this
patch seemed like the safest response.

Brian King f5832fa2f8 [SCSI] ibmvfc: Fix command completion handling
Commands which are completed by the VIOS are placed on a CRQ
in kernel memory for the ibmvfc driver to process. Each CRQ
entry is 16 bytes. The ibmvfc driver reads the first 8 bytes
to check if the entry is valid, then reads the next 8 bytes to get
the handle, which is a pointer the completed command. This fixes
an issue seen on Power 7 where the processor reordered the
loads from memory, resulting in processing command completion
with a stale handle. This could result in command timeouts,
and also early completion of commands.

Herton Ronaldo Krzesinski 9a908c1aa4 [SCSI] advansys: fix narrow board error path
Error handling on advansys_board_found is fixed, because it's buggy in
the case we have an ASC_NARROW_BOARD set and failure happens on
AscInitAsc1000Driver step: it was freeing items of wrong struct in the
dvc_var union of struct asc_board, which could lead to an oops in the
case we set some of the fields in struct of narrow board as code was
choosing to always freeing wide board fields, and not everything was
being freed/released properly.

Print all world wide node names (node, port and fabric) with the same

Chris Leech ce8b5df042 [SCSI] libfc: set both precision and field with when printing FC IDs
Most of the prints of fabric IDs were specified as %6x, which will not
print any leading 0s.  It's nice to see leading 0s for identifiers
like this, which are a fixed length.  This patch sets the precision
modifier as well, making the specifier %6.6x, which forces the
printing of leading 0s.

Vasu Dev da87bfab8a [SCSI] fcoe, fnic, libfc: increased CDB size to 16 bytes for fcoe.
No reason to restrict CDB size to 12 bytes in fcoe, so
increased to 16 so that 16 bytes SCSI CDB doesn't fail.

Uses common define to set max_cmd_len for fcoe and fnic,
fnic is already setting max_cmd_len to 16.

sg_readcap -l fails without this fix.

fnic_flush_tx() is used to send frames held while fabric login
is in progress.  The frames are held in tx_queue, but
fnic_flush_tx() was incorrectly flushing from recv_queue which

The FIP controler state wasn't being reset on a disable.
A disable/enable sequence should be treated as a link event.
Otherwise, when using disable to mask a time when the link

enable.

Robert Love 593abc0720 [SCSI] libfcoe: Don't fill MAC desc in FLOGI if FIP negotiated FPMA
FPMA indicates that the Fabric will provide the host's
N_Port's MAC address. When sending a FLOGI/FDISC frame
and FPMA was negotiated through FIP discovery we still
need to provide the MAC descriptor, as per the
specification, but the MAC should be zero'd out since
the FCF will be providing it in the FLOGI/FDISC ACC.

In FC-BB-5 section 7.8.7.4.2 (Fabric login) it states:

The MAC address field in the MAC address descriptor of a FIP FLOGI
Request operation or a FIP NPIV FDISC Request operation shall contain:
a) the proposed MAC address to use as VN_Port MAC address if the ENode
   is requesting to use SPMA (see table 27);
b) all zeroes to indicate no MAC address is proposed if the ENode is
   requesting to use FPMA (see table 27); or
c) the proposed MAC address to use as VN_Port MAC address if the ENode
   supports both SPMA and FPMA and leaves the decision of which
   addressing scheme to use to the FCF (i.e., if both the FP and SP
   bits are set to one, see table 27).

This patch fixes case B.

This patch also adds debug statements to illustrate
whether a FPMA or SPMA MAC is added to a FLOGI/FDISC
frame.

Kashyap, Desai 130b958a5d [SCSI] mpt2sas: Reworked scmd->result priority for _scsih_qcmd.
we added support to set the deleted flag prior to device scan,
then clear the flag for responding devices, leaving the deleted flag only
set for missing devices.  The problem is for internal generated host resets,
IO queues are not blocked at scsi mid layer level.  IO will be continued
sent to driver, and driver  will return SCSI_MLQUEUE_HOST_BUSY.  The problem
is the driver checks for the deleted flag before it checks for the
controller being in reset, so there is a window where the driver would be
returning DID_NO_CONNECT for responding devices.  This occurs during  the
time between calling  _scsih_prep_device_scan, and
_scsih_mark_responding_sas_device & _scsih_mark_responding_raid_device.

