Benjamin Herrenschmidt 7d43e57764 [POWERPC] Fix ppc32 single-stepping out of syscalls
The ppc32 kernel didn't properly set/clear the TIF_SINGLESTEP
flag, causing return from syscalls to not SIGTRAP, thus executing
one more instruction before stopping again.

This fixes it. The ptrace code is a bit of a mess, and is overdue
for at least a -proper- 32/64 bits split and possibly more cleanups
but this minimum fix should be ok for 2.6.22

Benjamin Herrenschmidt 832a791c58 [POWERPC] Fix powerpc vmlinux.lds.S
Sam's recent change in 7664709b44
broke things for us because we ended up with *(.text.*) before
*(.text), whereas previously *(.text) was first. This was
important because the start of the text section contains the
kernel entry point.

In fact, we don't need that *(.text.*) thing anymore and it
incorrectly matched .text.init.refok, thus putting it before
.text. .. ouch !

pmc.c has:

#ifndef MMCR0_PMA0

This one took a while to find. Unfortunately its the wrong define

Fix it simply for now, so that we can backport to stable.

Benjamin Herrenschmidt de821204db [POWERPC] Fix IO space on PCI buses created from of_platform
This changes the way of_platform_pci creates PCI host bridges such
that it uses request_phb_iospace() for mapping the IO ports, instead

will schmidt e147ec8f18 [POWERPC] Simplify smp_space_timers
Greatly simplify the function smp_space_timers.

The stolen time calculation (per comment within the code) doesn't need the
half-jiffy stagger any more. There isn't an issue with bouncing off global
locks, so we really shouldn't need any sort of staggering at all.

However, the last_jiffy value still needs to be set. This removes the
extra stagger logic, and just sets the values.

This change should benefit applications that rely on barrier
synchronization, and will help cut down OS jitter.

The difference with the __ versions is that it will update some per-processor

Roman Zippel f7e4217b00 rename thread_info to stack
This finally renames the thread_info field in task structure to stack, so that
the assumptions about this field are gone and archs have more freedom about
placing the thread_info structure.

Nonbroken archs which have a proper thread pointer can do the access to both
current thread and task structure via a single pointer.

It'll allow for a few more cleanups of the fork code, from which e.g. ia64
could benefit.

Rafael J. Wysocki 8bb7844286 Add suspend-related notifications for CPU hotplug
Since nonboot CPUs are now disabled after tasks and devices have been
frozen and the CPU hotplug infrastructure is used for this purpose, we need
special CPU hotplug notifications that will help the CPU-hotplug-aware
subsystems distinguish normal CPU hotplug events from CPU hotplug events
related to a system-wide suspend or resume operation in progress. This
patch introduces such notifications and causes them to be used during
suspend and resume transitions. It also changes all of the
CPU-hotplug-aware subsystems to take these notifications into consideration
(for now they are handled in the same way as the corresponding "normal"
ones).

Benjamin Herrenschmidt d0f13e3c20 [POWERPC] Introduce address space "slices"
The basic issue is to be able to do what hugetlbfs does but with
different page sizes for some other special filesystems; more
specifically, my need is:

 - Huge pages

 - SPE local store mappings using 64K pages on a 4K base page size
kernel on Cell

 - Some special 4K segments in 64K-page kernels for mapping a dodgy
type of powerpc-specific infiniband hardware that requires 4K MMU
mappings for various reasons I won't explain here.

The main issues are:

 - To maintain/keep track of the page size per "segment" (as we can
only have one page size per segment on powerpc, which are 256MB
divisions of the address space).

 - To make sure special mappings stay within their allotted
"segments" (including MAP_FIXED crap)

 - To make sure everybody else doesn't mmap/brk/grow_stack into a
"segment" that is used for a special mapping

Some of the necessary mechanisms to handle that were present in the
hugetlbfs code, but mostly in ways not suitable for anything else.

The patch relies on some changes to the generic get_unmapped_area()
that just got merged. It still hijacks hugetlb callbacks here or
there as the generic code hasn't been entirely cleaned up yet but
that shouldn't be a problem.

So what is a slice ? Well, I re-used the mechanism used formerly by our
hugetlbfs implementation which divides the address space in
"meta-segments" which I called "slices". The division is done using
256MB slices below 4G, and 1T slices above. Thus the address space is
divided currently into 16 "low" slices and 16 "high" slices. (Special
case: high slice 0 is the area between 4G and 1T).

Doing so simplifies significantly the tracking of segments and avoids
having to keep track of all the 256MB segments in the address space.

While I used the "concepts" of hugetlbfs, I mostly re-implemented
everything in a more generic way and "ported" hugetlbfs to it.

Slices can have an associated page size, which is encoded in the mmu
context and used by the SLB miss handler to set the segment sizes. The
hash code currently doesn't care, it has a specific check for hugepages,
though I might add a mechanism to provide per-slice hash mapping
functions in the future.

The slice code provide a pair of "generic" get_unmapped_area() (bottomup
and topdown) functions that should work with any slice size. There is
some trickiness here so I would appreciate people to have a look at the
implementation of these and let me know if I got something wrong.

Christoph Hellwig 1eeb66a1bb move die notifier handling to common code
This patch moves the die notifier handling to common code. Previous
various architectures had exactly the same code for it. Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at. avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

Gerd Hoffmann 69331af79c Fixes and cleanups for earlyprintk aka boot console
The console subsystem already has an idea of a boot console, using the
CON_BOOT flag. The implementation has some flaws though. The major
problem is that presence of a boot console makes register_console() ignore
any other console devices (unless explicitly specified on the kernel
command line).

This patch fixes the console selection code to *not* consider a boot
console a full-featured one, so the first non-boot console registering will
become the default console instead. This way the unregister call for the
boot console in the register_console() function actually triggers and the
handover from the boot console to the real console device works smoothly.
Added a printk for the handover, so you know which console device the
output goes to when the boot console stops printing messages.

The disable_early_printk() call is obsolete with that patch, explicitly
disabling the early console isn't needed any more as it works automagically
with that patch.

I've walked through the tree, dropped all disable_early_printk() instances
found below arch/ and tagged the consoles with CON_BOOT if needed. The
code is tested on x86, sh (thanks to Paul) and mips (thanks to Ralf).

