Yaniv Gardi 897efe628d scsi: ufs: add missing memory barriers
Performing several writes to UFS host controller registers has no
guarantee of ordering, so we must make sure register writes to setup
request list base address etc. are performed before the run/stop
register is enabled.  In addition, when setting up a task request, we
must make sure the updating of descriptors takes places before ringing
the doorbell, similarly to setting up a transfer request.

Yaniv Gardi d75f7fe495 scsi: ufs: reduce the interrupts for power mode change requests
DME commands such as Hibern8 enter/exit and gear switch generate 2
completion interrupts, one for confirmation that command is received by
local UniPro and 2nd one is the final confirmation after communication
with remote UniPro. Currently both of these completions are registered
as interrupt events which is not quite necessary and instead we can just
wait for the interrupt of 2nd completion, this should reduce the number
of interrupts and could reduce the unnecessary CPU wakeups to handle
extra interrupts.

Yaniv Gardi dc3c8d3a7d scsi: ufs: retry failed query flag requests
UFS flag query requests may fail sometimes due to timeouts etc.  Add a
wrapper function to retry up to 10 times in case of such failure,
similar to retries being made for attribute queries.

Yaniv Gardi 53c12d0ef6 scsi: ufs: fix error recovery after the hibern8 exit failure
Hibern8 exit can be called from 3 different contexts:
    - ufshcd_hibern8_exit_work
    - ufshcd_ungate_work
    - runtime/system resume

If hibern8 exit fails for some reason then we try to bring the link to
active state by link startup but this recovery mechanism results into
deadlock or errors from first 2 context listed above. This change fixes
the recovery by adding proper error handling mechanism.

Yaniv Gardi 87d0b4a6da scsi: ufs: add retries for hibern8 enter
If hibern8 enter command fails then UFS link state may be unknown which
may result into timeout of all the commands issued after failure.

This change does 2 things (for pre-defined number of retry counts) after
hibern8 enter failure:

1. Recovers the UFS link to active state

2. If link is recovered to active state, tries to put the UFS link in
   hibern8 enter again until retry count expires.

Yaniv Gardi 64238fbd15 scsi: ufs: add retries to dme_peer get and set attribute
The dme_peer get/set attribute commands are prone to errors, therefore
we add three retries for the UIC command sending.  Error code returned
from ufshcd_send_uic_cmd() is checked, and unless it was successful or
the retries have finished, another command will be sent.

Yaniv Gardi f05ac2e593 scsi: ufs: avoid exception event handler racing with PM callbacks
If device raises the exception event in the response to the commands
sent during the runtime/system PM callbacks, exception event handler
might run in parallel with PM callbacks and may see unclocked register
accesses. This change fixes this issue by not scheduling the exception
event handler while PM callbacks are running.

Yaniv Gardi e5ad406cec scsi: ufs: increase fDeviceInit query response timeout
fDeviceInit query response time for some devices is too long that
default query request timeout of 100ms may not be enough. Experiments
show that fDeviceInit response sometimes takes 500ms so to be on safer
side this change sets the timeout to 600ms. Without this change, we
might unnecessarily have to retry fDeviceInit query requests multiple
times and each query request timeout prints one error message.

Yaniv Gardi a48353f6d5 scsi: ufs: clear outstanding_request bit in case query timeout
When sending a query to the device returns with a timeout error, we
clear the corresponding bit in the DOORBELL register but we don't clear
the outstanding_request field as we should.  This patch fixes this bug.

Yaniv Gardi 14497328b6 scsi: ufs: verify command tag validity
A race condition appear to exist between request completion when
scsi_done() is called to end the request and set the tag back to -1 (at
blk_queue_end_tag() scsi_end_request), and scsi layer error handling
which aborts the command and reuses it to request sense data. Sending
the request sense is done with tag which was set to -1 and so it is
invalid.  Assert command tag passed from scsi layer is valid.

Yaniv Gardi 51047266f7 scsi: ufs: clear fields UTRD, UPIU req and rsp before new transfers
Some of the data structures (like response UPIU) and/or its elements
(unused fields) should be cleared before sending out the respective
command to UFS device.