Fix the queuecommand entry point so ioc->shost_recovery flag sanity check is
given higher presidence then the device "deleted flag" check.

Anil Veerabhadrappa 490475a993 [SCSI] bnx2i: Bug fixes related to MTU change issue when there are active iscsi sessions
bnx2i driver has to wait and cleanup all iscsi endpoints before
returning from bnx2i_stop(). This is to make sure all chip resources
are freed before chip is reset.

As the requirements for 1G and 10G chipsets is different, added
per-device 'hba_shutdown_tmo' parameter to adapter structure

If the connections are not torn down by the daemon within this timeout
period, 'cid's will be leaked in 10G device. 1G devices are more
flexible and do not leak any resources because the whole chip ports
gets reset when MTU is changed or ethtool selftest is run

fixed a minor issue in bnx2i_ep_poll() which unnecessarily forced
error return code when driver timed out waiting for TCP connect
request to complete

Kashyap, Desai 38c2911449 [SCSI] mpt2sas: sanity added to remove duplicate port from topology
There are few special cases which needs to be handled deleting old port.

CASE1: In topology you need cascaded expanders. Through sysfs just make sure
topology is up. Erase the manufacturing image of the cascaded expander and
reset the board. In some cases Adapter will receive Exapnder Add event
before expander delete. In such a case, driver needs to delete duplicate
port before adding new port.

CASE2: Enable Device Missing delay of HBA through lsiutils. If expander or
end device is hotswapped with different device before DMD timer expires,
driver will get device add for new device first and then device deletion
event for the original devices will arrive later at DMD timer expires. In
this case also driver need to delete duplicate port before adding port for
new device.

Added new function which will make sure when new port is
added, that its not claiming the same phy resources already in use by
another port. If it does, then it will delete the other port before adding
the new port.

Douglas Gilbert 78d4e5a07d [SCSI] scsi_debug: add max_queue + no_uld parameters
While testing the midlevel q_at_head and q_at_tail
patch for sg and the block SG_IO ioctl I found it
useful to reduce the queuing within the scsi_debug
driver. The reason is that the midlevel queue only
comes into play when the corresponding LLD queue
is full.

It is also useful when testing to be confident that
your program is the only thing issuing commands
to the (virtual) scsi_debug device. The no_uld=1
parameter will stop a scsi_debug virtual disk
appearing as /dev/sd* .

Changelog:
   - add max_queue parameter to reduce the number
     of queued commands the driver will accept.
     This parameter can be changed after the driver
     is loaded.
   - add no_uld parameter that restricts scsi_debug's
     virtual devices to the sg and bsg drivers
   - correct stale url

Hazard testing uncovered yet another bug in sd. Under heavy reset
activity the retry counter might be exhausted and the command will be
returned with sense UNIT_ATTENTION/0x29/00 (POWER ON, RESET, OR BUS
DEVICE RESET OCCURRED). In those cases we should just increase the
retry counter again, retrying one more to clear up this Unit Attention
state.

Mike Christie d7d05548a6 [SCSI] iscsi_tcp: fix relogin/shutdown hang
When I made this patch:
b64e77f70b
it was to solve a problem where we were already on the waitqueue
becuase a connection problem/logout caused us to be on there
when we were cleaning up the session. If we happen to get
on queue for more normal reasons like their just does not happen
to be any send space at the same time we are closing the connection
we hit a race and get stuck in the wait.

We should not check if the waitqueue is active
because we could race with the network code. If
the network xmit code is just about to enter the
prepare to wait when we check for the waitqueue to
be active then we will miss each other and the
network code will fall into the wait and we will
not run wake_up.