Changes to last version: Rediffed against -rc3, adapted to mips cleanups by
Ralf, fixed "udbg-immortal" cmd line arg on powerpc.

This prepares for Ebony/440 support by creating an

misc_44x.S file, into which is moved the 44x specific reset code from
head_44x.S (on the grounds that we should really stop clogging up the
head_* files with random asm helper routines).

At the same time, we disable the (empty save Kconfig and Makefile)
arch/powerpc/platforms/4xx directory from the arch/powerpc/platforms
Makefile. Contrary to the comment in
arch/powerpc/platforms/4xx/Makefile, attempting to build such an empty

Paul Mackerras 11fbb00c67 [POWERPC] Cope with PCI host bridge I/O window not starting at 0
Currently our code to set up the data structures for a PCI host bridge
and create the mapping for its I/O window assumes that the window
starts at I/O port 0 on the PCI side. If this is not true, we can end
up with I/O port numbers in the resources for PCI devices which will
cause an oops if a driver tries to access them via inb/outb etc.,
because there is no mapping for the corresponding addresses.

Normally the I/O window starts at 0, but there are some situations on
partitioned machines with a hypervisor where the window may not start
at 0.

This fixes the problem by allocating space for the range from 0 to the
end of the I/O window. That is, hose->io_base_virt contains the
virtual address for I/O port 0 on the PCI bus, and thus the assumption
that hose->io_base_virt - pci_io_base is the offset between the
"global" I/O port numbers (those in the PCI device resources) and the
I/O port numbers on the PCI bus is maintained.

For PCI host bridges that are present at boot, we only map the portion
of that range that correspond to the bridge's I/O window. For bridges
added after boot we ioremap the range from 0 to the end of the I/O
window, for now; in fact hot-added bridges should be using
reserve_phb_iospace() and __ioremap_explicit (so they get sensible
global port numbers), but we don't have the infrastructure yet to do
that (basically a free_phb_iospace() routine plus appropriate
locking).

Interestingly, this makes the two arms of the if statement in
get_bus_io_range do almost exactly the same thing; that function could
now be simplified in a further patch.

David Gibson abb4a23907 serial: define FIXED_PORT flag for serial_core
At present, the serial core always allows setserial in userspace to change the
port address, irq and base clock of any serial port. That makes sense for
legacy ISA ports, but not for (say) embedded ns16550 compatible serial ports
at peculiar addresses. In these cases, the kernel code configuring the ports
must know exactly where they are, and their clocking arrangements (which can
be unusual on embedded boards). It doesn't make sense for userspace to change
these settings.

Therefore, this patch defines a UPF_FIXED_PORT flag for the uart_port
structure. If this flag is set when the serial port is configured, any
attempts to alter the port's type, io address, irq or base clock with
setserial are ignored.

In addition this patch uses the new flag for on-chip serial ports probed in
arch/powerpc/kernel/legacy_serial.c, and for other hard-wired serial ports
probed by drivers/serial/of_serial.c.

Johannes Berg be9c94dd77 [POWERPC] Fix suspend states again
In commit 0fba3a1f39 (a very long time ago,
May 2006), I fixed a bug that caused powermacs to crash when you tried
entering standby/mem suspend states.

As I'm now getting more familiar with the suspend code I notice a few
more things:
 1. we previously misunderstood what pm_ops is for, it isn't supposed to be
for doing platform dependent suspend/resume stuff that needs to be done
for suspend to disk (as we currently try to use it!), it is instead for
entering platform dependent suspend states ("standby", "mem").
 2. due to the first point, we never properly save FPU and altivec states
when suspending to disk. It probably hasn't hurt yet because the process
that writes the "disk" to /sys/power/state uses neither and its context
    is used.

This patch addresses these points as follows:
 1. remove all pm_ops from powermac, powermac suspend to ram isn't currently
usable via /sys/power/state but is done via the PMU instead.
 2. move the code responsible for storing FPU/altivec state into
    save_processor_state and the set_context() call to restore_processor_state.
 3. add a call to kernel_enable_spe()

It may look like there is some code removal missing but that is
actually because the new suspend.h file overrides the ppc/suspend.h
one which was previously used.

Joachim Fenkes 0727702a3a [POWERPC] ibmebus: change probe/remove interface from using loc-code to DT path
In some cases, multiple OFDT nodes might share the same location code, so
the location code is not a unique identifier for an OFDT node. Changed the
ibmebus probe/remove interface to use the DT path of the device node instead
of the location code.

The DT path must be written into probe/remove right as it would appear in
the "devspec" attribute of the ebus device: relative to the DT root, with a
leading slash and without a trailing slash. One trailing newline will not
hurt; multiple newlines will (like perl's chomp()).

Example:

Add a device "/proc/device-tree/foo@12345678" to ibmebus like this:
    echo /foo@12345678 > /sys/bus/ibmebus/probe

 Remove the device like this:

Benjamin Herrenschmidt a741e67969 [POWERPC] Make tlb flush batch use lazy MMU mode
The current tlb flush code on powerpc 64 bits has a subtle race since we
lost the page table lock due to the possible faulting in of new PTEs
after a previous one has been removed but before the corresponding hash
entry has been evicted, which can leads to all sort of fatal problems.

This patch reworks the batch code completely. It doesn't use the mmu_gather
stuff anymore. Instead, we use the lazy mmu hooks that were added by the
paravirt code. They have the nice property that the enter/leave lazy mmu
mode pair is always fully contained by the PTE lock for a given range
of PTEs. Thus we can guarantee that all batches are flushed on a given
CPU before it drops that lock.

We also generalize batching for any PTE update that require a flush.

Batching is now enabled on a CPU by arch_enter_lazy_mmu_mode() and
disabled by arch_leave_lazy_mmu_mode(). The code epects that this is
always contained within a PTE lock section so no preemption can happen
and no PTE insertion in that range from another CPU. When batching
is enabled on a CPU, every PTE updates that need a hash flush will
use the batch for that flush.

Jake Moilanen 569975591c [POWERPC] DMA 4GB boundary protection
There are many adapters which cannot handle DMAing across any 4 GB
boundary. For instance, the latest Emulex adapters.