This change clears the UPIU response data structure for query commands
and NOP command before sending out the command. We also initialize the
PRDT table length to zero which should take care of commands which
doesn't have any data associated with it. We are also clearing the
unused fields in request UPIU for NOP command.

Yaniv Gardi 52ac95fe06 scsi: ufs: clear UTRD, UPIU req and rsp before new transfers
Clear the UFS data structures before sending new request.

The SCSI command is sent to the device within the UFS UPIU request.
As part of the transfer UPIU preparation, the SCSI command is copied
to the UPIU structure according to the SCSI command size.
As different SCSI commands differ in size from each other, we need
to clear the whole SCSI command field to prevent sending uninitialized
data to the device.

The UPIU response doesn't always include the sense data and can differ
in size.
Hence, the UPIU response should also be cleared before the transfer.

Arnd Bergmann 0c994c03c9 scsi_dh: force modular build if SCSI is a module
When the scsi_dh core was moved into the scsi core module,
CONFIG_SCSI_DH became a 'bool' option, and now anything depending on it
can be built-in even when CONFIG_SCSI=m. This of course cannot link
successfully:

Unfortunately, I don't see any helpful way to add proper error handling
for this case, and the failure scenario for rd_reg seems rather obscure,

Arnd Bergmann 120f83f8d6 scsi: fdomain: drop fdomain_pci_tbl when built-in
The fdomain SCSI host driver is one of the last remaining drivers that
manually search the PCI bus using pci_get_device rather than registering
a pci_driver instance.

This means the module device table is unused when the driver is
built-in, and we get a warning about it:

Johannes Thumshirn 19db230736 lpfc: Remove redundant code block in lpfc_scsi_cmd_iocb_cmpl
This removes a redundant code block that will either be executed if the
ENABLE_FCP_RING_POLLING flag is set in phba->cfg_poll or not. The code
is just duplicated in both cases, hence we unify it again.

This probably is a left over from some sort of refactoring.

Suganath prabu Subramani ce61c57427 mpt3sas: Used IEEE SGL instead of MPI SGL while framing a SMP Passthrough request message.
As driver was using MPI SGL while framing the SMP Passthrough request
message due to which firmware unable to post the Reply Data in the host
memory and timeout is observed for this SMP Passthrough request message
and so unable to perform phy disable operation.

Kai Makisara 8038e6456a st: Fix MTMKPART to work with newer drives
Change the MTMKPART operation of the MTIOCTOP ioctl so that it works
also with current drives (LTO-5/6, etc.). Send a separate FORMAT MEDIUM
command if the partition mode page indicates that this is required. Use
LOAD to position the tape at the beginning of tape.

The operation is extended so that if the argument is negative, its
absolute value specifies the size of partition 0, which is the
physically first partition of the tape.

The hisi_sas driver is required to support both device tree and
ACPI. The scanning of the device properties now uses the Unified Device
Properties API, which serves both OF and ACPI.

Since syscon is not supported by ACPI, syscon is only used in the driver
when device tree is used.

With kernel change to split session lock in frwd_lock and back_lock for

Jitendra Bhivare 1868379be7 be2iscsi: Fix ExpStatSn in management tasks
Connection resets observed from some targets when NOP-Out with wrong
ExpStatSn is sent.

FW keeps track of StatSn and fills up ExpStatSn accordingly.  The header
filled up by the stack needs to be modified by driver to clear
ExpStatSn. If the field is not cleared, FW recalculates ExpStatSn and
wrong offset'ed ExpStatSn is seen in the wire trace.

Jitendra Bhivare 090e2184ba be2iscsi: Couple MCC tag and WRB alloc and free
WARN_ON(atomic_read(&mccq->used) >= mccq->len) seen when FW gets into
UE.

MCCQ overflow is happening because driver discards any new request and
frees up the tag. The tag allocation controls the number of MCC WRB
posted.  It is being replenished but WRBs are not hence the WARN_ON.