Kashyap, Desai 1469585309 [SCSI] mpt2sas : IOs needs to be pause until handles are refreshed for all device after recovery
After Host Reset firmware will have new list of device handles for the target.
Device handle refresh in driver is part of Rescan topology logic.
(See functions like *_search_responding_*). This needs to be done from Host
Reset context before making shost_recovery to 0. Currently it is done in
Firwmare event context, which may leads IO to a wrong device.

Now handler refresh is moved to HBA reset context.
Apart from this, Now driver will stop IOs for all device setting deleted
flag to 1 at the time of HBA Reset through _scsih_prep_device_scan.
It will only unblock devices, if devices has been found as part of RESCAN.
This way it will make more safe IO blocking at the time of HBA reset at
mpt2sas driver layer.

Hannes Reinecke f87146bba5 [SCSI] sd: quiet spurious error messages in READ_CAPACITY(16)
sd always tries to submit a READ_CAPACITY(16) CDB,
regardless whether the host actually supports it.
queuecommand() will then return DID_ABORT, which is
not qualified enough to detect the true cause here.
So better check in sd_try_rc16 first if the cdblen
is supported.

Anil Veerabhadrappa ce2d763a2c [SCSI] bnx2i: link bnx2i hba and cnic device before registering the device
When bnx2/cnic/bnx2i drivers are loaded in certain order, bnx2i will
will not initialize the device correctly because 'hba->cnic' will be
NULL when bnx2i_start() is called from register_device() context.
Under this condition 'ifdown' and 'ifup' of associated network
interface is required to bring iscsi adapter state to ready state so
that it will accept iscsi connection setup within the chip

Initializing 'hba->cnic' before calling register_device() will fix
this issue

Christof Schmitt 2f2eb58762 [SCSI] Allow FC LLD to fast-fail scsi eh by introducing new eh return
If the scsi eh is running and then a FC LLD calls
fc_remote_port_delete, the SCSI commands sent from the eh will fail.
To prevent this, a FC LLD can call fc_block_scsi_eh from the eh
callback, blocking the eh thread until the dev_loss_tmo fires or the
remote port is available again.

If (e.g. for a multipathing setup) the dev_loss_tmo is set to a very
large value, thus preventing the scsi device removal , the scsi eh can
block for a long time. For multipathing, the fast_io_fail_tmo is then
set to a low value to detect path problems sooner.

This patch introduces a new return code FAST_IO_FAIL. The function
fc_block_scsi_eh now returns FAST_IO_FAIL when the fast_io_fail_tmo
fires. This indicates that the LLD terminated all pending I/O requests
and there are no more pending SCSI commands for the scsi eh to wait
for. This return code can be passed back to the scsi eh to stop the
escalation and finish the recovery process for this device.

This patch (as1358) fixes a bug in the error pathway of
scsi_target_add().  If registration fails, the target should not be
reaped.  The reaping occurs later, when scanning is finished and all
the child devices are removed.  The current code leaves an unbalanced
value in starget->reap_ref.

Removed all the mutex's for ioc->tm_cmds.mutex, then created one
single mutex inside the function mpt2sas_scsih_issue_tm. This is the
single function used when sending task management. Also the sanity
checks required for scsi mid layer escalation were moved to inside the
same function because these checks need to be done while the mutex is
held. The ioc->tm_cmds.mutex inside the IOCTL branch is really not
required since there is another mutex in this code called for ctl_cmds
handling this sync.

milliseconds, which was due to the variable being long. To fix the problem,
the variable needs to be u64. Also the microseconds conversion to
milliseconds was incorrect; it should be divide by 1000 instead of divide by
8.

Joe Eykholt f018b73af6 [SCSI] libfc, libfcoe, fcoe: use smp_processor_id() only when preempt disabled
When the kernel is configured for preemption, using smp_processor_id()
when preemption is enabled causes a warning backtrace and is wrong
since we could move off of that CPU as soon as we get the ID,
and we would be referencing the wrong CPU, and possibly an invalid one
if it could be hotswapped out.