This normally is not an issue as firmware gives dma-windows under
4gigs. However, some of the new System-P boxes have dma-windows above
4gigs, and this present a problem.

During initialization of the IOMMU tables, the last entry at each 4GB
boundary is marked as used. Thus no mappings can cross the boundary.
If a table ends at a 4GB boundary, the entry is not marked as used.

A boot option to remove this 4GB protection is given w/ protect4gb=off.
This exposes the potential issue for driver and hardware development
purposes.

Joachim Fenkes 6bccf755ff [POWERPC] ibmebus: dynamic addition/removal of adapters, some code cleanup
This adds two sysfs attributes to /sys/bus/ibmebus which can be used to
notify the ebus driver of added / removed ebus devices in the OF device
tree.

Echoing the device's location code (as found in the OFDT "ibm,loc-code"
property) into the "probe" attribute will notify ebus of addition of the
device and cause the appropriate device driver's probe function to be called
on the device.

Likewise, echoing the location code into the "remove" attribute will cause
the device to be removed from the system.

The writes will block until the respective operation has finished and return
an error code if the operation failed.

In addition, two minor tidbits are fixed:

- The fake root device used to provide a common parent for all ebus devices
is now based on device instead of of_device - it had no associated devtree
node. This saves several checks throughout the ebus driver.

- The sysfs attributes are now generated automagically by device_register()
instead of by the ibmebus code, which saves a few compiler warnings about
  unused return codes.

Jake Moilanen 618d3adc35 [POWERPC] DMA 4GB boundary protection
There are many adapters which can not handle DMAing acrosss any 4 GB

This normally is not an issue as firmware gives us dma-windows under

I propose fixing it in the IOMMU allocation instead of making each
driver protect against it as it is more efficient, and won't require
changing every driver which has not considered this issue.

This patch checks to see if the mapping spans a 4 gig boundary, and if
it does, retries the allocation. It tries the next allocation at the

David Gibson 30437b3e74 [POWERPC] Automatically lmb_reserve() initrd
At present, when an initrd is passed to the kernel used flat device
tree properties, the memory the initrd occupies must also be reserved
in the flat tree's reserve map, or the kernel may overwrite it. That
makes life more complicated than it could be for the bootwrapper.

This patch makes the kernel automatically reserve the initrd's space.
That in turn requires parsing the initrd parameters earlier than they
are currently, in early_init_dt_scan_chosen() instead of
check_for_initrd().

David Gibson eb6de28637 [POWERPC] Allow duplicate lmb_reserve() calls
At present calling lmb_reserve() (and hence lmb_add_region()) twice
for exactly the same memory region will cause strange behaviour.

This makes life difficult when booting from a flat device tree with
memory reserve map. Which regions are automatically reserved by the
kernel has changed over time, so it's quite possible a newer kernel
could attempt to auto-reserve a region which is also explicitly listed
in the device tree's reserve map, leading to trouble.

This patch avoids the problem by making lmb_reserve() ignore a call to
reserve a previously reserved region. It also removes a now redundant
test designed to avoid one specific case of the problem noted above.

At present, this patch deals only with duplicate reservations of an
identical region. Attempting to reserve two different, but
overlapping regions will still cause problems. I might post another
patch later dealing with this case, but I'm avoiding it now since it
is substantially more complicated to deal with, less likely to occur
and more likely to indicate a genuine bug elsewhere if it does occur.

Paul Mackerras bb72c481e9 [POWERPC] Harden validate_sp against stack corruption
If something has overflowed or corrupted the stack and causes an oops,
and we try to print a stack trace, that will call validate_sp, which
can itself cause an oops if the cpu field of the thread_info struct at
the bottom of the stack has been corrupted (if CONFIG_IRQSTACKS is
set). This makes debugging harder.

To avoid the second oops, this adds a check to make sure that the cpu
number is reasonable before using it to check whether the stack is on
the softirq or hardirq stack.

Eric W. Biederman f5f2b13129 [PATCH] msi: sanely support hardware level msi disabling
In some cases when we are not using msi we need a way to ensure that the
hardware does not have an msi capability enabled. Currently the code has been
calling disable_msi_mode to try and achieve that. However disable_msi_mode
has several other side effects and is only available when msi support is
compiled in so it isn't really appropriate.

Instead this patch implements pci_msi_off which disables all msi and msix
capabilities unconditionally with no additional side effects.

pci_disable_device was redundantly clearing the bus master enable flag and
clearing the msi enable bit. A device that is not allowed to perform bus
mastering operations cannot generate intx or msi interrupt messages as those
are essentially a special case of dma, and require bus mastering. So the call
in pci_disable_device to disable msi capabilities was redundant.

quirk_pcie_pxh also called disable_msi_mode and is updated to use pci_msi_off.

Richard Purdie 28ee086d5b backlight: Fix external uses of backlight internal semaphore
backlight_device->sem has a very specific use as documented in the
header file. The external users of this are using it for a different
reason, to serialise access to the update_status() method.

backlight users were supposed to implement their own internal
serialisation of update_status() if needed but everyone is doing
things differently and incorrectly. Therefore add a global mutex to
take care of serialisation for everyone, once and for all.

Locking for get_brightness remains optional since most users don't
need it.

Eric W. Biederman 0b4d414714 [PATCH] sysctl: remove insert_at_head from register_sysctl
The semantic effect of insert_at_head is that it would allow new registered
sysctl entries to override existing sysctl entries of the same name. Which is
pain for caching and the proc interface never implemented.

I have done an audit and discovered that none of the current users of
register_sysctl care as (excpet for directories) they do not register
duplicate sysctl entries.

So this patch simply removes the support for overriding existing entries in
the sys_sysctl interface since no one uses it or cares and it makes future
enhancments harder.

Paul Mackerras e56a6e20f3 [POWERPC] Clear RI bit in MSR before restoring r13 when returning to userspace
Some instruction tracing tools use the RI (recoverable interrupt) bit
in the MSR to indicate when it's safe to single-step. Currently we
clear RI after restoring r13 when returning to userspace. However,
if we single-step past the point where r13 is restored, we'll corrupt
r13 in the exception entry code and not restore it. This moves the
clearing of RI to just before r13 is restored so this doesn't happen.