Allocation and freeing of WRB and tags for MCC is now done in one place.
This helps to achieve proper accounting of WRB indices and MCC tags.

During EEH PCI hotplug activity kernel unloads and loads the driver,
causing character device to be unregistered(aac_remove_one).When the
driver is loaded back using aac_probe_one the character device needs
to be registered again for the AIF management tools to work.

Fixed by adding code to register character device in aac_probe_one if
it is unregistered in aac_remove_one.

Raghava Aditya Renukunta ecc479e00d aacraid: Set correct msix count for EEH recovery
During EEH recovery number of online CPU's might change thereby changing
the number of MSIx vectors. Since each fib is allocated to a vector,
changes in the number of vectors causes fib to be sent thru invalid
vectors.In addition the correct number of MSIx vectors is not updated in
the INIT struct sent to the controller, when it is reinitialized.

Fixed by reassigning vectors to fibs based on the updated number of MSIx
vectors and updating the INIT structure before sending to controller.

aac_fib_map_free() calls pci_free_consistent() without checking that
dev->hw_fib_va is not NULL and dev->max_fib_size is not zero.If they are
indeed NULL/0, this will result in a hang as pci_free_consistent() will
attempt to invalidate cache for the entire 64-bit address space
(which would take a very long time).

Fixed by adding a check to make sure that dev->hw_fib_va and
dev->max_fib_size are not NULL and 0 respectively.

Raghava Aditya Renukunta 3f4ce057d5 aacraid: Fix RRQ overload
The driver utilizes an array of atomic variables to keep track of IO
submissions to each vector. To submit an IO multiple threads iterate
through the array to find a vector which has empty slots to send an
IO. The reading and updating of the variable is not atomic, causing race
conditions when a thread uses a full vector to submit an IO.

Fixed by mapping each FIB to a vector, the submission path then uses
said vector to submit IO thereby removing the possibly of a race
condition.The vector assignment is started from 1 since vector 0 is
reserved for the use of AIF management FIBS.If the number of MSIx
vectors is 1 (MSI or INTx mode) then all the fibs are allocated to
vector 0.

Raghava Aditya Renukunta 6bf3b630d0 aacraid: SCSI blk tag support
The method to allocate and free FIB's in the present code utilizes
spinlocks. Multiple IO's have to wait on the spinlock to acquire or free
fibs creating a performance bottleneck.

An alternative solution would be to use block layer tags to keep track
of the fibs allocated and freed. To this end aac_fib_alloc_tag was
created to utilize the blk layer tags to plug into the Fib pool.These
functions are used exclusively in the IO path. 8 fibs are reserved for
the use of AIF management software and utilize the previous spinlock
based implementations.

Add code to initialise the hardware.

Support is also added to deal with the "am-max-transmissions" (amt)
limitation in hip06 controller #1. This is how many connection requests
we can send on the system bus before waiting for a response.  Due to
chip bus design, controller #1 is limited to 32 amt, while, by design, a
controller supports 64.  The default value for the nibbles in the
relevant registers is 0x40; these need to be programmed with 0x20.

Since v2 hw only supports 2048 itct entries, as opposed to 4096 for v1
hw, set the max itct entries to the lower of the two.

It is not anticipated that any device with v1 will ever require to

Lee Duncan 126a4fe010 scsi: Use ida for host number management
Update the SCSI hosts module to use ida to manage its host_no index
instead of an atomic integer. This means that the SCSI host number will
now be reclaimable.

Use the ida "simple" mechanism, since there should be no need for a
separate spin lock current usage. Ida was chosen over idr because the
hosts module already has its own instance and locking mechanisms that
aren't easily changed.

This patch will add capability in driver to tell firmware that it can
throttle IOs in case controller's queue depth is downgraded post OFU
(online firmware upgrade). This feature will ensure firmware can be
downgraded from higher queue depth to lower queue depth without needing
system reboot. Added throttling code in IO path of driver, in case OS
tries to send more IOs than post OFU firmware's queue depth.