Remove the fc_lport_get_stats() function and explicitly use per_cpu_ptr()
to get the statistics.  Where preemption has been disabled by holding
a _bh lock continue to use smp_processor_id(), but otherwise use
get_cpu()/put_cpu().

In fcoe_recv_frame() also changed the cases where we return in the
middle to do a goto to the code which bumps ErrorFrames and does
a put_cpu().  Two of these cases didn't bump ErrorFrames before, but
doing so is harmless because they "can't happen", due to prior length
checks.

Also rearranged code in fcoe_recv_frame() to have only one call to
fc_exch_recv().  It's just as efficient and saves a call to put_cpu().

In fc_fcp.c, adjusted a FIXME comment for code which doesn't need fixing.

DID_ERROR cases can be ambigouos. Debugging FCP error cases
will be much easier if we have debug statements when we hit
these error conditions.

This patch simply adds debug messages using the FC_FCP_DBG
macro when we return DID_ERROR to SCSI. This way if a DID_ERROR
is reproducible turning on debug_logging will give a clue
to developers as to what the problem might be.

Vasu Dev 5c12c418e9 [SCSI] libfc: fix fcp pkt recovery in fc_fcp_recv_data
Currently fc_fcp_recv_data calls fc_fcp_retry_cmd to
retry failed IO but in this case tgt is still sending
data frames, therefore exchange needs to be aborted
first before initiating retry. So this patch fixes
this by aborting exchange first then have retry.

Renames fc_timeout_error to fc_fcp_recovery since
fc_timeout_error is already called from several other
places beside from fcp timeout handler and then
used fc_fcp_recovery for abort & retry from
fc_fcp_recv_data, this rename also required renaming
FC_CMD_TIME_OUT status to FC_CMD_RECOVERY to be
consistent with new fc_fcp_recovery.

Data frames are not expected for an DDPed exchange and
potentially it could be tampered data frame, so does
recovery in this case by calling fc_fcp_recovery.

Vasu Dev a104c84457 [SCSI] libfc: fixes unnecessary seq id jump
In some cases seq is incremented twice causing unnecessary
seq jump, for instance fc_exch_recv_seq_resp increments
seq id when fc_sof_is_init is true and that is true for
each incoming xfer ready but then fc_fcp_send_data does
another seq increment to send data for xfer ready.

This patch removes all such seq id jumps, at least it
eliminates few calls to fc_seq_start_next using ex_lock.

Also removes seq id update with incoming frame's seq id
as this is not needed since each end (I or T) just need
to send incremented their own seq id on each TSI from
other end & before sending new sequence within a
exchange.

The link and last_link fields in the fcoe_ctlr struct are no
longer useful, since they are always set to the same value,
and FIP always calls libfc to pass link information to the lport.

Eliminate those fields and rename link_work to timer_work, since

The method we've been using for point-to-point mode requires
that the LS_ACC for the FLOGI uses the D_ID and S_ID assigned
to the remote port and local port, not those in the exchange.

This is not the correct method, but for now, it's what works
with the old target, as well as with new targets based on libfc.

This patch changes the addresses used accordingly.

from received FLOGI requests to use as the destination MAC
for all outgoing frames.  We stopped doing that at some point.

and pass them to fcoe_ctlr_recv_flogi() so it can save the source MAC.

In point-to-point mode, if the PLOGI to the remote port times
out, it can get deleted by the remote port module.  Since there's
no reference by the local port, lport->ptp_data points to a freed
rport, and when the local port is reset and tries to logout again,
an oops occurs in mutex_lock_nested().

Hold a reference count on the point-to-point rdata.