Livio Soares 449d846dbc [POWERPC] Fix performance monitor exception
To the issue: some point during 2.6.20 development, Paul Mackerras
introduced the "lazy IRQ disabling" patch (very cool work, BTW).
In that patch, the performance monitor unit exception was marked as
"maskable", in the sense that if interrupts were soft-disabled, that
exception could be ignored. This broke my PowerPC profiling code.
The symptom that I see is that a varying number of interrupts
(from 0 to $n$, typically closer to 0) get delivered, when, in
reality, it should always be very close to $n$.

The issue stems from the way masking is being done. Masking in
this fashion seems to work well with the decrementer and external
interrupts, because they are raised again until "really" handled.
For the PMU, however, this does not apply (at least on my Xserver
machine with a 970FX processor). If the PMU exception is not handled,
it will _not_ be re-raised (at least on my machine). The documentation
states that the PMXE bit in MMCR0 is set to 0 when the PMU exception
is raised. However, software must re-set the bit to re-enable PMU
exceptions. If the exception is ignored (as currently) not only is
that interrupt lost, but because software does not re-set PMXE, the
PMU registers are "frozen" forever.

[This patch means that performance monitor exceptions are taken and
handled even if irqs are off, as long as some other interrupt hasn't
come along and caused interrupts to be hard-disabled. In this sense
the PMU exception becomes like an NMI. The oprofile code for most
powerpc processors does nothing that is unsafe in an NMI context, but
the Cell oprofile code does a spin_lock_irqsave. However, that turns
out to be OK because Cell doesn't actually use the performance
monitor exception; performance monitor interrupts come in as a
regular interrupt on Cell, so will be disabled when irqs are off.

Gautham R Shenoy b282b6f8a8 [PATCH] Change cpu_up and co from __devinit to __cpuinit

WARNING: vmlinux - Section mismatch: reference to .init.data: from
.text between '_cpu_up' (at offset 0xc0141c37) and 'cpu_up'

This is because cpu_up, _cpu_up and __cpu_up (in some architectures) are
defined as __devinit
AND
__cpu_up calls some __cpuinit functions.

Since __cpuinit would map to __init with this kind of a configuration,
we get a .text refering .init.data warning.

This patch solves the problem by converting all of __cpu_up, _cpu_up
and cpu_up from __devinit to __cpuinit. The approach is justified since
the callers of cpu_up are either dependent on CONFIG_HOTPLUG_CPU or
are of __init type.

Thus when CONFIG_HOTPLUG_CPU=y, all these cpu up functions would land up
in .text section, and when CONFIG_HOTPLUG_CPU=n, all these functions would
land up in .init section.

Paul Mackerras 1c9bb1a01a [POWERPC] Fix register save area alignment for swapcontext syscall
For 32-bit processes, the getcontext side of the swapcontext system
call (i.e. the saving of the context when the first argument is
non-NULL) has to set the ctx->uc_mcontext.uc_regs pointer to the place
where it saves the registers. Which it does, but it doesn't ensure
that the pointer is 16-byte aligned. 16-byte alignment is needed
because the Altivec/VMX registers are saved in there, and they need to
be on a 16-byte boundary.

There is an overall reduction of code, since module_32/64 duplicated several

Unfortunately there's no way to tell gcc that BUG won't return, so the BUG

Benjamin Herrenschmidt dae4828d66 [POWERPC] Fix irq routing on some 32-bit PowerMacs
The changes to use pci_read_irq_line() broke interrupt parsing
on some 32-bit powermacs (oops). The reason is a bit obscure.
The code to parse interrupts happens earlier now, during
pcibios_fixup() as the PCI bus is being probed. However, the
current implementation pci_device_to_OF_node() for 32-bit
powerpc relies, on machines like PowerMac which renumber PCI buses,
on a table called pci_OF_bus_map containing a map of bus numbers
between the kernel and the firmware which is setup only later.
Thus, it fails to match the device node. In addition, some of
Apple internal PCI devices lack a proper PCI_INTERRUPT_PIN, thus
preventing the fallback mapping code to work.

This patch fixes it by making pci_device_to_OF_node() 32-bit
implementation use a different algorithm that works without
using the pci_OF_bus_map thing (which I intend to deprecate
anyway). It's a bit slower but that function isn't called in
any hot path hopefully.

Paul Mackerras 0204568a08 [POWERPC] Support ibm,dynamic-reconfiguration-memory nodes
For PAPR partitions with large amounts of memory, the firmware has an
alternative, more compact representation for the information about the
memory in the partition and its NUMA associativity information. This
adds the code to the kernel to parse this alternative representation.

The other part of this patch is telling the firmware that we can
handle the alternative representation. There is however a subtlety
here, because the firmware will invoke a reboot if the memory
representation we request is different from the representation that
firmware is currently using. This is because firmware can't change
the representation on the fly. Further, some firmware versions used
on POWER5+ machines have a bug where this reboot leaves the machine
with an altered value of load-base, which will prevent any kernel
booting until it is reset to the normal value (0x4000). Because of
this bug, we do NOT set fake_elf.rpanote.new_mem_def = 1, and thus we
do not request the new representation on POWER5+ and earlier machines.
We do request the new representation on POWER6, which uses the
ibm,client-architecture-support call.

Kim Phillips aa42c69c67 [POWERPC] Add support for FP emulation for the e300c2 core
The e300c2 has no FPU. Its MSR[FP] is grounded to zero. If an attempt
is made to execute a floating point instruction (including floating-point
load, store, or move instructions), the e300c2 takes a floating-point
unavailable interrupt.

This patch adds support for FP emulation on the e300c2 by declaring a
new CPU_FTR_FP_TAKES_FPUNAVAIL, where FP unavail interrupts are
intercepted and redirected to the ProgramCheck exception path for
correct emulation handling.

(If we run out of CPU_FTR bits we could look to reclaim this bit by adding
support to test the cpu_user_features for PPC_FEATURE_HAS_FPU instead)

It adds a nop to the exception path for 32-bit processors with a FPU.