Sumit Saxena 308ec459bc megaraid_sas: Dual queue depth support
1. For iMR controllers, firmware will report two queue depths:

   - Controller-wide queue depth
   - LDIO queue depth (240)

Controller-wide queue depth will be greater among the two. Using this
new feature, iMR can provide larger Queue depth(QD) for JBOD and limited
QD for Virtual Disk(VD).

2. megaraid_sas driver will throttle read/write LDIOs based on "LDIO
Queue Depth".

3. Dual queue depth can be enabled/disabled via module parameter. It is
enabled by default if the firmware supports it. Only specific firmware
builds will enable the feature.

4. Added sysfs parameter "ldio_outstanding" which permits querying the
number of outstanding LDIO requests at runtime.

Sumit Saxena 179ac14291 megaraid_sas: Reply Descriptor Post Queue (RDPQ) support
This patch will create a reply queue pool for each MSI-X index and will
provide an array of base addresses instead of the single address of
legacy mode. Using this new interface the driver can support higher
queue depths through scattered DMA pools.

If array mode is not supported driver will fall back to the legacy
method of reply pool allocation. This limits controller queue depth to
1K max. To enable a queue depth of more than 1K driver requires firmware
to support array mode and scratch_pad3 will provide the new queue depth
value.

When RDPQ is used, downgrading to an older firmware release should not
be permitted. This may cause firmware fault and is not supported.

Sumit Saxena 18365b1385 megaraid_sas: Task management support
This patch adds task management for SCSI commands. Added functions are
task abort and target reset.

1. Currently, megaraid_sas driver performs controller reset when any IO
times out.  With task management support added, task abort and target
reset will be tried to recover timed out IO. If task management fails,
then controller reset will be performaned. If the task management
request times out, fail the request and escalate to the next
level (controller reset).

2. mr_device_priv_data will be allocated for all generations of
controller, but is_tm_capable flag will never be set for
controllers (prior to Invader series) as firmware support is not
available for task management.

3. Task management capable firmware will set is_tm_capable flag in
firmware API.

Sumit Saxena 6d40afbc7d megaraid_sas: MFI IO timeout handling
This patch will do proper error handling for DCMD timeout failure cases
for Fusion adapters:

1. For MFI adapters, in case of DCMD timeout (DCMD which must return
SUCCESS) driver will call kill adapter.

2. What action needs to be taken in case of DCMD timeout is decided by
function dcmd_timeout_ocr_possible().  DCMD timeout causing OCR is
applicable to the following commands:

	MR_DCMD_PD_LIST_QUERY
	MR_DCMD_LD_GET_LIST
	MR_DCMD_LD_LIST_QUERY
	MR_DCMD_CTRL_SET_CRASH_DUMP_PARAMS
	MR_DCMD_SYSTEM_PD_MAP_GET_INFO
	MR_DCMD_LD_MAP_GET_INFO

3. If DCMD fails from driver init path there are certain DCMDs which
must return SUCCESS. If those DCMDs fail, driver bails out. For optional
DCMDs like pd_info etc., driver continues without executing certain
functionality.

Sumit Saxena 11c71cb4ab megaraid_sas: Do not allow PCI access during OCR
This patch will do synhronization between OCR function and AEN function
using "reset_mutex" lock.  reset_mutex will be acquired only in the
first half of the AEN function which issues a DCMD. Second half of the
function which calls SCSI API (scsi_add_device/scsi_remove_device)
should be out of reset_mutex to avoid deadlock between scsi_eh thread
and driver.

During chip reset (inside OCR function), there should not be any PCI
access and AEN function (which is called in delayed context) may be
firing DCMDs (doing PCI writes) when chip reset is happening in parallel
which will cause FW fault. This patch will solve the problem by making
AEN thread and OCR thread mutually exclusive.

If we fail to queue an srb for an async login we should set the
relogin flag so it will be retried as the reason for the queuing
failure was most likely transient.  Failure to do this can lead to

Async link event provides port_speed info. Cache the port_speed info
and use the same to report in ISCSI_HOST_PARAM_PORT_SPEED query.