Kashyap, Desai 1278b11f46 [SCSI] mpt2sas : Device removal algorithm in interrupt context only
external host not connecting after controller reboot: The
problem is : devices are not coming back after having the cable
disconnected then reconnected. The problem is because the
driver/firmware device removal handshake is failing. Due to this failure,
the controller firmware is not sending out device add events when the target
is reconnected. This is root caused to a race in the driver/firmware device
removal algorithm. There is duplicate code in both interrupt and user
context; where target reset is being issue from user context path while
sas_iounit_control(OP_REMOVE) is being sent from interrupt context. An
active target_reset will fail the OP_REMOVE. To fix this problem, the
duplicate code has been removed from user context path.

Add the cancel_pending_work flag from the fw_event_work structure, and then to
set the flag during host reset, check the flag later from work threads
context and if cancel_pending_work_flag is set ingore those events.

Now Rescan after host reset is changed.
Added special task MPT2SAS_RESCAN_AFTER_HOST_RESET. This task will be queued
at the time of HBA reset. this task is treated as barrier. All work after
MPT2SAS_RESCAN_AFTER_HOST_RESET will be treated as new work and will be
server by callback handle. If host_recovery is going on while running RESCAN
task, it will wait for shos_recovery_done completion which will be called
from HBA reset DONE context.

adpt_i2o_delete_hba() calls kfree() so we have to save "pHba->next"
before calling it.  Also inside adpt_i2o_delete_hba() itself, there
was another use after free bug which I fixed by moving the kfree()
down a line.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Michael Reed 421e33d004 [SCSI] qla1280: retain firmware for error recovery
The qla1280 driver acquires its firmware via udev.  During boot the firmware
is located in the initrd.  If, after root is mounted, the adapter needs to
reload firmware (host reset), the firmware load may fail if the root device
is on the adapter being reset.  This patch modifies qla1280 to retain the
firmware loaded via the initial request_firmware() for use during error
recovery.

Herton Ronaldo Krzesinski cf7474451c [SCSI] advansys: fix regression with request_firmware change
On newer kernels users of advansys module are reporting system hang when
trying to load it without firmware files present. After looking closely
at description on https://qa.mandriva.com/show_bug.cgi?id=53220, I think
this is related to commit "[SCSI] advansys: use request_firmware". The
problem is that after switch to request_firmware, asc_dvc->err_code
isn't being set when firmware files aren't found or loading fails.

err_code is used by the driver to judge if there was a fatal error or
not, as can be seen for example on advansys_board_found, which will only
return -ENODEV when err_code is set. Because err_code isn't being set
when request_firmware fails, this is a change of behaviour of the code
before request_firmware addition, making it continue to load and it
fails later as the firmware wasn't really loaded.

Mike Christie 4ae0a6c15e [SCSI] libiscsi: Fix recovery slowdown regression
We could be failing/stopping a connection due to libiscsi starting
recovery/cleanup, but the xmit path or scsi eh thread path
could be dropping the connection at the same time.

As a result the session->state gets set to failed instead of in
recovery. We end up not blocking the session
and so the replacement timeout never gets started and we only end up
failing the IO when scsi_softirq_done sees that the
cmd has been running for (cmd->allowed + 1) * rq->timeout secs.

We used to fail the IO right away so users are seeing a long
delay when using dm-multipath. This problem was added in
2.6.28.

Meelis Roos 77d3926306 qlogicpti: Remove slash in QlogicPTI irq name
qlogicpti driver registers its irq with a name containing slash.
This results in

Gal Rosen 0d9dc7c8b9 [SCSI] scsi_transport_fc: Fix synchronization issue while deleting vport
The issue occur while deleting 60 virtual ports through the sys
interface /sys/class/fc_vports/vport-X/vport_delete. It happen while in
a mistake each request sent twice for the same vport. This interface is
asynchronous, entering the delete request into a work queue, allowing
more than one request to enter to the delete work queue. The result is a
NULL pointer. The first request already delete the vport, while the
second request got a pointer to the vport before the device destroyed.
Re-create vport later cause system freeze.

Solution: Check vport flags before entering the request to the work queue.