Benjamin Herrenschmidt 396a1a5832 [POWERPC] Fix mmap of PCI resource with hack for X
The powerpc version of pci_resource_to_user() and associated hooks
used by /proc/bus/pci and /sys/bus/pci mmap have been broken for some
time on machines that don't have a 1:1 mapping of devices (basically
on non-PowerMacs) and have PCI devices above 32 bits.

This attempts to fix it as well as possible.

The rule is supposed to be that pci_resource_to_user() always converts
the resources back into a BAR values since that's what the /proc
interface was supposed to deal with. However, for X to work on
platforms where PCI MMIO is not mapped 1:1, it became a habit of
platforms like powerpc to pass "fixed up" values there since X expects
to be able to use values from /proc/bus/pci/devices as offsets to mmap
of /dev/mem...

So we keep that contraption here, causing also /sys/*/resource to
expose fully absolute MMIO addresses instead of BAR values, which is
ugly, but should still work as long as those are only used to calculate
alignment within a page.

X is still broken when built 32 bits on machines where PCI MMIO can be
above 32-bit space unfortunately.

Masami Hiramatsu b4c6c34a53 [PATCH] kprobes: enable booster on the preemptible kernel
When we are unregistering a kprobe-booster, we can't release its
instruction buffer immediately on the preemptive kernel, because some
processes might be preempted on the buffer. The freeze_processes() and
thaw_processes() functions can clean most of processes up from the buffer.
There are still some non-frozen threads who have the PF_NOFREEZE flag. If
those threads are sleeping (not preempted) at the known place outside the
buffer, we can ensure safety of freeing.

However, the processing of this check routine takes a long time. So, this
patch introduces the garbage collection mechanism of insn_slot. It also
introduces the "dirty" flag to free_insn_slot because of efficiency.

The "clean" instruction slots (dirty flag is cleared) are released
immediately. But the "dirty" slots which are used by boosted kprobes, are
marked as garbages. collect_garbage_slots() will be invoked to release
"dirty" slots if there are more than INSNS_PER_PAGE garbage slots or if
there are no unused slots.

Paul Mackerras 974a76f513 [POWERPC] Distinguish POWER6 partition modes and tell userspace
This adds code to look at the properties firmware puts in the device
tree to determine what compatibility mode the partition is in on
POWER6 machines, and set the ELF aux vector AT_HWCAP and AT_PLATFORM
entries appropriately.

Specifically, we look at the cpu-version property in the cpu node(s).
If that contains a "logical" PVR value (of the form 0x0f00000x), we
call identify_cpu again with this PVR value. A value of 0x0f000001
indicates the partition is in POWER5+ compatibility mode, and a value
of 0x0f000002 indicates "POWER6 architected" mode, with various
extensions disabled. We also look for various other properties:

Maynard Johnson 18f2190d79 [POWERPC] cell: Add oprofile support
Add PPU event-based and cycle-based profiling support to Oprofile for Cell.

Oprofile is expected to collect data on all CPUs simultaneously.
However, there is one set of performance counters per node. There are
two hardware threads or virtual CPUs on each node. Hence, OProfile must
multiplex in time the performance counter collection on the two virtual
CPUs.

The multiplexing of the performance counters is done by a virtual
counter routine. Initially, the counters are configured to collect data
on the even CPUs in the system, one CPU per node. In order to capture
the PC for the virtual CPU when the performance counter interrupt occurs
(the specified number of events between samples has occurred), the even
processors are configured to handle the performance counter interrupts
for their node. The virtual counter routine is called via a kernel
timer after the virtual sample time. The routine stops the counters,
saves the current counts, loads the last counts for the other virtual
CPU on the node, sets interrupts to be handled by the other virtual CPU
and restarts the counters, the virtual timer routine is scheduled to run
again. The virtual sample time is kept relatively small to make sure
sampling occurs on both CPUs on the node with a relatively small
granularity. Whenever the counters overflow, the performance counter
interrupt is called to collect the PC for the CPU where data is being
collected.

The oprofile driver relies on a firmware RTAS call to setup the debug bus
to route the desired signals to the performance counter hardware to be
counted. The RTAS call must set the routing registers appropriately in
each of the islands to pass the signals down the debug bus as well as
routing the signals from a particular island onto the bus. There is a
second firmware RTAS call to reset the debug bus to the non pass thru
state when the counters are not in use.

Benjamin Herrenschmidt 4cb3cee03d [POWERPC] Allow hooking of PCI MMIO & PIO accessors on 64 bits
This patch reworks the way iSeries hooks on PCI IO operations (both MMIO
and PIO) and provides a generic way for other platforms to do so (we
have need to do that for various other platforms).

While reworking the IO ops, I ended up doing some spring cleaning in
io.h and eeh.h which I might want to split into 2 or 3 patches (among
others, eeh.h had a lot of useless stuff in it).

A side effect is that EEH for PIO should work now (it used to pass IO
ports down to the eeh address check functions which is bogus).

Also, new are MMIO "repeat" ops, which other archs like ARM already had,
and that we have too now: readsb, readsw, readsl, writesb, writesw,
writesl.

In the long run, I might also make EEH use the hooks instead
of wrapping at the toplevel, which would make things even cleaner and
relegate EEH completely in platforms/iseries, but we have to measure the
performance impact there (though it's really only on MMIO reads)

Since I also need to hook on ioremap, I shuffled the functions a bit
there. I introduced ioremap_flags() to use by drivers who want to pass
explicit flags to ioremap (and it can be hooked). The old __ioremap() is
still there as a low level and cannot be hooked, thus drivers who use it
should migrate unless they know they want the low level version.

The patch "arch provides generic iomap missing accessors" (should be
number 4 in this series) is a pre-requisite to provide full iomap
API support with this patch.

Benjamin Herrenschmidt 7eebde700f [POWERPC] Souped-up of_platform_device support
This patch first splits of_device.c and of_platform.c, the later containing
the bits relative to of_platform_device's. On the "breaks" side of things,
drivers uisng of_platform_device(s) need to include asm/of_platform.h now
and of_(un)register_driver is now of_(un)register_platform_driver.

In addition to a few utility functions to locate of_platform_device(s),
the main new addition is of_platform_bus_probe() which allows the platform
code to trigger an automatic creation of of_platform_devices for a whole
tree of devices.