Jitendra Bhivare cdde6682ab be2iscsi: Fix to use atomic bit operations for tag_state
beiscsi_mccq_compl sets MCC_TAG_STATE_TIMEOUT before setting up
tag_mem_state. be_mcc_compl_process_isr checks for MCC_TAG_STATE_TIMEOUT
first then accesses tag_mem_state which might be still getting populated
in the process context.

Fix: Set MCC_TAG_STATE_TIMEOUT after tag_mem_state is populated.
Removed MCC_TAG_STATE_COMPLETED. When posted its in running state and
the running state is cleared in be_mcc_compl_process_isr.  be_mcc_notify
now takes tag argument to set it to running state.  Use bit operations
for tag_state. Use barriers before setting the state.

Jitendra Bhivare 6ac999efee be2iscsi: Fix soft lockup in mgmt_get_all_if_id path using bmbx
We are taking mbox_lock spinlock which disables pre-emption before we
poll for mbox completion. Waiting there with spinlock held in excess of
20s will cause soft lockup.

Actual fix is to change mbox_lock to mutex. The changes are done in
phases. This is the first part.

1. Changed mgmt_get_all_if_id to use MCC where after posting lock is
released.

2. Changed be_mbox_db_ready_wait to busy wait for 12s max and removed
wait_event_timeout. Added error handling code for IO reads.
OPCODE_COMMON_QUERY_FIRMWARE_CONFIG mbox command takes 8s time when
unreachable boot targets configured.

Mika Westerberg 5c6fab9d55 scsi: Drop runtime PM usage count after host is added
Runtime PM of the SCSI host is already handled by calls to
scsi_autopm_get_host() and scsi_autopm_put_host() from appropriate places
whenever the host needs to be powered on. This works fine when there is
device connected to the host as once it runtime suspends the host will too.

However, if there is no device connected the host is never runtime
suspended (the usage counter is always 0).

Allow runtime suspend of host even if it has no devices connected by
calling scsi_autopm_put_host() at the end of scsi_add_host_with_dma(). We
temporarily increase runtime PM usage counter first so call to
scsi_autopm_put_host() will result idle request to be scheduled for the
device.

Mika Westerberg 356fd2663c scsi: Set request queue runtime PM status back to active on resume
We treat system suspend of SCSI devices pretty much the same as runtime
suspend. If the device is already runtime suspended we leave it to that
state during system suspend. On resume from system sleep we then resume the
device and correct the runtime PM status back to "active".

There is a problem with this because runtime PM status of the request queue
in question is not changed (it will be in "suspended" state). When SCSI
disk driver (sd.c) resumes the disk it sends START message to the device
and because the request queue is still in "suspended" state
blk_pm_peek_request() returns NULL preventing resume of the disk.

The issue can be reproduced with following commands:

  # echo auto > /sys/block/sda/device/power/control
  # echo 15000 > /sys/block/sda/device/power/autosuspend_delay_ms

Now suspend the machine:

This ends up in soft lockup because resume is not proceeding accordingly
and userspace is never restarted. Also there is nothing printed to the
console.

Fix this by forcing request queue status to "active" before the disk is
resumed.

Dave Hansen d4edcf0d56 mm/gup: Switch all callers of get_user_pages() to not pass tsk/mm
We will soon modify the vanilla get_user_pages() so it can no
longer be used on mm/tasks other than 'current/current->mm',
which is by far the most common way it is called.  For now,
we allow the old-style calls, but warn when they are used.
(implemented in previous patch)

This patch switches all callers of:

	get_user_pages()
	get_user_pages_unlocked()
	get_user_pages_locked()

to stop passing tsk/mm so they will no longer see the warnings.

What appears to be happening is that something has pinned the target
so it can't go into STARGET_DEL via final release and the loop in
scsi_remove_target spins endlessly until that happens.

The fix for this soft lockup is to not keep looping over a device that
we've called remove on but which hasn't gone into DEL state.  This
patch will retain a simplistic memory of the last target and not keep
looping over it.