Uwe Kleine-König 1b07193b34 platform-drivers: move probe to .devinit.text in drivers/scsi
A pointer to a probe callback is passed to the core via
platform_driver_register and so the function must not disappear when the
.init sections are discarded.  Otherwise (if also having HOTPLUG=y)
unbinding and binding a device to the driver via sysfs will result in an
oops as does a device being registered late.

An alternative to this patch is using platform_driver_probe instead of
platform_driver_register plus removing the pointer to the probe function
from the struct platform_driver.

bfad.c & bfad_drv.h:
  * Created a kernel thread from pci_probe that does the bfad start
    operations after BFA init done on a firmware mismatch.
  * The kernel thread on a fw mismatch waits for an event from IOC
    call back and is woken up from bfa_cb_init() on BFA init success.
  * In normal cases of no firmware mismatch this thread is terminated
    in pci_probe.

bfa_fcs_lport.c, fabric.c, fcs_lport.h & vport.c:
  * Split the lport init to attach time and init time code, so that
    proper config attributes are set after firmware mismatch.

bfa_iocfc.c:
  * Handle an IOC timer issue, where the IOC timer would expire before
    the init completion and send Init fail event to the driver,
    however IOC init continues and completes successfully at the later
    stage. The bfa and driver were not handling this kind of deferred
    init completion.

Krishna Gudipati a046bf0559 [SCSI] bfa: Fix to allow creation of only 190 vports on CNA.
Brocade CNA currently supports only 190 vports (instead of 191),
since there are only 192 unicast cam entries reserved for FCoE.

Brocade CNA has a total of 256 unicast cam entries (192 FCoE + 64 LL)

192 cam entries = 1 burned in mac + 1 baseport FPMA mac + 190 vport
FPMA macs.

Made changes to the code to support only 190 vports.

James Bottomley fac829fdca [SCSI] raid_attrs: fix dependency problems
RAID attributes uses scsi_is_sdev_device() to gate some SCSI specific
checking code.  This causes two problems.  Firstly if SCSI == n just
defining scsi_is_sdev_device() to return false might not be enough to
prevent gcc from emitting the code (and thus referring to undefined
symbols), so this needs surrounding with an ifdef.  Secondly, using
scsi_is_sdev_device() when SCSI is either y or m gives a subtle
problem in the m case: raid_attrs must also be m to use the symbol.
Do the usual Kconfig jiggery-pokery to fix this.

This uses the scatter-gather chaining feature of Smart Array
controllers.  32 scatter-gather elements are embedded in the
"command list", and the last element in the list may be marked
as a "chain pointer", and point to an additional block of
scatter gather elements.  The precise number of scatter gather
elements supported is dependent on the particular kind of
Smart Array, and is determined at runtime by querying the
hardware.

Mike Miller 31468401cc [SCSI] hpsa: remove scan thread
The intent of the scan thread was to allow a UNIT ATTENTION/LUN
DATA CHANGED condition encountered in the interrupt handler
to trigger a rescan of devices, which can't be done in interrupt
context.  However, we weren't able to get this to work, due to
multiple such UNIT ATTENTION conditions arriving during the rescan,
during updating of the SCSI mid layer, etc.  There's no way to tell
the devices, "stand still while I scan you!"  Since it doesn't work,
there's no point in having the thread, as the rescan triggered via
ioctl or sysfs can be done without such a thread.

Wayne Boyer d7b4627f5f [SCSI] ipr: adds PCI ID definitions for new adapters
This patch adds the PCI ID definitions for new adapters based on the next
generation 64 bit IOA PCI interface chip.  New entries have been added to the

Wayne Boyer f72919ec2b [SCSI] ipr: implement shutdown changes and remove obsolete write cache parameter
This patch adds a reboot notifier that will issue a shutdown prepare command
to all adapters.  This helps to prevent a problem where the primary adapter can
get shut down before the secondary adapter and cause the secondary adapter to
fail over and log and error.
This patch also removes the "enable_cache" paramater as it is obsolete.  Write
cache for an adapter is now controlled from the iprconfig utility.

that both 32 bit and 64 bit based adapters can work with the driver.