The function acts based on the type of the various "parent" devices encountered
from a provided root, using either a default known list of bus types that can be
"probed" or a passed-in list. It will only register devices on busses matching
that list, which mean that typically, it will not register PCI devices, as
expected (since they will be picked up by the PCI layer).

This will be used by Cell platforms using 4xx-type IOs in the Axon bridge

Benjamin Herrenschmidt f90bb153b1 [POWERPC] Make pci_read_irq_line the default
This patch reworks the way IRQs are fixed up on PCI for arch powerpc.

It makes pci_read_irq_line() called by default in the PCI code for
devices that are probed, and add an optional per-device fixup in
ppc_md for platforms that really need to correct what they obtain
from pci_read_irq_line().

It also removes ppc_md.irq_bus_setup which was only used by pSeries
and should not be needed anymore.

I've also removed the pSeries s7a workaround as it can't work with
the current interrupt code anyway. I'm trying to get one of these
machines working so I can test a proper fix for that problem.

I also haven't updated the old-style fixup code from 85xx_cds.c
because it's actually buggy :) It assigns pci_dev->irq hard coded
numbers which is no good with the new IRQ mapping code. It should
at least use irq_create_mapping(NULL, hard_coded_number); and possibly
also set_irq_type() to set them as level low.

John Rose ae883cab94 [POWERPC] pseries: Force 4k update_flash block and list sizes
The enablement of 64k pages on pseries platforms exposed a bug in
the RTAS mechanism for updating firmware. RTAS assumes 4k for flash
block and list sizes, and use of any other sizes results in a failure,
even though PAPR does not specify any such requirement.

This patch changes the rtas_flash module to force the use of 4k memory
block and list sizes when preparing and sending a firmware image to
RTAS. The rtas_flash function now uses a slab cache of 4k blocks with
4k alignment, rather than get_zeroed_page(), to allocate the memory for
the flash blocks and lists. The 4k alignment requirement is specified
in PAPR.

Linas Vepstas 5d2efba64b [POWERPC] Use 4kB iommu pages even on 64kB-page systems
The 10Gigabit ethernet device drivers appear to be able to chew
up all 256MB of TCE mappings on pSeries systems, as evidenced by
numerous error messages:

 iommu_alloc failed, tbl c0000000010d5c48 vaddr c0000000d875eff0 npages 1

Some experimentation indicates that this is essentially because
one 1500 byte ethernet MTU gets mapped as a 64K DMA region when
the large 64K pages are enabled. Thus, it doesn't take much to
exhaust all of the available DMA mappings for a high-speed card.

This patch changes the iommu allocator to work with its own
unique, distinct page size. Although the patch is long, its
actually quite simple: it just #defines a distinct IOMMU_PAGE_SIZE
and then uses this in all the places that matter.

As a side effect, it also dramatically improves network performance
on platforms with H-calls on iommu translation inserts/removes (since
we no longer call it 16 times for a 1500 bytes packet when the iommu HW
is still 4k).

In the future, we might want to make the IOMMU_PAGE_SIZE a variable
in the iommu_table instance, thus allowing support for different HW
page sizes in the iommu itself.

all counters), so the code has been restructured to have
cpu_setup take the current counter config array as an argument,
  with op_powerpc_setup() invoking op_powerpc_cpu_setup() through
  on_each_cpu(), and op_powerpc_cpu_setup() invoking the
model-specific cpu_setup function with an argument. The
argument is ignored on all other platforms at present.

This patch fixes a few issues in offb:

 - A test was inverted causing the palette hack to never work
(no device node was passed down to the init function)

 - Some cards seem to have their assigned-addresses property in a random
order, thus we need to try using of_get_pci_address() first, which will
fail if it's not a PCI device, and fallback to of_get_address() in that
case. of_get_pci_address() properly parsees assigned-addresses to test
the BAR number and thus will get it right whatever the order is.

 - Some cards (like GXT4500) provide a linebytes of 0xffffffff in the
device-tree which does no good. This patch handles that by using the
screen width when that happens. (Also fixes btext.c while at it).

 - Add detection of the GXT4500 in addition to the GXT2000 for the

Benjamin Herrenschmidt 0909c8c2d5 [POWERPC] Support feature fixups in vdso's
This patch reworks the feature fixup mecanism so vdso's can be fixed up.
The main issue was that the construct:

.long label (or .llong on 64 bits)

will not work in the case of a shared library like the vdso. It will
generate an empty placeholder in the fixup table along with a reloc,
which is not something we can deal with in the vdso.

The idea here (thanks Alan Modra !) is to instead use something like:

1:
        .long label - 1b

That is, the feature fixup tables no longer contain addresses of bits of
code to patch, but offsets of such code from the fixup table entry
itself. That is properly resolved by ld when building the .so's. I've
modified the fixup mecanism generically to use that method for the rest
of the kernel as well.

Another trick is that the 32 bits vDSO included in the 64 bits kernel
need to have a table in the 64 bits format. However, gas does not
support 32 bits code with a statement of the form:

.llong label - 1b (Or even just .llong label)

That is, it cannot emit the right fixup/relocation for the linker to use
to assign a 32 bits address to an .llong field. Thus, in the specific
case of the 32 bits vdso built as part of the 64 bits kernel, we are
using a modified macro that generates:

        .long 0xffffffff
        .llong label - 1b

Note that is assumes that the value is negative which is enforced by
the .lds (those offsets are always negative as the .text is always
before the fixup table and gas doesn't support emiting the reloc the
other way around).

Benjamin Herrenschmidt 42c4aaadb7 [POWERPC] Consolidate feature fixup code
There are currently two versions of the functions for applying the
feature fixups, one for CPU features and one for firmware features. In
addition, they are both in assembly and with separate implementations
for 32 and 64 bits. identify_cpu() is also implemented in assembly and
separately for 32 and 64 bits.

This patch replaces them with a pair of C functions. The call sites are
slightly moved on ppc64 as well to be called from C instead of from
assembly, though it's a very small change, and thus shouldn't cause any
problem.