Quinn Tran cb43285ff7 qla2xxx: Fix stale pointer access.
[ Upstream Commit 84e32a06f4 ]

Commit 84e32a0 ("qla2xxx: Use pci_enable_msix_range() instead of
pci_enable_msix()") introduced a regression when target mode is enabled.
In qla24xx_enable_msix(), ha->max_rsp_queues was incorrectly set
to a value higher than the number of response queues allocated causing
an invalid dereference. Specifically here in qla2x00_init_rings():
    *rsp->in_ptr = 0;

Add additional check to make sure the pointer is valid. following
call stack will be seen

Bjorn Helgaas 952bbcb078 PCI: Remove includes of asm/pci-bridge.h
Drivers should include asm/pci-bridge.h only when they need the arch-
specific things provided there.  Outside of the arch/ directories, the only
drivers that actually need things provided by asm/pci-bridge.h are the
powerpc RPA hotplug drivers in drivers/pci/hotplug/rpa*.

Remove the includes of asm/pci-bridge.h from the other drivers, adding an
include of linux/pci.h if necessary.

Martin K. Petersen 0fb5b1fb30 block/sd: Return -EREMOTEIO when WRITE SAME and DISCARD are disabled
When a storage device rejects a WRITE SAME command we will disable write
same functionality for the device and return -EREMOTEIO to the block
layer. -EREMOTEIO will in turn prevent DM from retrying the I/O and/or
failing the path.

Yiwen Jiang discovered a small race where WRITE SAME requests issued
simultaneously would cause -EIO to be returned. This happened because
any requests being prepared after WRITE SAME had been disabled for the
device caused us to return BLKPREP_KILL. The latter caused the block
layer to return -EIO upon completion.

To overcome this we introduce BLKPREP_INVALID which indicates that this
is an invalid request for the device. blk_peek_request() is modified to
return -EREMOTEIO in that case.

Without in-depth understanding of SCSI layer and the Marvell controller,
I suspect this happens because when the link goes down (because of an
error) we schedule scsi_rescan_device() which again fails to read VPD
data... ad infinitum.

Since VPD data cannot be read from the device anyway we prevent the SCSI
layer from even trying by blacklisting the device. This gets away the
error and the system starts up normally.

Hannes Reinecke d2d06d4fe0 scsi_dh_rdac: always retry MODE SELECT on command lock violation
If MODE SELECT returns with sense '05/91/36' (command lock violation)
it should always be retried without counting the number of retries.
During an HBA upgrade or similar circumstances one might see a flood
of MODE SELECT command from various HBAs, which will easily trigger
the sense code and exceed the retry count.

Quinn Tran a07100e00a qla2xxx: Fix TMR ABORT interaction issue between qla2xxx and TCM
During lun reset, TMR thread from TCM would issue abort
to qla driver.  At abort time, each command is in different
state.  Depending on the state, qla will use the TMR thread
to trigger a command free(cmd_kref--) if command is not
down at firmware.

Alan Stern 13b4389143 SCSI: fix crashes in sd and sr runtime PM
Runtime suspend during driver probe and removal can cause problems.
The driver's runtime_suspend or runtime_resume callbacks may invoked
before the driver has finished binding to the device or after the
driver has unbound from the device.

This problem shows up with the sd and sr drivers, and can cause disk
or CD/DVD drives to become unusable as a result.  The fix is simple.
The drivers store a pointer to the scsi_disk or scsi_cd structure as
their private device data when probing is finished, so we simply have
to be sure to clear the private data during removal and test it during
runtime suspend/resume.

Nicholas Krause 7296f62f03 megaraid: Fix possible NULL pointer deference in mraid_mm_ioctl
This adds the needed check after the call to the function
mraid_mm_alloc_kioc in order to make sure that this function has not
returned NULL and therefore makes sure we do not deference a NULL
pointer if one is returned by mraid_mm_alloc_kioc.  Further more add
needed comments explaining that this function call can return NULL if
the list head is empty for the pointer passed in order to allow furture
users to understand this required pointer check.