Rafael J. Wysocki 4cb077d93a PM: Allow SCSI devices to suspend/resume asynchronously
Set power.async_suspend for all SCSI devices, targets and hosts, so
that they can be suspended and resumed in parallel with the main
suspend/resume thread and possibly with other devices they don't
depend on in a known way (i.e. devices which are not their parents or
children).

The power.async_suspend flag is also set for devices that don't have
suspend or resume callbacks, because otherwise they would make the
main suspend/resume thread wait for their "asynchronous" children
(during suspend) or parents (during resume), effectively negating the
possible gains from executing these devices' suspend and resume
callbacks asynchronously.

Martin K. Petersen 086fa5ff08 block: Rename blk_queue_max_sectors to blk_queue_max_hw_sectors
The block layer calling convention is blk_queue_<limit name>.
blk_queue_max_sectors predates this practice, leading to some confusion.
Rename the function to appropriately reflect that its intended use is to
set max_hw_sectors.

Also introduce a temporary wrapper for backwards compability.  This can
be removed after the merge window is closed.

Martin K. Petersen 77c9cfc51b [SCSI] Fix printing of failed 32-byte commands
Having the large CDB allocation logic in sd.c means that
scsi_io_completion does not have access to the command buffer. That in
turn causes garbage to be printed when a 32-byte command fails. Move the
command printing to sd_done where the command buffer is intact.  Clear
the command buffer pointer after the extended CDB has been freed.

Make scsi_print_command ignore commands with NULL CDB pointers to
inhibit printing of garbled command strings.

Bart Van Assche 78d85019ba [SCSI] libsrp: fix bug in ADDITIONAL CDB LENGTH interpretation
Fix a bug in the interpretation of the ADDITIONAL CDB LENGTH (add_cdb_len)
field of SRP_CMD requests. According to the SRP specification, the layout
of this single-byte field is as follows:
* Bits 0 and 1 are reserved.
* Bits 2 to 7 represent the ADDITIONAL CDB LENGTH field, symbolically
  represented as n.
* Still according to the SRP specification, the ADDITIONAL CDB section
  takes 4*n bytes.

Currently libsrp is only used by the ibmvscsi driver. Since the ibmvscsi
driver doesn't support large CDB's, this bug hasn't caused any problems yet.

Andrew Vasquez 715848ca6f [SCSI] qla2xxx: Correct use-after-free issue in terminate_rport_io callback.
The explicit logout (LOGO) issued at the end of the callback will
flush (via normal scsi_cmnd->done()) any outstanding commands
(FCP2) the firmware is holding.  While iterating through the
outstanding_cmnd array in qla2x00_abort_fcport_cmds(), locking
and unlocking of the hardware spinlock, opens-up the driver to
cases where the processed SRB (sp) could be used after the
command completed from interrupt context.

FIP solication frames on a well-known multicast address.

allow the firmware interface to now register mcast addresses for fnic too.

Robert Love e9ee2cf438 [SCSI] fcoe: Only rmmod fcoe.ko if there are no active connections
Currently we're gracefully tearing down each active connection
when fcoe.ko is removed. We shouldn't allow the user to destroy
connections by removing the module. We should force the user to
destroy each connection and then the module can be removed.

This patch makes it so a refrerence count on the module is taken
each time a fcoe_interface is created. The reference count
is dropped when the fcoe_interface is destroyed. This makes it
so that module_exit() doesn't get called unless all fcoe_interfaces
have been destroyed.

This patch leaves the removal of interfaces in the module_exit
routine so that if the user does a 'rmmod -f' we'll clean everything
up before removing the module.