Stephen Rothwell cbcdb93d44 [POWERPC] Simplify stolen time calculation
In calculating stolen time, we were trying to actually account for time
spent in the hypervisor. We don't really have enough information to do
that accurately, so don't try. Instead, we now calculate stolen time as
time that the current cpu thread is not actually dispatching instructions.
On chips without a PURR, we cannot do this, so stolen time will always
be zero. On chips with a PURR, this is merely the difference between
the elapsed PURR values and the elapsed TB values.

This gives us much more sane vaules from tools such as mpstat, even if
they are still a bit strange e.g. 2 busy threads on one cpu will both
appear to have 50% user time and 50% stolen time while 1 busy thread on
a cpu will look like 100% user on one of them and 100% idle on the other.

Paul Mackerras d04c56f73c [POWERPC] Lazy interrupt disabling for 64-bit machines
This implements a lazy strategy for disabling interrupts. This means
that local_irq_disable() et al. just clear the 'interrupts are
enabled' flag in the paca. If an interrupt comes along, the interrupt
entry code notices that interrupts are supposed to be disabled, and
clears the EE bit in SRR1, clears the 'interrupts are hard-enabled'
flag in the paca, and returns. This means that interrupts only
actually get disabled in the processor when an interrupt comes along.

When interrupts are enabled by local_irq_enable() et al., the code
sets the interrupts-enabled flag in the paca, and then checks whether
interrupts got hard-disabled. If so, it also sets the EE bit in the
MSR to hard-enable the interrupts.

This has the potential to improve performance, and also makes it
easier to make a kernel that can boot on iSeries and on other 64-bit
machines, since this lazy-disable strategy is very similar to the
soft-disable strategy that iSeries already uses.

This version renames paca->proc_enabled to paca->soft_enabled, and
changes a couple of soft-disables in the kexec code to hard-disables,
which should fix the crash that Michael Ellerman saw. This doesn't
yet use a reserved CR field for the soft_enabled and hard_enabled
flags. This applies on top of Stephen Rothwell's patches to make it
possible to build a combined iSeries/other kernel.

Benjamin Herrenschmidt 41550c5128 [POWERPC] Don't get PCI IRQ from OF for devices with no IRQ
This patch adds checking of the PCI_INTERRUPT_PIN register before
using standard OF parsing to retreive PCI interrupts. The reason is
that some PCI devices may have no PCI interrupt, though they may have
interrupts attached via other means. In this case, we shall not use
irq->pdev, but device-specific code can later retreive those interrupts
instead.

Without that patch, Maple and derivatives don't get the right interrupt
for the second IDE channel as the linux IDE code fallsback to the PCI
irq instead of trying to use the legacy ones for the on-board controller
(which has no PCI_INTERRUPT_PIN). Having no PCI IRQ assign to it (as it
doesn't request any) fixes it.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
     the input_dev struct.

(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
     pointer or not.

(*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

David Howells afefdbb28a [PATCH] VFS: Make filldir_t and struct kstat deal in 64-bit inode numbers
These patches make the kernel pass 64-bit inode numbers internally when
communicating to userspace, even on a 32-bit system. They are required
because some filesystems have intrinsic 64-bit inode numbers: NFS3+ and XFS
for example. The 64-bit inode numbers are then propagated to userspace
automatically where the arch supports it.

Problems have been seen with userspace (eg: ld.so) using the 64-bit inode
number returned by stat64() or getdents64() to differentiate files, and
failing because the 64-bit inode number space was compressed to 32-bits, and
so overlaps occur.

This patch:

Make filldir_t take a 64-bit inode number and struct kstat carry a 64-bit
inode number so that 64-bit inode numbers can be passed back to userspace.

The stat functions then returns the full 64-bit inode number where
available and where possible. If it is not possible to represent the inode
number supplied by the filesystem in the field provided by userspace, then
error EOVERFLOW will be issued.

Similarly, the getdents/readdir functions now pass the full 64-bit inode
number to userspace where possible, returning EOVERFLOW instead when a
directory entry is encountered that can't be properly represented.

Note that this means that some inodes will not be stat'able on a 32-bit
system with old libraries where they were before - but it does mean that
there will be no ambiguity over what a 32-bit inode number refers to.

Note similarly that directory scans may be cut short with an error on a
32-bit system with old libraries where the scan would work before for the
same reasons.

It is judged unlikely that this situation will occur because modern glibc
uses 64-bit capable versions of stat and getdents class functions
exclusively, and that older systems are unlikely to encounter
unrepresentable inode numbers anyway.

Serge E. Hallyn 96b644bdec [PATCH] namespaces: utsname: use init_utsname when appropriate
In some places, particularly drivers and __init code, the init utsns is the
appropriate one to use. This patch replaces those with a the init_utsname
helper.

Changes: Removed several uses of init_utsname(). Hope I picked all the
right ones in net/ipv4/ipconfig.c. These are now changed to
utsname() (the per-process namespace utsname) in the previous
	patch (2/7)

This patch adds a nsproxy structure to the task struct. Later patches will
move the fs namespace pointer into this structure, and introduce a new utsname

The vserver and openvz functionality, then, would be implemented in large part
by virtualizing/isolating more and more resources into namespaces, each
contained in the nsproxy.

Atsushi Nemoto 8ef386092d [PATCH] kill wall_jiffies
With 2.6.18-rc4-mm2, now wall_jiffies will always be the same as jiffies.
So we can kill wall_jiffies completely.

This is just a cleanup and logically should not change any real behavior
except for one thing: RTC updating code in (old) ppc and xtensa use a
condition "jiffies - wall_jiffies == 1". This condition is never met so I
suppose it is just a bug. I just remove that condition only instead of
kill the whole "if" block.

Atsushi Nemoto 3171a0305d [PATCH] simplify update_times (avoid jiffies/jiffies_64 aliasing problem)
Pass ticks to do_timer() and update_times(), and adjust x86_64 and s390
timer interrupt handler with this change.

Currently update_times() calculates ticks by "jiffies - wall_jiffies", but
callers of do_timer() should know how many ticks to update. Passing ticks
get rid of this redundant calculation. Also there are another redundancy
pointed out by Martin Schwidefsky.

This cleanup make a barrier added by
5aee405c66 needless. So this patch removes
it.