Insu Yun d63c7dd5bc ipr: Fix out-of-bounds null overwrite
Return value of snprintf is not bound by size value, 2nd argument.
(https://www.kernel.org/doc/htmldocs/kernel-api/API-snprintf.html).
Return value is number of printed chars, can be larger than 2nd
argument.  Therefore, it can write null byte out of bounds ofbuffer.
Since snprintf puts null, it does not need to put additional null byte.

Mike Christie 9055082fb1 scsi: add Synology to 1024 sector blacklist
Another iscsi target that cannot handle large IOs, but does not tell us
a limit.

The Synology iSCSI targets report:

Block limits VPD page (SBC):

  Maximum compare and write length: 0 blocks
  Optimal transfer length granularity: 0 blocks
  Maximum transfer length: 0 blocks
  Optimal transfer length: 0 blocks

  Maximum unmap block descriptor count: 0
  Optimal unmap granularity: 0
  Unmap granularity alignment valid: 0
  Unmap granularity alignment: 0
  Maximum write same length: 0x0 blocks

and the size of the command it can handle seems to depend on how much
memory it can allocate at the time. This results in IO errors when
handling large IOs. This patch just has us use the old 1024 default
sectors for this target by adding it to the scsi blacklist. We do not
have good contacs with this vendors, so I have not been able to try and
fix on their side.

I have posted this a long while back, but it was not merged. This
version just fixes it up for merge/patch failures in the original
version.

Driver has following initialization sequence for Target mode

1. Driver initialization starts

2. ISP Abort is scheduled when the target is enabled.

qla2xxx [0000:04:00.0]-4807:25: ISP abort scheduled
qla2xxx [0000:04:00.0]-00af:25: Performing ISP error recovery - ha=ffff880caa9e0000.

3. DPC thread starts the ISP Abort

4. While DPC is resetting the chip and initializing the firmware, we get
   async events from the firmware about P2P mode, LOOP UP and PORT UPDATE.

5. PRLI from a initiator is delivered to us followed by a PLOGI and then a
   SCSI command which creates a session.

6. If the SCSI command is a WRITE in this case, we issue XFR RDY and it
   gets dropped as can be seen with messages RESET-XFR because ISP Abort
   is still active

qla2xxx [0000:04:00.0]-e902:25: RESET-XFR active/old-count/new-count = 1/1/1.

7. If the SCSI command is a READ, we issue RESPONSE and they get dropped
   as well because Abort is still active.

qla2xxx [0000:04:00.0]-e901:25: RESET-RSP active/old-count/new-count = 1/1/1

8. Now eventually, ISP Abort finishes clearing the DPC flags.

qla2xxx [0000:04:00.0]-8822:25: qla2x00_abort_isp succeeded.
qla2xxx [0000:04:00.0]-4808:25: ISP abort end.

9. Since we dropped SCSI commands silently (without any responses sent
   to the initiator) initiator waits for a SCSI timeout (which is 60
   seconds in our case), Sends an ABTS which fails since there
   no se_cmd found for the tag that ABTS is referencing as the
   commands were cleaned up in Step 6 and 7.

10. Initiator send an IO after the ABTS which succeed fine.

To fix the above case, the following changes have been made:
  - To prevent target from dropping commands silently, use the online flag
    instead to check for an active chip reset. Once the port is online during
    a chip reset phase, we are good to process the commands.
  - Clean up qla2x00_restart_isp to not set the online flag and process ATIO
    as it is unnecessary. During a chip reset, interrupts are enabled only
    after setting the online flag to 1, so ATIO's won't be missed and hence
    no need to process ATIO's after setting the online flag.

Quinn Tran 2f424b9b36 qla2xxx: Move atioq to a different lock to reduce lock contention
99% of the time the ATIOQ has SCSI command.  The other 1% of time
is something else.  Most of the time this interrupt does not need
to hold the hardware_lock.  We're moving the ATIO interrupt thread
to a different lock to reduce lock contention.