The module_put line was put before the out_putdev goto line because
we should only be decrementing the reference count if a
fcoe_interface is actually destroyed. If we can't find the netdev
or the fcoe_interface then it's assumed that something else has
destroyed the fcoe_interface and it would have decremented the
reference count at that time.

libfcoe module doesnt send port keep alive every
FIP_VN_KA_PERIOD due to improper assignment of timeout value.
Update the port_ka_time appropriately by incrementing it by
FIP_VN_KA_PERIOD in fcoe_ctlr_timeout(), so that the link_work
is scheduled to send the port LKA.

Vasu Dev be88d3b268 [SCSI] libfc: call ddp setup for only FCP reads to avoid accessing junk fsp pointer
Adds check to call fc_fcp_ddp_setup for only FCP read cmds to avoid
accessing junk fsp pointer at least in ESX since non FCP frame had
junk fsp value, though fsp is implicitly initialized to null
by __alloc_skb but with this patch no more relying on fsp
initialized to null value and hitting junk fsp ptr access.

Removes fsp pointer checking in fc_fcp_ddp_setup as this is not
needed any more since its only caller for FCP read will always
have a valid fsp.

If the nvram is invalid qla4xxx tries to set Asuint32_t
based on the card type. If the card type is not listed

Mike Christie 92ed4d6993 [SCSI] libiscsi: reset cmd timer if cmds are making progress
This patch resets the cmd timer if cmds started before
the timedout command are making progress. The idea is
that the cmd probably timed out because we are trying
to exeucte too many commands. If it turns out that the
device the IO timedout on was bad or the cmd just got
screwed up but other IO/devs were ok then we will
will figure this out when the cmds ahead of the timed
out one complete ok.

This also fixes a bug where we were sort of detecting
this by setting the last_timeout and last_xfer to the
same value when the task was allocated. That caught
the case where we never got to send any IO for it. However,
if the problem had started right before we started the
new task, then we were forced to wait an extra cmd
timeout seconds to start the scsi eh.

Mike Christie 9010b94636 [SCSI] cxgb3i: check for setup netdev
If the netdev has not been setup when the host is, we will oops when
the iscsi layer calls into the driver and a it tries to reference the
netdev in hba->ndev.

This can happen if the iscsi driver is loaded before ifup is
done. This patch just adds a check, so we can gracefully fail the
operation.

Stephen M. Cameron 2a8ccf3187 [SCSI] hpsa: fix bug in adjust_hpsa_scsi_table
fix bug in adjust_hpsa_scsi_table which caused devices which have
changed size, etc. to do the wrong thing.

The problem was as follows:

The driver maintains its current idea of what devices are present
in the h->dev[] array.  When it updates this array, it scans the
hardware, and produces a new list of devices, call it sd[], for
scsi devices.

Then, it compares each item in h->dev[] vs. sd[], and any items which
are not present sd it removes from h->dev[], and any items present
in sd[], but different, it modifies in h->dev[].

Then, it looks for items in sd[] which are not present in h->dev[],
and adds those items into h->dev[].  All the while, it keeps track
of what items were added and removed to/from h->dev[].

Finally, it updates the SCSI mid-layer by removing and adding
the same devices it removed and added to/from h->dev[]. (modified
devices count as a remove then add.)

originally, when a "changed" device was discovered, it was
removed then added to h->dev[].  The item was added to the *end*
of h->dev[].  And, the item was removed from sd[] as well
(nulled out).  As it processed h->dev[], these newly added items
at the end of the list were encountered, and sd[] was searched,
but those items were nulled out.  So they ended up getting removed
immediately after they were added.

The solution is to have a way to replace items in the h->dev[]
array instead of doing a remove + add.  Then the "changed" items.
are not encountered a second time, and removed.

Don Brace 303932fd4f [SCSI] hpsa: Allow multiple command completions per interrupt.
This is done by adding support for the so-called "performant mode"
(that's really what they called it).  Smart Array controllers
have a mode which enables multiple command completions to be
delivered with a single interrupt, "performant" mode.  We want to use
that mode, as some newer controllers will be requiring this mode.