As a bonus, this cleanup make wall_jiffies can be removed easily, since now
wall_jiffies is always synced with jiffies. (This patch does not really
remove wall_jiffies. It would be another cleanup patch)

Anton Blanchard 0ddd3e7d07 [POWERPC] Always call cede in pseries dedicated idle loop
The smt_snooze_delay logic changed a bit when the idle loops were
consolidated. A value of 0 used to mean we always polled, now it means
we always sleep. Instead of restoring the old behaviour, lets put a
reasonable default in smt_snooze_delay. This means we spin for a bit
(in case an external interrupt comes in) and then sleep.

Also the pseries dedicated idle loop currently does not cede both
threads in an SMT pair. The hypervisor wants us to call in so it can
power manage, so lets do that.

Michael Ellerman e12514650b [POWERPC] Fix loop logic in irq_alloc_virt()
There's a bug in irq_alloc_virt() if it's asked for more than 1 interrupt,
if it can't find a slot it might look past the end of the irq_map.
To be clear: the bug is that the continue affects the inner for loop,
not the outer one, so i becomes j + 1 and then we continue the inner
loop without checking if i is still <= limit.

This fixes it. No one in the kernel actually calls this with count >
1, so it's not critical.

David Woodhouse 94983cb788 [POWERPC] Fix PPC32 SECCOMP, unexport do_syscall_trace_{enter,leave}
The secure_computing() call which automatically aborts a process if it
tries to execute a syscall it shouldn't is much more useful if we
actually do it _before_ the syscall, rather than afterwards. PPC64 got
this right, but the original incorrect behaviour inherited from arch/ppc
was preserved by ifdefs. Make it the same on PPC32 too.

Also, I see no need to export do_syscall_trace_{leave,enter} on ppc32 --
they were only exported because the old do_syscall_trace() (which they

Paul Mackerras f007cacffc [POWERPC] Fix MMIO ops to provide expected barrier behaviour
This changes the writeX family of functions to have a sync instruction
before the MMIO store rather than after, because the generally expected
behaviour is that the device receiving the MMIO store can be guaranteed
to see the effects of any preceding writes to normal memory.

To preserve ordering between writeX and readX, and to preserve ordering
between preceding stores and the readX, the readX family of functions
have had an sync added before the load.

Although writeX followed by spin_unlock is not officially guaranteed
to keep the writeX inside the spin-locked region unless an mmiowb()
is used, there are currently drivers that depend on the previous
behaviour on powerpc, which was that the mmiowb wasn't actually required.
Therefore we have a per-cpu flag that is set by writeX, cleared by
__raw_spin_lock and mmiowb, and tested by __raw_spin_unlock. If it is
set, __raw_spin_unlock does a sync and clears it.

This changes both 32-bit and 64-bit readX/writeX. 32-bit already has a
sync in __raw_spin_unlock (since lwsync doesn't exist on 32-bit), and thus
doesn't need the per-cpu flag.

Tested on G5 (PPC970) and POWER5.

Paul Mackerras eb36c2884a [PATCH] ppc32: fix last_jiffy time comparison
This fixes a hang on ppc32.

The problem was that I was comparing a 32-bit quantity with a 64-bit
quantity, and consequently time wasn't advancing. This makes us use a
64-bit quantity on all platforms, which ends up simplifying the code
since we can now get rid of the tb_last_stamp variable (which actually
fixes another bug that Ben H and I noticed while going carefully through
the code).

This works fine on my G4 tibook. Let me know how it goes on your
machines.

Paul Mackerras 467c37801c [POWERPC] Fix irq enable/disable in smp_generic_take_timebase
Eran Ben-Avi <eranpublic@yahoo.com> pointed out that the arch/ppc version
of smp_generic_take_timebase disables interrupts on entry but exits without
restoring them. However, both it and the arch/powerpc version have another
problem, which is that they use local_irq_disable/enable rather than
local_irq_save/restore, and they are called with interrupts disabled.

This fixes both problems; it changes a return to a break in the arch/ppc
version, and changes both versions to use local_irq_save/restore.

Paul Mackerras e0d872d536 [POWERPC] Fix problem with time not advancing on 32-bit platforms
This fixes a problem introduced in 5db9fa9593.
The last_jiffy per-cpu variable is only 32 bits on 32-bit machines, but it
was being compared with a 64-bit quantity (tb_next_jiffy), which resulted in
time not advancing.

This fixes it by changing last_jiffy to be 64 bits on all platforms. With
this, we no longer need tb_last_stamp as a 32-bit version of tb_last_jiffy,
so this gets rid of tb_last_stamp and we just use tb_last_jiffy instead.
This also fixes a bug when the boot cpu is not online, because using
tb_last_stamp could have caused the wrong timebase origin value to be used
when calculating the time of day.

Benjamin Herrenschmidt 006b64de60 [POWERPC] Make OF irq map code detect more error cases
Device-tree bugs on js20 with some versions of SLOF were causing the
interrupt for IDE to not be parsed correctly and fail to boot. This
patch adds a bit more sanity checking to the parser to detect some of
those errors and fail instead of returning bogus information. The
powerpc PCI code can then trigger a fallback that works on those
machines.

Benjamin Herrenschmidt 8ec8f2e85c [POWERPC] Fix performance regression in IRQ radix tree locking
When reworking the powerpc irq code, I figured out that we were using
the radix tree in a racy way. As a temporary fix, I put a spinlock in
there. However, this can have a significant impact on performances. This
patch reworks that to use a smarter technique based on the fact that
what we need is in fact a rwlock with extremely rare writers (thus
optimized for the read path).

Olaf Hering 39ed2fe62c [POWERPC] reboot when panic_timout is set
Only call into RTAS when booted with panic=0 because the RTAS call
does not return. The system has to be rebooted via the HMC or via the
management console right now. This is cumbersome and not what the
default panic=180 is supposed to do.

Michael Ellerman 6f3d5d3cc4 [POWERPC] Add a helper for calculating RTAS "config_addr" parameters
Several RTAS calls take a "config_addr" parameter, which is a particular
way of specifying a PCI busno, devfn and register number into a 32-bit word.
Currently these are open-coded, and I'll be adding another soon, replace
them with a helper that encapsulates the logic. Be more strict about masking
the busno too, just in case.

Booted on P5 LPAR.