Quinn Tran 7560151b6b qla2xxx: Remove dependency on hardware_lock to reduce lock contention.
Sessions management (add, deleted, modify) currently are serialized
through the hardware_lock.  Hardware_lock is a high traffic lock.
This lock is accessed by both the transmit & receive sides.

Sessions management is now moved off to another lock call sess_lock.
This is done to reduce lock contention and increase traffic throughput.

Alexei Potashnik b7bd104e6f qla2xxx: Wait for all conflicts before ack'ing PLOGI
Until now ack'ing of a new PLOGI has only been delayed if there
was an existing session for the same WWN. Ack was released when
the session deletion completed.

If there was another WWN session with the same fc_id/loop_id pair
(aka "conflicting session"), PLOGI was still ack'ed immediately.
This potentially caused a problem when old session deletion logged
fc_id/loop_id out of FW after new session has been established.

Two work-arounds were attempted before:
1. Dropping PLOGIs until conflicting session goes away.
2. Detecting initiator being logged out of FW and issuing LOGO
to force re-login.

This patch introduces proper solution to the problem where PLOGI
is held until either existing session with same WWN or any
conflicting session goes away. Mechanism supports one session holding
two PLOGI acks as well as one PLOGI ack being held by many sessions.

Alexei Potashnik 71cdc07964 qla2xxx: Delete session if initiator is gone from FW
1. Initiator A is logged in with fc_id(1)/loop_id(1)
2. Initiator A re-logs in with fc_id(2)/loop_id(2)
3. Part of old session deletion async logoout for 1/1 is queued
4. Initiator B logs in with fc_id(1)/loop_id(1), starts
   passing data and creates session.
5. Async logo from 3 is processed by DPC and sent to FW

Now initiator B has the session but is logged out from FW.

This condition is detected first with CTIO error 29 at which
point we should delete current session. During session
deletion we will send LOGO to initiator to force re-login.

Under rare circumstances initiator might be logged out of FW,
not have driver session, but still think it's logged in.
E.g. the above sequence plus session deletion due to re-config.
Incoming commands will fail to create local session because
initiator is not found in FW. In this case we also issue LOGO
to initiator to force him re-login.

Finally this patch fixes exchange leak when commands where
received in logged out state. In this case loop_id must be
set to FFFF when corresponding exchange is terminated. The
patch modifies exchange termination to always use FFFF,
since in certain scenarios it's impossible to tell whether
command was received in logged in or logged out state.

Ondrej Zary c6084cbce4 ncr5380: Add support for HP C2502
HP C2502 cards (based on 53C400A chips) use different magic numbers for
software-based I/O address configuration than other cards.
The configuration is also extended to allow setting the IRQ.

Move the configuration to a new function magic_configure() and move
magic the magic numbers into an array. Add new magic numbers for these
HP cards and hp_c2502 module parameter to use them, e.g.:
modprobe g_NCR5380 ncr_irq=7 ncr_addr=0x280 hp_c2502=1

Tested with HP C2502 and DTCT-436P.

Ondrej Zary aeb51152bb ncr5380: Enable PDMA for DTC chips
Add I/O register mapping for DTC chips and enable PDMA mode.

These chips have 16-bit wide HOST BUFFER register and it must be read
by 16-bit accesses (we lose data otherwise).

Large PIO transfers crash at least the DTCT-436P chip (all reads result
in 0xFF) so this patch actually makes it work.

The chip also crashes when we bang on the C400 host status register too
heavily after PDMA write - a small udelay is needed.

Tested on DTCT-436P and verified that it does not break 53C400A.

Ondrej Zary f03946210d ncr5380: Fix pseudo DMA transfers on 53C400
Pseudo-DMA (PDMA) has been broken for ages, resulting in hangs on
53C400-based cards.

According to 53C400 datasheet, PDMA transfer length must be a multiple
of 128. Check if that's true and use PIO if it's not.

This makes PDMA work on 53C400 (Canon FG2-5202).
