Add reviewer name alias for reviewers to hide their actual name
...
It's used when exposing reviewer name to developers for reviewer
actions - regular actions still use the normal name, even when
made by a reviewer.

cookies instead of unnecessarily complicated through an API.

Make sure git-extraction can happen for disabled/deleted versions.

This implements a few cleanups along the way.
* version source extraction only happens explicitly from now on (e.g
during migration or upload)
* added a few tests for git extraction related tasks in versions.tasks

This should fix issues at submission time with add-ons that have
a particularly complex slug that causes the submission URL, which
we record as the referrer, to be > 255 chars.
